STATEMENT OF WORK
FOR
STATIC AIRCRAFT RUBBER SURFACE REPLACEMENT
TYNDALL AFB, FLORIDA
32403
15 June 2020
1.0 DESCRIPTION OF SERVICES.
1.1. General. The Contractor shall provide all management, supervision, labor, materials, equipment, transportation, and incidental items necessary to replace approximately 11,404 square feet of damaged rubber overlay of four static aircraft mounds and remove approximately 14,764 square feet of white rock from four static aircraft mounds and replace with rubber. Contractor will be responsible for removal and off-site disposal of rubber and rocks that are currently installed. Once all rubber and rocks are removed, contractor will replace/re-grade substrate to ensure new rubber can be installed.
The Contractor shall perform to the standards in this contract. This includes the planning, coordination, and surveillance of the activities necessary to ensure disciplined work performance and timely resources application to accomplish all tasking under the contract. The Contractor shall be responsible for maintaining communication with the Contracting Officer Representative (COR) and to immediately notify of any problems that would prevent timely performance of this contract. The Contractor is responsible for and required to implement, and maintain management control systems necessary to plan, organize, direct, and control all activities under this contract.
1.1.1. Description of Services. The contractor shall remove, re-grade and replace rubberized surface underneath four static aircraft mounds at AF North, building 1210 and remove white rock from four static aircraft mound and replace with rubber at Flag Park on Tyndall AFB.
1.1.2. Material/Installation. The contractor is responsible for the completion of the subsurface in the area where the poured-in-place rubberized surfacing is to be applied, and contractor shall:
- Establish and maintain required lines and elevation for grade.
- Replace and re-grade substrate.
- Base shall slope to provide positive drainage at a minimum of 1” per 10’ and shall not pond water at any location.
- Compacted stone base should be water permeable and subgrade should allow for free drainage by weeps and/or slumps.
- Install 1” of Shredded SBR cushion course and EPDM wearing surface system per manufacturer’s instructions.
- Install ½ “EPDM mat. Color must be approved prior installation. Picture of required color will be attached with this Statement of Work.
- Top of concrete curb to flush with finished surface, tooled rounded edges.
- Cure shall be not less than manufacture’s recommendations.
1.2. Standards. All work completed shall be performed in accordance with all applicable codes, Unified Facilities Criteria (UFCs), industry standards, and manufacturer’s instructions and recommendations. All Contractors will submit a detailed work plan and schedule for review.
1.3. Reference Standards and Publications. The standards and publications listed in each section of these specifications form part of these specifications to the extent referenced. The Contractor shall conform to the provision requirements of the dated issue of the listed standard or publication including:
• Unified Facilities Guide Specifications (Air Force)
• Code of Federal Regulations (CFR)
• 29 CFR 1910 Occupational Safety and Health Standards
• 29 CFR 1926 Safety and Health Regulations for Construction
1.4. Bid Submittals Required.
a. All necessary certifications/licenses, detailed placement methods and material specifications shall be submitted by the Contractor and approved by the USAF Government representative.
b. Bid package shall be broken down to indicate each material used including, but not limited to, foundations, shipping, handling and storage, installation, labor and costs.
c. Proposed preliminary schedule.
d. Proposed material finishes and color schemes.
2.0. GENERAL INFORMATION
2.1. Hours. All work is to be accomplished during normal day to day operations for this facility which start at 7:00 AM and conclude at 3:00 PM CT Monday-Friday. The Contractor will contact Steve DuPont 850-814-2561 to coordinate prior to start and when all work has been completed.
2.1.1. Recognized Holidays.*

*If the holiday falls on a Saturday, it will be observed on the preceding Friday. If the holiday falls on Sunday, it will be observed on the following Monday.
2.2. Contractor’s Use of Premises.
General: The Contractor shall limit his use of the premises to the work indicated.
Use of the Site: Confine operations at the site and rights-of-way to the areas permitted under the contract. Portions of the site beyond areas on which work is indicated are not to be disturbed.
The Contractor shall keep the work area, including storage areas, free from accumulations of waste materials on a daily basis and comply with all federal, state and local regulation, to include AFI 32-7042, pertaining to the storage, transport and disposal of wastes.
2.3. Site Visit. A mandatory site visit is not required at this time.
2.4 Coordination. The Contractor shall coordinate and cooperate with the using personnel and Government representative in scheduling operations to minimize conflict and mission impact. The Contractor shall coordinate worksite activities with all applicable personnel to ensure the protection of human health and the prevention of damage to property, materials and supplies.
2.5. Qualifications. The Contractor shall provide copies to the Contracting Officer’s Representative, all current Licenses required for this work, and will provide documentation of similar type work within the last 2 years.
3.0 Security
3.1. General
3.1.1. The Contractor shall comply with all base security regulations and with actions dictated by force protection condition changes/measures. For real-world situations/exercises, changes to force protection conditions could impact Contractor performance of work and can include denial of access to the installation to perform work.
