About this file
This document is a Parent Interview Form (Form C-2.3B) used by the State of South Carolina's juvenile justice system to conduct comprehensive assessments of juveniles in detention. The form is designed to gather detailed information from parents or caregivers of youth involved in the criminal justice system and serves as a foundational assessment tool for psychological evaluation services. The form collects information across multiple domains including current charges, family background, family functioning, developmental history, mental health symptoms, school performance, peer relations, and community living arrangements. The interview captures data on parental custody status, family composition, child behavioral history, and any prior involvement with the Department of Social Services or law enforcement.
The form systematically documents family dynamics, including disciplinary practices, sibling relationships, household responsibilities, and curfew compliance. It captures developmental milestones, medical history, substance use, and any history of abuse or neglect. The mental health section specifically queries symptoms of depression, anxiety, attention difficulties, aggression, self-harm, and behavioral concerns. Additionally, the form assesses academic performance, special education services, school discipline, peer relationships, gang involvement, online activity, sexual activity, and community functioning. The form concludes by soliciting parental perspectives on the child's strengths, family strengths, placement appropriateness, and any concerns for discussion, providing a comprehensive assessment foundation for psychological evaluation services in the juvenile justice system.
