STATEMENT OF WORK

PAINT REMOVAL AND ACRYLA TEC PAINT APPLICATION

ON

ANDERSEN AFB, GUAM

STATEMENT OF WORK

FOR

PAINT REMOVAL AND ACRYLA TEC PAINT APPLICATION

1. DESCRIPTION OF SERVICES. The Contractor shall provide all management, tools, supplies, equipment, parts and labor necessary to perform rust removal services and Acryla Tec paint application on AGE equipment and Expeditionary Small Asset Protection (ESAP

2) Facility Bldg. 20021 located on Andersen Air Force Base, Guam. The contractor shall accomplish all services below to meet the requirements of this Statement of Work (SOW).

Work shall be in compliance with all Federal and local laws, regulations, codes and license requirements for rust removal services and paint application.

1.1 BASIC SERVICES. The Contractor shall remove existing rust, resurface needed areas, and apply Acryla Tec Paint provided by the Government to three AGE equipment assets to include a mule, oil cart, and 10-ton jack and the ESAP 2 facility in accordance with the manufacturer’s recommendations and standard commercial practices. ESAP 2 unit exterior dimensions are 60 feet wide x 61 feet long x 33 feet high. Interior dimensions are 44 feet wide (3 bays) x 49 feet long x 14 feet high.

1.2 DISPOSAL OF WASTE. All waste will be disposed of at proper disposal site off Andersen Air Force Base in accordance with applicable federal and local pollution laws and regulations.

Contractor must obey all current Air Force policies and regulations. All waste disposals are the responsibility of the contractor to include any fees or permits.

1.3 LIABILITY. The contractor shall be liable for any damages resulting from any carelessness or failure to follow proper safety, installation, engineering, or maintenance procedures and standards.

2. SERVICE DELIVERY SUMMARY

Performance Objective SOW Para Performance Threshold Rust Removal Services and Paint application.

Remove existing rust. Resurface needed areas. All other rust removal services as intended. Acryla Tec Paint application as intended.

3. GOVERNMENT FURNISHED PROPERTY AND SERVICES. The government shall provide, without cost, the facilities, equipment, materials and/or services below.

3.1 Reasonable quantities of utilities shall be available to the contractor without charge. Any temporary lines and connections shall be made through existing outlets, and shall be maintained and removed by the contractor at his expense and in a timely manner satisfactory to the COR. The government will provide emergency medical transportation and treatment if required. The contractor shall reimburse the Government for the cost of these medical services at current rates.

4. GENERAL INFORMATION

4.1 Hours of Operation.

4.1.1 Normal Duty Hours. The normal hours of operation for the project are from 0730 to 1630, Monday through Friday, excluding holidays and PACAF Family days.

The contractor shall perform work required under this contract during the hours specified.

Normal duty hours are 7:30 am to 4:30 pm., Monday through Friday excluding holidays and PACAF Family days. The COR or CO must approve in advance, excluding legal holidays, any exceptions to work hours and designated workdays. Contractor must submit a “Request to Work Outside Normal Hours Worksheet, 3 days prior to any work commencing.

4.1.2 Federal Holidays. The Contractor is not required to provide service (except where otherwise specified in this contract, i.e. emergencies) on New Year’s Day, Martin Luther King’s Birthday, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day.

4.2 Contractor personnel. The contractor shall provide point of contact (POC) for all contractual matters during all hours of operation. The POC shall respond on-site within 1 hour during normal duty hours of operation and within 2 hours during non-duty hours to discuss concerns with government personnel designated by the CO to discuss problem areas. The Contractor shall maintain a current Employee List and submit a copy to both the CO and COR.

The list shall include employee’s name, supervisor, company, and level of security clearance.

4.3 Quality Control. Contractor shall develop and maintain a quality control program (QCP) to ensure test, inspections, cleaning, and disposal services. As a minimum, the Contractor shall develop quality control procedures that address the areas identified in Paragraph 2, Service Summary. The QCP shall demonstrate how the Contractor ensures quality performance during the contract period of performance. The Contractor shall maintain the QCP throughout the period of performance of the contract. The CO will be provided updates to the QCP as they occur during the period of performance. The QCP shall have the inspection forms and records which shall be used for the service. The Contractor shall identify to the CO/COR the responsible quality control inspector to notify in case of customer complaints.

4.4 Quality Assurance. The Government will periodically evaluate the Contractor’s performance IAW the Quality Assurance Surveillance Plan.

4.5 Security.

4.5.1 Passes and Badges. All Contractor employees shall obtain the required employee and vehicle passes. Each employee shall wear the Government issued badge over the front of the outer clothing. When an employee leaves the Contractor's service, the employee's Passes and Badges shall be returned within 10 calendar days.

4.5.2 Retrieving identification media. The contractor shall return all identification media and/or visitor passes from employees who depart for any reason before the contract expires, i.e., terminated for cause, reassignment, retirement, etc.

4.5.3 Traffic laws. The contractor and its employees shall comply with base traffic regulations. Traffic regulations include but are not limited to: Employees shall wear seatbelts when operating or riding in any vehicle (military, company or private owned) while on installation. Speed limits on Andersen AFB are 25 MPH unless otherwise posted. Be aware of reduced speed limits and exercise caution when driving in base housing and school zones. All vehicle operators on the installation shall not use cell phones unless the vehicle is safely parked, or they are using a 100% hands free device.

The wearing of any other portable headphones, earphones or other listening devices while operating a motor vehicle is prohibited.

4.5.4 Weapons, Firearms, and Ammunition. Employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within vehicles (military, contractor-owned, or privately owned) while on Andersen AFB.

