Statement of Work TTHU/MHU - Haul and Install

1. Project Title

This Haul and Install Service is to support California Housing Mission DR-4610-CA.

2. General Effects of wildfires impacted the State of California causing widespread damage throughout large portions of the State. On August 24, 2021, the President signed a disaster declaration for the State of California authorizing FEMA assistance, including direct housing services, to impacted counties. All direct housing options are authorized under Section 408 of the Robert T. Stafford Disaster Relief and Emergency Assistance Act as amended (Stafford Act).

3. Period of Performance The period of performance (POP) shall be October 25, 2021 to November 24, 2021.

4. Areas of Operation The contractor may be tasked with performing TTHU/MHU -related activities in any DR-4610-CA Individual Assistance (IA) - declared DR-4610-CA, Plumas and Lassen County in California.

5. Scope:

The Hauling and Installing (H&I) contractor shall perform all work in accordance with the terms and conditions set forth in contract.

The H&I contractor shall be paid for actual work performed. However, there is no guarantee that all funds awarded under this Performance Order shall be paid unless the H&I contractor has been directed to perform work in accordance with the contract.

The H&I Contractor shall furnish all management, supervision, personnel, equipment, and material to perform services as defined in the Task.

All TTHUs units are located in Manteca (600 Spreckels Ave, Manteca, CA 95336) and all MHUs units are located in Sharpe Depot (700 E Roth Road, French Camp, CA

In general, the H&I contractor shall provide Site Inspections, TTHU/MHU Hauling, TTHU/MHU Installing, TTHU/MHU Maintenance, TTHU/MHU, for all TTHUs/MHUs assigned. The contractor shall be responsible for installing up to about 6 TTs and 2 MHUs. Sites will only include commercial and/or private. The contractor shall maintain these units until a new contract is awarded for maintenance and deactivation. Other services within the contract are applicable at the discretion of the Contracting Officer and Contracting Officer’s Representative in order to meet the mission requirements such as COVID-19 sanitizing as directed. The scope of work of the Purchase Order is limited. Only the available funded CLINS can be utilized.

6. Purchase Order Details

FEMA’s principal objective under this TO is to promptly provide direct housing services to impacted disaster survivors. The H&I Contractor shall accept up to 3 TTHU and H&I Work Orders (WOs) per day.

Task 1: All units will be hauled and installed to commercial and/or private site(s) Task 2: Contractor responsible for leveling each unit with leveling blocks as needed on each pad.

Task 3: Installation requirements shall include all utilities, stairs, UFAS ramps as needed.

Task 4: All units shall be tested and be fully mission capable, i.e. water hookup, drain hose, connections with black/grey water, and power supply connection at each 50 AMP.

Task 5: Contractor responsible for reactivating the pop-outs and disengaging once pop-outs are fully engaged.

Task 6: The contractor shall also provide up to three (3) COVID-19 treatments for each

TTHUs/MHUs: at RFO and deactivation, and possibly during FEMA applicant occupancy as directed. Proper documentation of each treatment for sanitation will be provided to the COR’s. At the discretion of the COR, the COR may task the contractor to sanitize the TTHUs/MHUs. The sanitizing agent shall effectively remove the COVID-19 virus.

Task 7: The Contractor shall Coordinate with all State, County, and Local officials to obtain any required permits and waivers, and ensure that all State, county, and local codes are adhered to.

Task 8: Preventive Maintenance Inspection and Repairs (includes minor repair labor and materials).

Task 9: The contractor shall provide Emergency Repair Labor: Labor for emergency repairs performed. The contractor shall make repairs to the extent there is no longer a safety hazard, life or death situation, or emergency issue. The Contractor shall respond and initiate the repair within two hours. Contractor shall notify the COR by the next day. shall notify the COR by the next day.

7. Deliverables

The Contractor shall:

Provide all of the material, labor, equipment, and services necessary for the mission.

Actively coordinate with the FEMA CO or COR.

Perform work and work order activities on time and notify COR of potential project constraints.

Ensure Contractor performance meets the requirements outlined in this SOW.

8. Contractor Safety:

a. The Contractor shall furnish all required safety equipment and/or clothing, which may be required for personnel as mandated by Federal, State, and Local code. This will include maintaining a safe working environment for the Contractor’s employees, unit occupants, FEMA staff, other Contractors and the general public. At no time shall the Contractor leave unattended or otherwise unprotected, any excavation or safety hazard that might cause injury to persons.

b. The Contractor shall comply with the Department of Labor and the Occupational Safety and Health Administration (OSHA) safety regulations, rules and requirements, as well as any Federal, State, County and Local laws, codes, and ordinances.

If applicable, the Contractor shall obtain all permits and licenses, as required, at the expense of the Contractor.

9. Mission Constraints FEMA anticipates an initial push of seven (6) TTHU and (2) MHUs WOs as commercial/private site(s) come under pad lease contract, with a potential of about or up to about thirty (30) days’ time before a next accumulation occurs based from FEMA pad lease contract awards.

All items that require Government intervention (utility company production capabilities, applicant qualifications, applicant issues including domestic, floodplain determinations, etc.) will be considered to be outside the contractor’s control, and incumbent on the Government for remedy.

The Contractor shall be responsible to immediately apprise the COR of any issues made known to the contractor that causes the contractor to be unable to, or potentially unable to perform within the requirement (i.e. timeline, etc.).
