Attachment 2 – QASP

QUALITY ASSURANCE SURVEILLANCE PLAN

For Contract Peace Corp STRAT Program Contract Number {enter contract number} Contractor’s Name {School Name}

1. PURPOSE

This Quality Assurance Surveillance Plan (QASP) provides a systematic method to evaluate performance for the stated contract. This QASP explains the following:

a. What will be monitored?

b. How monitoring will take place?

c. Who will conduct the monitoring?

d. How monitoring efforts and results will be documented?

The QASP is created with the premise that the contractor is responsible for management and quality control actions to meet the terms of the contract. It is the Government’s responsibility to be objective, fair, and consistent in evaluating performance. In addition, the QASP should recognize that unforeseen and uncontrollable situations may occur.

This QASP is a “living document” and the Government may review and revise it on a regular basis. However, the Government shall coordinate changes with the contractor. Updates shall ensure that the QASP remains a valid, useful, and enforceable document. Copies of the original QASP and revisions shall be provided to the contractor.

2. GOVERNMENT ROLES AND RESPONSIBILITIES

The following personnel shall oversee and coordinate surveillance activities.

a. Contracting Officer (CO) - The CO shall ensure performance of all necessary actions for effective contracting, ensure compliance with the contract terms, and shall safeguard the interests of the United States in the contractual relationship. The CO shall also assure that the contractor receives impartial, fair, and equitable treatment under this contract. The CO is ultimately responsible for the final determination of the adequacy of the contractor’s performance.

b. Contracting Officer’s Representative (COR) - The COR is responsible for technical administration of the contract and shall assure proper Government surveillance of the contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. The contractor shall refer any changes they deem may affect contract price, terms, or conditions to the CO for action.

3. PERFORMANCE MEASUREMENTS

a. Strat Recruiter is hired and is working on average 20 hours per week. (The number of hours clarifies the definition of part-time).

NOTE: If a Strat Recruiter is not hired, hired late in the contract work year, or leaves before the end of the contractual work year, the University shall only bill Peace Corps and Peace Corps will only pay for the time period a Strat has been working. Payment will be pro-rated based on the following Payment Formula:

· # of weeks in a Year that the School has committed to having a Strat work per the contract DIVIDED by the overall Annual Contract $$’s= $$’s per week.

· Payment will be pro-rated based on # of weeks a Strat is hired and working.

· Full Payment= # Weeks Committed in Contract (same as # of Weeks Worked) x $$’s per week

· Partial Payment= # of Weeks Worked x $$’s per week.

b. Office space and supporting material/resources as detailed in the Contract are provided by the school to the Strat.

c. Involvement of Peace Corps in the Strat interviewing and selection process

d. Strat Recruiter has:

· Taken the mandatory Strat Training(s)

· Submitted an Annual Recruitment Plan in the format and by the deadline provided by Peace Corps, reviewed and approved by Peace Corps Recruiter

· Submitted Monthly Recruitment Dashboard in format and by the deadline provided by Peace Corps

· Submitted a Mid-Year Report in format and by the deadline provided by Peace Corps

· Submitted a Final Year-End Report in format and by the deadline provided by Peace Corps

e. Quality of the approved/final Annual Recruitment Plan

· During the mandatory Strat Training, Peace Corps will provide guidance on activities and deliverables to include in the Annual Recruitment Plan along with targets for leads and applications. This guidance will include:

i. # and type of Recruitment Events

ii. # and type of Influencer and Stakeholder Meetings

iii. # of office hours

iv. Target: # of Leads

v. Target: # of Applications

f. Progress towards execution of the Approved Annual Recruitment Plan

4. METHODS OF QUALITY ASSURANCE SURVEILLANCE

Various methods exist to monitor performance. The COR shall use the surveillance methods listed below in the administration of this QASP.

a. Direct Observation during Strat check-in meetings and other status meetings.

b. 100% Inspection analysis and evaluation of reports: the final Annual Recruitment Plan, the Mid-Year Report, and the Annual report.

c. Observation of deliverables (See deliverables below)

Regardless of the surveillance method, the COR shall always contact the contractor representative when a defect is identified and inform the contractor of the specifics of the problem.

5. DELIVERABLES

a. Peace Corps- the Annual Recruitment Plan

b. Monthly Reports

c. Mid-Year Report

d. Annual Report.

6. RATINGS

OUTSTANDING:
Performance significantly exceeds contract requirements to the Government’s benefit.
SATISFACTORY:
Performance meets contractual requirements.
UNSATISFACTORY:
Performance does not meet contractual requirements.

7. PERFORMANCE REPORT

The Quality Assurance Surveillance Plan is the process Peace Corps will use to monitor, measure, and provide feedback to a Strat School on the quality of their contract performance. The outcome of the QASP process is to determine if the contract performance is sufficient in order to award a contract option year if available per the terms on the base contract. It is important to note that an option year award is at the full discretion of Peace Corps and not guaranteed even if contract performance is being met.

Peace Corps Recruitment will prepare a formal written update on the quality of contract performance after receipt of the final Annual Recruitment Plan, the Mid-Year Report, and the Annual report.

Acceptable Performance. The Government shall document positive performance.

Unacceptable Performance. When unacceptable performance occurs, the COR shall inform the contractor. This will normally be in writing unless circumstances necessitate verbal communication. In any case the COR shall document the discussion and place it in the COR file.
