About this file

This document is an attachment detailing Hospital Standards and Requirements for contractors at the Department of State Hospitals - Atascadero (DSH-Atascadero) in California. The document outlines four key policies that contractors, their personnel, subcontractors, and service providers must follow: a detailed dress code policy, neckware policy, tool control procedures for secured areas, and influenza vaccination requirements.

The dress code policy prohibits khaki clothing, requires proper ID badge placement, and restricts certain types of clothing and logos. The neckware policy prohibits items that encircle the neck except for clip-on attachments and state-issued lanyards. Tool control procedures require inventory documentation and DPS officer verification for bringing maximum 10 tools into secured areas. The influenza vaccination policy requires either proof of vaccination or completion of a declination form, with unvaccinated providers required to wear masks during flu season in patient areas. Free flu vaccines are available upon request.
