STATEMENT OF WORK (SOW) FLOORING
REPLACEMENT/INSTALLATION
Alpha-01, Bravo-01, Charlie-01, Delta-01, Echo-01
1. DESCRIPTION OF SERVICES: The Contractor shall provide all management, tools, supplies, equipment, and labor necessary to perform all work in accordance with these specifications. Work includes installing and/or replacing: ceramic tile, cove base, patching floor, carpet tile and providing labor to remedy unusual requirements related to the installation or replacement of flooring materials. Work is to be performed at Missile Alert Facilities (MAFs) in the 740th Missile Squadron located in McLean and McHenry Counties, North Dakota. Ceramic tile will be supplied by the Government.
1.1 WORK INCLUDED.
1.1.1 Remove existing carpet, edge molding/transition strips, vinyl cove base, rear entryway pvc baseboard and tile.
1.1.2 Prepare and measure all surfaces to receive new flooring.
1.1.3 Install carpet tile and government furnished ceramic tile as specified at locations identified in attached drawings.
1.1.4 Install vinyl cove base, PVC cove base, adhesives, ceramic tile, and flooring material as identified in Section 2 of the SOW.
1.1.5 Install edge molding/transition strips where flooring joins at other floor finishes.
1.1.6 The Contractor shall dispose of all scraps and debris after installation at an off base location and vacuum the entire area where flooring was installed at no additional cost to the Government. Disposal shall be in accordance with existing local, state, and federal regulations and shall not be placed in base refuse dumpsters.
1.1.7 Upon completion of installation the contractor shall promptly notify the Government Inspector for final inspection and acceptance of service.
1.2 WORK EXCLUDED.
1.2.1. All furniture and equipment will be moved by the Using Agency’s facility occupants. Where the contractor elects to perform movement of furniture or equipment to gain access to additional work areas, the work will be done at no cost to the Government. Any furnishings moved by the contractor shall be returned to their original location, and the contractor shall be liable for any damages caused by the movement of furnishings.
1.3 REFERENCE STANDARDS. All work shall be accomplished in accordance with CRI-2011 Carpet and Rug Institute Standards, the Manufacturers Specifications, Engineering Technical Letter (ETL) 07-4: Air Force Carpet Standard, and The Air Force Carpet Selection Handbook.
1.3.1 Ceramic tile and grout will be in accordance with Unified Facilities Guide Specifications (UFGS) Section 09 30 10 Ceramic, Quarry, and Glass Tiling.
2. PRODUCTS:
2.1 Adhesives and primers shall be consistent with the type recommended by the manufacturer for the floor covering being installed. Adhesives shall be used in strict accordance with manufacturer's printed instructions.
2.2 Edge molding shall be heavy duty vinyl designed for the type of flooring being installed. Floor flange shall be minimum 2" wide. Color shall match the cove base -- black.
2.3 After flooring is installed, the Contractor shall install a black in color, set-on type, rolled, rubber cove base 1/8 inch thick and 4”in height in the carpeted areas. An adequate coved backing strip to prevent "poke-thru" shall be provided.
2.4 Install 6” PVC cove base in the rear entryway.
2.5 CERAMIC TILE: Class V – Heavy Commercial 13” x 13”, 5/16” thick, IAW ANSI 137.1. Daltile glazed porcelain – STYLE: Cotto Contempo, COLOR: Wall Street. Ceramic tile shall be supplied by the Government.
2.6 GROUT: TEC Power grout or approved equal. COLOR: Light Pewter.
2.6.1 Dry-set Portland cement mortar. TCNA Handbook for Ceramic, Glass, and Stone Tile Installation.
2.6.2 Latex-portland cement mortar. TCNA Handbook for Ceramic, Glass, and Stone Tile Installation.
2.6.3 Ceramic tile grout shall be petroleum-free and plastic-free, latex-portland cement grout. TCNA Handbook for Ceramic, Glass, and Stone Tile Installation.
2.7 CARPET: Shaw Carpet Tile, 24”x24”. STYLE: Captivate Tile, COLOR: Parachute (or approved equal). Carpet shall be installed using the standard for commercial carpet CRI-104.
2.7.1 COMPLIANCE WITH COMMODITIES CONTRACT: In accordance with the Government’s Commodities Contract policy, the Government shall choose carpet types and patterns from each of the Enterprise Carpet Contract vendors for this contract. For carpet and adhesive material costs, if required per task order, the contractor shall utilize the mandatory sources below, and adhere to the Not to Exceed (NTE) pricing and types of materials offered from the following vendors in the Enterprise Carpet program:

3. EXECUTION:
3.1 The Government Inspector shall work in cooperation with the Contractor and the Contracting Officer (CO) in order to schedule and coordinate the installation of flooring with the facility where the work is to be performed.