4.5.5 For Official Use Only (FOUO). The contractor shall comply with DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.

4.5.6 Reporting Requirements. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified/unclassified defense information. Employees shall be briefed by their immediate supervisor upon initial on-base assignment.

4.5.7 Physical Security. The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment and materials shall be secured.

4.5.8 Controlled/Restricted Areas. The contractor shall implement local base procedures for entry to AF Controlled/Restricted areas where contractor personnel will work. An AF Form 2586, Unescorted Entry Authorization Certificate, must be completed and signed by 36 MXG security manager before restricted area badges (RAB) will be issued. Employees must have a completed and favorable NACI investigation before receiving a RAB. Interim access can be granted in accordance with AFI 31-501, Personnel Security Program Management.

4.5.9 Military Exercises. No compensation will be claimed for any delays resulting from re-entry procedures due to exercises.

4.6 Company Vehicle. The company name shall be displayed on each of the Contractor's vehicles in a manner and size that is clearly visible. All vehicles shall display a valid Guam license plate that complies with Guam Vehicle Code. Vehicles shall meet all other requirement of the Guam Vehicle Code, such as safety standards, and shall carry proof of insurance and Guam registration, if applicable. All vehicles shall be maintained with a clean and orderly appearance. Vehicles shall not be cleaned, washed, or rinsed off on Government property.

4.7 Access to Installation.

4.7.1 All Contractor personnel shall obtain access to the installation by participating in the Defense Biometric Identification System (DBIDS) or by obtaining passes each day from the Base Pass and Identification Office. Cost for obtaining passes through the DBIDS program is the responsibility of the contractor. One-day passes, issued through the Base Pass and Identification Office, will be furnished without charge.

4.7.2 The Contractor shall furnish a completed Employment Eligibility Verification (DHS FORM I-9) form for all personnel requesting badges. This form is available at http://www.uscis.gov/portal/site/uscis by searching or selecting Employment Verification (Form I-9). Immediately report instances of lost or stolen badges to the Contracting Officer. All Contractor personnel shall obtain access to the installation by obtaining passes from the Visitor Control Center in Andersen Air Force Base, Guam for Andersen Air Force Base, Guam.

4.8 Access to Buildings.

4.8.1 The Contractor shall comply with security requirements, plus those imposed by the installation Commander at all times. Personnel with access to special areas shall have the appropriate screening and/or security clearance, and personnel requiring routine access to restricted areas shall wear special badges authorizing access for those areas. Contractor personnel shall not enter restricted or controlled areas or installation facilities unless specifically authorized in performance of their duties. The Contractor shall secure all buildings and facilities entered during non-duty hours and shall secure all building and facilities under the Contractor’s cognizance at the end of each workday or shift period.

4.8.2 The Contractor shall make all arrangements through the appropriate office necessary to obtain access to buildings, facilities and other work areas, and when necessary, arrange for them to be opened and closed by the controlling authority. The Government may issue keys to the Contractor. The Contractor shall use due diligence and be responsible for compromised security systems to include replacement costs that result from its action or inaction.

4.8.3 The Contractor shall make arrangements for Government escort into secured areas requiring escort. The CO shall provide information on applicable buildings, spaces and the appropriate point of contact. The Contractor may experience delays while waiting for escorts. The Government estimates the wait period can be up to 30 minutes. The Contractor shall notify the Government COR and appropriate point of contract if an escort is not available after 30 minutes or access to accomplish the work is denied.

Unscheduled requirements, e.g., trouble calls, may require a longer wait for an escort.

4.9 Performance of Services during Crisis Declared by the National Command Authority or the Local Installation Commander. This service does not meet the criteria for continuation of performance during a crisis.

4.10 Environmental Control. The Contractor shall comply, and assure that all subcontractors comply, with all applicable federal, territory, and local laws, regulations, ordinances, policies and standards related to environmental matters. Where applicable, the Contractor shall use environmentally safe products in the course of completion of their project. The Contractor shall maintain, in company vehicle on site, Safety Data Sheets (SDS) for all chemicals. A copy of all SDS shall be provided to the CO and COR. The SDS data is subject to random checks by the government. Contractor shall maintain, and provide on demand, an inventory of materials being brought on the Government facility. The Contractor shall complete, and provide on demand, inventories of HAZMAT used, including but not limited to: solvents, paints, degreasers, greases, Ozone Depleting Substances, and oils brought on to the Government facility. If the Contractor spills or releases any HAZMAT or other substance contained in 40 CFR 302 into the environment, the Contractor shall immediately notify the CO and COR and appropriate emergency responders. The Contractor is responsible for all costs associated with clean-up and restoration, including any applicable fines and/or penalties. The Contractor shall maintain a spill plan as required by federal, territory, and local laws and regulations.

4.11 Schedule Coordination. The Contractor shall be responsible for coordinating all phases of his/her operations with the appropriate base personnel through the CO/COR. The contractor shall submit a detail schedule that includes locations, service time, and POC 15 days prior to service.

4.12 Special Qualifications. The Contractor shall be licensed by the territory in which the work is conducted to provide the services specified in this contract. All work shall be performed by qualified personnel who are trained and qualified for the systems and equipment. Contractor personnel shall be certified by appropriate federal and territory regulatory agencies to meet federal and local certification requirements. Copies of all certification/licensing documentation shall be provided to the COR.

5. WARRANTY.

5.1. Contractor shall guarantee all workmanship and labor from defects for a period of 12 months from date of acceptance of the work.
