About this file
This Business Proposal Attachment E document is part of RFP 26-86529 issued by the State of Indiana. The attachment serves as the primary vehicle for respondents to provide detailed information about their organizations, financial stability, relevant experience, and commitment to contract requirements. Respondents must complete all sections with information demonstrating their company structure, legal formation, business divisions, and organizational charts. The proposal requires submission of financial documentation including Dunn & Bradstreet reports or audited financial statements for the two most recently completed fiscal years, with certification by the CEO and/or CFO regarding the accuracy of all financial information. Additionally, respondents must provide three client references through Attachment H, which should be submitted directly to idoareferences@idoa.in.gov by the proposal due date specified in RFP Section 1.24, demonstrating experience with similar products or services.
The proposal submission requires respondents to address experience serving state governments and quasi-governmental accounts, describe relevant experience with similarly-sized clients, and provide general business information including federal tax identification number, employee count, years of experience, revenue figures, and office locations. Respondents must confirm registration to do business with the Indiana Secretary of State prior to contract negotiation and provide proof of signatory authority through corporate bylaws or board resolutions. The document further requires respondents to demonstrate engagement efforts with Minority & Women's Business Enterprises (MBE/WBE) and Indiana Veteran-Owned Small Business (IVOSB) subcontractors, document business continuity and disaster recovery plans, describe data security protocols, and indicate willingness to extend pricing to other governmental bodies. For cloud-based systems, respondents must review and accept or propose redlined edits to Additional Terms and Conditions provided in Attachments B1, B2, and B3 for Infrastructure-as-a-Service, Platform-as-a-Service, and Software-as-a-Service offerings, respectively.