3.2 The possibility of Asbestos Containing Material (ACM) may exist in the area to be worked. If there is ACM in the area, the Contractor shall not use any mechanical means to remove the ACM. In the event during carpet removal it is discovered that the ACM must be removed, the Contractor must stop immediately and notify the Inspector and the CO.
3.3 REMOVAL
3.3.1 The most suitable removal method of floor coverings and associated items shall be determined by the contractor.
3.3.2 Work shall be performed in a manner which creates minimum disturbance to the occupants.
3.3.3 Remove any residual glue, oil, grease, paint, plaster, and other substances detrimental to proper performance of adhesive. Allow floors to thoroughly dry before laying floor covering. Floors shall be free of any foreign materials that would affect the proper installation of new floor covering.
3.4 Patching and Preparation of Surfaces. Inspect all areas after existing flooring removal and before installation of new flooring. Cracks, rough areas, and holes and depressions up to 1/2” deep shall be repaired using polymer fortified patching compounds recommended by the flooring or adhesive manufacturer that are compatible with existing surfaces to ensure a smooth, finished appearance. Minimum floor temperature before, during and after installation and requirements for conditioning adhesive, shall comply with the floor covering and adhesive manufacturer's instructions. Floor temperature shall be a minimum of 60° Fahrenheit for at least 24 hours prior to, during, and after installation. Contractor shall anticipate up to 10% of the total project floor area needing repair per this paragraph.
3.5 INSTALLATION
3.5.1 Vacuum/sweep clean subfloor. Apply skim coat to entire floor surface.
3.5.2 Cut and fit floor covering neatly around projections through floor and to walls and other vertical surfaces.
3.5.3 Install molding/transition strips where installed flooring joins other floor coverings. Use full-length transition pieces only. Butt tight to vertical surfaces. Where splicing cannot be avoided, butt ends tight and flush.
3.5.4 Install cove base where removed. Score interior corners and heat mold exterior corners for proper installation.
4.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES:
4.1 UTILITIES. All reasonable quantities of utilities shall be made available at no cost to the Contractor.
4.2 STORAGE. Material and equipment storage may be allowed at the worksite; proposed storage location on-site shall be reviewed by the Government for approval. If approved, materials shall be placed in a neat and orderly fashion, and in a manner that safely allows continued facility use.
4.3 DAMAGE TO GOVERNMENT PROPERTY. The Contractor shall notify the Government when damage to government property is discovered. Any damage to government property caused by the Contractor shall be repaired or replaced by the Contractor at the Contractor’s expense. The Contractor shall annotate on the inspection report the damage, the cause of damage (if known), and a projected proposal for repair.
5.0 GENERAL INFORMATION:
5.1 HOURS OF OPERATION. 7:30 AM to 4:00 PM, Monday through Friday, except Federal Holidays listed in paragraph 5.2.
5.2 FEDERAL HOLIDAYS.

5.3 BASE CLOSURES. Work scheduled but not accomplished because of base closure due to weather, exercises, real-world alerts, or health safety reasons, shall commence as soon as possible after reopening the base, unless otherwise directed by the CO through coordination of the Inspector. All Government-caused work stoppages must be documented by the Government Inspector/Escort at the time of occurrence and reported by the Contractor, in writing, to the Contracting Officer within 48 hours of occurrence.
5.4 SECURITY REQUIREMENTS.
5.4.1 The Missile Alert Facilities are manned facilities operating 24 hours per day, seven days per week. Contractor personnel will be limited to work areas and will be accompanied by a Government Inspector/Escort at all times. Only Contractor personnel/vehicles listed on an Entry Authority List (EAL) will be granted access to the site. The Contractor shall allow 30 minutes at the start and 30 minutes at the end of each work day for personnel to process onto and off the site; this time will not be considered delay time. Any site access delays above and beyond these times must be reported by the Contractor, in writing, to the Contracting Officer within 24 hours of occurrence.
5.4.1.1 PROCESSING AN EAL FOR USE ON MISSILE SITES. In order to gain entry onto a facility, Contractor personnel and mission essential Contractor owned/leased vehicles must be listed on an EAL, identified by the Government Inspector/Escort, and physically escorted on site by the Government Inspector.
5.4.1.1.1 Twenty (20) working days prior to the initial commencement of work, the Contractor must submit to the Contracting Officer a summarized list of employees and vehicle information along with a legible photocopy of each employee’s REAL ID Act approved picture identification (ID). These summarized lists and photocopies will be used to prepare the Contractor’s EAL referenced above. Foremen, supervisors, and superintendents must be identified as such on this list of employee information. Each employee will have their identity proofed and their access vetted through the National Crime Information Center database and the Security Forces Information Management System using their name, date of birth, and Social Security number.
5.4.2 The Contractor’s summarized list of employee information must show each employee’s:
1) Full name exactly as it appears on the REAL ID Act approved ID submitted for use in conjunction with the Contractor’s EAL.
2) Date of Birth.
