DEPARTMENT OF THE AIR FORCE
HEADQUARTERS 36TH WING (PACAF)
ANDERSEN AIR FORCE BASE GUAM
STATEMENT OF WORK
FOR
DISASSEMBLY AND REMOVAL
OF CRYOGENIC PLANT
ANDERSEN AFB, GUAM
Date: 20 July 2020
1. GENERAL: The Contractor shall disassemble, remove, and dispose of a liquid oxygen-generating plant in accordance with the Statement of Work (SOW) and DLA disposition instructions.
2. DESCRIPTION OF SERVICES:
2.1. This is a services contract to remove all equipment related to the cryogenics plant that is out-of-service in Bldg. 26224 on Andersen AFB, Guam.
2.1.1. The Government shall not exercise any supervision or control over the contract service providers performing the services herein. Such contract service providers shall be accountable solely to the Contractor who, in turn is responsible to the Government.
3. REQUIREMENTS: The contractor shall provide all the necessary personnel, labor, tools, equipment, and transportation for the removal and disposition of the cryogenics plant and its two 40ft shipping containers.
3.1. CRYOGENICS PLANT DISASSEMBLY:
Contractor shall disassemble and dispose all components to include:

4.1. All components listed in paragraph 3.1 including the two 40 ft shipping containers are to be removed from site and transported to DLA
Disposition Services.
4.2. Removal and disposition of the cryogenics plant shall be in accordance with DLA disposition instructions in the DLA Disposition Turn In
Smart Book. For further disposition instructions contact DLA Defense
Reutilization Management Office (DRMO) at 671-339-4056.

4.3. Contractor shall leave all areas pertaining to the cryogenics plant cleared of all trash and debris.
4.4. Contractor Personnel: The contractor shall provide a point of contact
(POC) for all contractual matters.
5. PERFORMANCE:

5.2. PERIOD OF PERFORMANCE:
5.2.1. The period of performance (PoP) shall be 60 Days ADC.
5.3. HOURS OF OPERATION:
5.3.1.1. The contractor is responsible for conducting business, between the hours of 0730hrs to 1630hrs (7:30am to 4:30pm)
Monday thru Friday except Federal holidays or when the
Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.

5.4. GOVERNMENT POCS:

6. LIABILITY: The contractor shall be liable for any damages resulting from any carelessness or failure to follow proper safety, installation, engineering, or maintenance procedures and standards.
7. GOVERNMENT FURNISHED SERVICES:
7.1. Escort Services: An escort shall be provided by calling the Fuels Service
Center at 671-366-6291 during the provided work hours of 0730 to 1630, M-F.
7.2. Emergency Services: The government will provide emergency medical transportation and treatment if required. The contractor shall reimburse the
Government for the cost of these medical services at current rates.
8. SECURITY
8.1. PASS AND IDENTIFICATION ITEMS: The contractor shall ensure the pass and identification items required for contract performance are obtained for employees.
8.1.1. In addition to acquiring base passes, an Entry Authorization List
(EAL) must be submitted to 36 LRS for access to the restricted area on base for all personnel that will be on-site. The point of contact for that is at TSgt Eric Hyde at 366-5115. The EAL process takes about 2 weeks. Once awarded, keep EAL on your persons while in the restricted fuels yard.
8.2. RETRIEVING IDENTIFICATION MEDIA: The contractor shall
return all identification media and/or visitor passes from employees who depart for any reason before the contract expires; i.e. terminated for cause, reassignment, retirement, etc.
8.3. TRAFFIC LAWS: The contractor and its employees shall comply with base traffic regulations. Traffic regulations include, but are not limited to:
8.3.1. Employees shall wear seatbelts when operating or riding in any vehicle
(military, company or private owned) while on installation.
8.3.2. Speed limits on Andersen AFB are 25 MPH unless otherwise posted.
Be aware of reduced speed limits and exercise caution when driving in base housing and school zones.
8.3.3. All vehicle operators on the installation shall not use cell phones unless the vehicle is safely parked or they are using a 100% hands free device.
The wearing of any other portable headphones, earphones or other listening devices while operating a motor vehicle is prohibited.
8.4. WEAPONS, FIREARMS, AND AMMUNITION: Employees are prohibited from possessing weapons, firearms, or ammunition on themselves or within vehicles (military, contractor-owned, or privately owned) while on Andersen
AFB.
8.5. FOR OFFICIAL USE ONLY (FOUO): The contractor shall comply with
DoD 5400.7-R, Chapter 4, DoD Freedom of Information Act (FOIA)
Program, requirements. This regulation sets policy and procedures for the disclosure of records to the public and for marking, handling, transmitting, and safeguarding FOUO material.
8.6. REPORTING REQUIREMENTS: Contractor personnel shall report to an appropriate authority any information or circumstances of which they are aware may pose a threat to the security of DoD personnel, contractor personnel, resources, and classified/unclassified defense information.
Employees shall be briefed by their immediate supervisor upon initial on-base assignment.
8.7. PHYSICAL SECURITY: The contractor shall be responsible for safeguarding all Government property and controlled forms provided for contractor use. At the end of each work period, all Government facilities, equipment and materials shall be secured.
