Subject: Asbestos Containing Material Building Inspection Report Lake Sherwood Facilities 10454 Sherwood Lake Road White Sulphur Springs, Greenbrier County, West Virginia GEG Project Number: USF-2021-014

Dear Ms. Rice, Greenbrier Environmental Group, Inc. (GEG) has completed an asbestos containing material (ACM) inspection for the facilities located at the Lake Sherwood Recreational Area in White Sulphur Springs, Greenbrier County, West Virginia. The inspection provided information on the presence and/or absence of ACM at three restroom facilities at the Lake Sherwood Recreational Area in White Sulphur Springs, West Virginia. The inspection was performed on February 16, 2021, by Kinder Tuckwiller

(WV Asbestos Inspector License No. AI010555). The following sections of this report provide specific information concerning the inspection completed by GEG.

Description of Sampling Areas

The areas of concern for the ACM inspection completed at the above listed property were both the interior and exterior of the three restroom facilities. The survey areas consisted of three approximately 200 square foot (sq. ft.) structures. The one-story structures serve as restroom facilities for the nearby campsites and picnic areas. The exterior of the structures consisted of wood with asphalt shingle roofs. The interiors were comprised of wood and wallboard with a concrete floor. The inspector identified and collected samples of the following suspect building materials:

WB-01-01 through WB-01-02 – Wallboard – Pine Run Bathroom;

WB-02-03 through WB-02-04 – Wallboard – Cales Knoll Bathroom;

WB-03-05 through WB-03-06 – Wallboard – Picnic Shelter Bathroom;

RS-01-07 – Roof Shingle – Pine Run Bathroom;

RS-02-08 – Roof Shingle – Cales Knoll Bathroom; and, RS-03-09 – Roof Shingle – Picnic Shelter Bathroom.

Bulk samples of suspect materials were collected for purposes of assessing the materials for asbestos content.

Bulk Sampling and Analysis of Suspect Materials

A minimum of one (1) bulk sample was randomly collected of each suspect material identified within the survey areas. The samples were provided with a unique sample identification number and submitted under standard chain-of-custody to

Eurofins/CEI, a State of West Virginia licensed asbestos laboratory (license #LT000545), for analysis by polarized light microscopy (PLM). The analytical report is enclosed as Attachment I. A sample location site map is enclosed as Attachment II.

Laboratory Analytical Results

The laboratory analytical results indicate that asbestos is present in the samples collected from the wallboard present in all three of the bathroom facilities. Photographic documentation of confirmed ACM is enclosed as Attachment III. In total, nine

(9) samples were collected from six (6) identified suspect ACM. Federal and State of West Virginia regulations define ACM as a material containing greater than one (1) percent asbestos by weight.

Table 1 contains a list of the confirmed ACM identified at the three restroom facilities at the Lake Sherwood Recreational Area in White Sulphur Springs, West Virginia. The information provided in this table includes detailed ACM descriptions, friability

(ability to be reduced to powder by hand pressure), location, percent, and type of asbestos and approximate quantities. Table 2, Homogeneous Material List, provides a complete list of all materials sampled during the ACM inspection. Each sample material is grouped by sample ID, type, texture, and color.

ACM Assessment Conclusions

Based upon the asbestos inspection completed by GEG, asbestos containing materials are present in some of the building components sampled. The ACM inspection completed by GEG included all suspect materials which were readily accessible. If during demolition/renovation, suspect materials other than the materials sampled are encountered, the contractor should stop work and have the material sampled by a WV licensed asbestos building inspector.

LABORATORY ANALYTICAL REPORT

March 1, 2021

Greenbrier Environmental Group, Inc 292 Oak Carriage Drive Lewisburg, WV 24901

CLIENT PROJECT: Lake Sherwood

CEI LAB CODE: A211076

Dear Customer:

Enclosed are asbestos analysis results for PLM Bulk samples received at our laboratory on February 22, 2021. The samples were analyzed for asbestos using polarizing light microscopy (PLM) per the EPA 600 Method.

Sample results containing >1% asbestos are considered asbestos-containing materials (ACMs) per EPA regulatory requirements. The detection limit for the EPA 600 Method is <1% asbestos by weight as determined by visual estimation.

Thank you for your business and we look forward to continuing good relations.

Due to the limitations of the EPA 600 method, nonfriable organically bound materials (NOBs) such as vinyl floor tiles can be difficult to analyze via polarized light microscopy (PLM). EPA recommends that all NOBs analyzed by PLM, and found not to contain asbestos, be further analyzed by Transmission Electron Microscopy (TEM). Please note that PLM analysis of dust and soil samples for asbestos is not covered under NVLAP accreditation. Estimated measurement of uncertainty is available on request.