3.1.2. The Contractor and its employees shall comply with all base traffic regulations.
3.1.3. Contractor employees are prohibited from possessing weapons, firearms or ammunition, on themselves or within Contractor-owned or privately owned vehicle while on Air Force installations.
3.1.4. The Contractor shall report suspicious activity (refer to Tyndall AFB Contractor Antiterrorism Awareness pamphlet) to Security Forces at 850-283-2254 in a timely manner.
3.2. Additional Security Requirements
3.2.1. Physical Security. The Contractor shall be responsible for safeguarding all Government equipment, information and property provided for Contractor use. At the close of each work period, Government facilities, equipment, and materials shall be secured.
3.2.2. Contractor Access to Air Force Installations. All Contractor employees will be subject to a criminal history background check IAW Air Force Manual (AFMAN) 31- 113, Installation Perimeter Access Control, as stipulated in the Installation Commander’s memorandum to the 325th Security Forces Office dated 23 Apr 2015 (the AFMAN and memorandum are both limited access documents but more details on the requirements/process in terms of background checks can be provided by the Unit Security Manager).
Vehicle registration, proof of insurance, and a valid driver's license must be presented for vehicles. Installation Commanders may deny access and credentials based on information obtained during identity vetting that indicates the individual may present a threat to the good order, discipline, and morale of the installation.
Base Access: The Contractor shall ensure the following pass and identification items required for contract performance are obtained for employees and non-Government owned vehicles:
a. DBIDS pass. Obtained through submitting personal identifiable information to Security Forces for criminal background check.
b. DoD Common Access Card (CAC), AFI 36-3026(I). Process through USM, Trusted Agent Sponsorship System (TASS) point of contact for a CAC card.
c. AF Form 75, Visitor/Vehicle Pass (AFI 31-204): The Contractor shall adhere to local security policy when applying for short/long term visitors’ passes. Any Contractor requesting a pass for more than 3 days will complete a Local Files Check with 325 SFS/Pass & ID. Vehicle passes (personal vehicles) are no longer required.
3.2.3. Visitors/Contractors with driver’s licenses from ME, MN, MO, MT, and WA (exceptions are EDL-Enhanced driver’s licenses from MN & WA) will not be able to obtain base access without another form of identification as an identity proofing document. The list of authorized IDs can be found in the REAL ID Act of 2005. Please visit the Department of Homeland Security’s (DHS) website to get the latest information, https://www.dhs.gov/current-status-states-territories.
3.2.4 During performance of the contract, the Contractor shall be responsible for obtaining required identification for newly assigned personnel.
The Contractor shall be responsible for prompt return of credentials and vehicle passes for: 1) any employee who no longer requires access to the work site; 2) upon request by a CO; and 3) upon completion or termination of the contract. The prime Contractor shall ensure that all base identification passes issued to employees and subcontractor employees are returned to the issuing office. Failure to comply with these requirements may result in withholding of payment.
The Contractor shall maintain a current listing of employees. The list shall include employee's name, social security number and level of security clearance or base access (to include if escort privileges are authorized), on company letterhead. The list shall be validated and signed by the company facility security officer (FSO), provided to the COR (and Unit Security Manager) prior to the contract start date, and to the Contracting
Officer (upon request). Updated listings shall be provided when an employee's status or information changes.
4.0 Safety
It is the Contractor's responsibility to ensure its employees and managers have a comprehensive understanding of and full compliance with OSHA requirements. Detailed information is available on the OSHA website at http://www.osha.gov. It is the Contractor's sole responsibility for compliance with OSHA, Public Law 91-596. The Contractor shall comply with all federal, state and local laws, and Air Force Instructions for the protection of their employees while working on an Air Force Installation or property. The protection of Contractor employees is exclusively the obligation of the Contractor. Contractor shall manage all work areas to ensure the safety of building occupants, base personnel, or visitors in or near the areas.
The Contractor shall submit a Safety and Health Plan that describes the capabilities and processes to ensure compliance with safety and health requirements during the performance of the work described in the solicitation. The Contractor's plan shall include appropriate measures to ensure the Contractor reacts promptly to investigate, correct and track alleged safety and health violations and/or uncontrolled hazards in Contractor work areas. Contractor must ensure that personnel are properly trained on the equipment they are tasked to operate. The Contractor shall develop site specific mishap reporting procedures which result in damage to Government property or injury to Air Force personnel. These procedures are to be included in the safety and health plan for review.
In accordance with AFI 91-202, AFMC Supplement, Safety Program, the Contractor shall submit a Safety and Health Plan to the CO for review and acceptance by the safety office prior to contract award.
4.1 Safety Equipment. The Contractor shall furnish personal safety equipment for its employees in performance of this contract. Safety equipment includes shoes or boots, eyewear, hearing protection, respirators, and gloves.
4.2 Safety Threats. The Government reserves the right to restrict the employment, under this contract, of any Contractor employee, or prospective Contractor employee who is identified as a potential threat to health, safety, security, general wellbeing, or the operational mission of the installation and its population. Contractor shall perform background checks, at Contractor's expense, to ensure that said employee has not been convicted of any sexual crime or any crime against children.