3) Full Social Security number.
4) Name and control number (exactly as it appears on the REAL ID Act approved ID).
5) Legible copy of the individual’s REAL ID Act approved picture ID.
Individuals who do not possess a driver’s license or ID card compliant with the REAL ID Act, and do not possess an acceptable alternate identification credential will be denied access. For the current status on state and territories’ driver’s licenses and IDs that are approved, visit the Department of Homeland Security website at the following location: https://www.dhs.gov/real-id.
5.4.3 The Contractor’s summarized list of vehicle information must include:
1) Vehicle Make
2) Vehicle Model
3) Vehicle Year
4) Vehicle Primary Color
5) Vehicle License Plate number, including state of issue. If no license plate, vehicle identification number or serial number.
6) Indication if vehicle is owned or leased.
Vehicles include mission essential company owned/leased vehicles driven onto the site. Mission essential equipment (skid-steers, forklifts, loaders/backhoes, and any equipment being transported), trailers, and materials need not be listed individually on the EAL. No privately owned vehicles will be permitted on the Alert Facility.
5.4.4 Upon arrival at the site, the Government Inspector/Escort will identify and search all Contractor personnel and vehicles. All Contractor personnel must provide the Government Escort with two (2) forms of identification, at least one of which is a photo ID, stating the employee’s full name and SSN. These two forms of ID shall be the same two forms used to generate the EAL. Contractor vehicles shall be clearly marked identifying each contracting firm.
5.4.4.1 Contractor personnel not listed on the EAL, or those who are listed cannot provide proper identification, will not be permitted on the site. Delays caused by lack of proper EAL data or proper identification shall be at no cost to the Government.
5.4.5 REPORTING REQUIREMENTS. Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware that may pose a threat to the security of DoD personnel, contractor personnel, resources, and/or classified or unclassified information. Contractor’s employees shall be briefed by their immediate supervisor prior to entering the MAF.
5.5 NOTIFICATION OF ENVIRONMENTAL SPILLS. The Contractor or their authorized representative shall report any spills immediately by calling the Minot AFB Fire Department Alarm Room at 701-723-2461 and the Base Environmental Section at 701-723-4825 or 701-723-1964. The Base Environmental Section will be responsible for notifying the National Response Center, the North Dakota Department of Health, and the United States Air Force and will complete the required regulatory reporting. The Contractor shall be held liable for the spill or release of such substances, and is expected to clean up all spills of less than 25 gallons. The Contractor shall be liable for the cleanup and cost of any spill greater than 25 gallons that occurs and is determined to be contractor negligence.
5.5.1 The Contractor is required to have and maintain Safety Data Sheets (SDS) on all Hazardous Material (HM). The Contractor shall provide a list of all HM to the CO at least 30 days prior to entering the base. The CO will distribute the list to all appropriate agencies for approval. The list shall contain the following information: the name of any HM expected to be used, including a copy of the SDS, an estimate on the quantity of the HM expected to be used during the performance of the contract, and anticipated storage location for each HM intended for use.
5.6 COMPLIANCE WITH LAWS AND REGULATIONS. The Contractor shall be knowledgeable of and comply with all applicable federal, state, and local laws, regulations, and requirements regarding environmental protection to include AFI 32-7080, Pollution Prevention Program, AFI 32-7042, Solid and Hazardous Waste Compliance, and ACC Manual 32-7051, Environmental Quality Manual, and 40 CFR 273, 40 CFR 133, North Dakota Administrative Code 33-24-02-06, and base Hazardous Waste Management Plan (September 2005) 5 BW-32-7043. In the event of environmental laws/regulations/requirements change during the term of the contract, the Contractor shall comply with such laws/regulations as changed. The Contractor shall comply with the base Hazardous Waste Management Plan (September 2005) when turning in wastes generated from spill cleanup.
5.7 TRAFFIC LAWS. The Contractor and its employees shall comply with base traffic regulations.
5.8 WEAPONS, FIREARMS, AND AMMUNITION. Contractor employees are prohibited from possessing weapons, firearms, or ammunition, on themselves or within their contractor-owned or privately owned vehicle while on Minot Air Force Base and the MAF.
5.9 FOR OFFICIAL USE ONLY (FOUO). The contractor shall comply with DoD 5400-7-R, Chapter 4, DoD Freedom of Information Act (FOIA) Program requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
5.10 PERSONNEL. Contractor personnel shall present a neat appearance. Contractor personnel shall be easily recognizable while on the installation in conjunction with this contract. This shall be accomplished through the wear of distinctive clothing, overcoats, or hats, bearing the company name or logo. The coloring or design of the items selected should be such that identifies personnel easily and quickly for reasons of safety and personal protection.
The Government is authorized to restrict the employment under the contract of any contractor employee or prospective contractor employee, who is identified as a potential threat to the health, safety, security, general well-being, or operational mission of the installation and its population.
