About this file
This is a Required Affidavit for Bidders, Offerors and Contractors issued by the Commonwealth of Kentucky pursuant to KRS 45A.110 and 45A.115. The affidavit is effective for one year from the date of execution and must be completed and notarized by an authorized representative of any bidder, offeror, or contractor seeking to be awarded a contract or contract renewal with the Commonwealth of Kentucky. The document serves as an attestation that the contractor has not knowingly violated Kentucky's campaign finance laws and that accepting the contract award will not violate any provision of those laws. For purposes of the affidavit, "knowingly" is defined as awareness or reasonable awareness of a violation's existence.
The affidavit requires the authorized representative to sign and print their name, title, and date of execution before a notary public. The notarized document must include the bidder or offeror's legal name, complete address, Commonwealth of Kentucky vendor code (if available), the state and county where notarization occurred, the notary's signature, and the notary's commission expiration date. The Commonwealth retains the right to request an updated affidavit at any time during the contract period.
