About this file

This is a Confidentiality Agreement form (SCVR 457, Revised 12/2018) issued by the South Carolina Vocational Rehabilitation Department (SCVRD) applicable to all employees, interns, practicum students, work-study students, and contractors working with or conducting business with the agency. The agreement establishes mandatory confidentiality protocols governing access to and handling of restricted and confidential information, including federal, state, and local protected data. Covered materials include medical and psychological records, personally identifiable information (PII), social security numbers, payroll documents, personnel files, consumer files, home addresses, telephone numbers, and all associated electronic communications. The agreement requires signatories to secure all confidential materials when not in active use, prohibits unauthorized reproduction or retention of copies, and mandates destruction of unnecessary materials through approved shredding procedures. Employees must assume all information is confidential when uncertain and restrict access to authorized personnel with legitimate work-related needs.

The agreement identifies specific conduct violations including careless discussion of consumer or employee information, unauthorized access to restricted data, sharing of information beyond authorized parties, disclosure through social media platforms, and improper disposal of confidential documents in unsecured waste. All individuals are required to report confidentiality breaches to SCVRD supervisors, the legal department, IT department, or Human Resources department. Signed acknowledgment of the agreement is maintained in personnel files, and the confidentiality standards must be observed during and after employment or business completion with the agency. The form includes signature and date lines for both the employee and an SCVRD designee to document receipt and discussion of the confidentiality requirements.
