MUNISING WORK CENTER REPAVING PROJECT
HIAWATHA NATIONAL FOREST
MUNISING RANGER DISTRICT
ALGER COUNTY, MI
1.0 PROJECT INTRODUCTION
Project Name: Munising Work Center Repaving Project
1.1 Scope
In accordance with the terms and conditions of the Contract, the Contractor shall perform the work of this Statement of Work (SOW) for the USDA-Forest Service (FS) as described below.
The Contractor shall perform inspection, and construction services in accordance with the requirements within the contract. FS will provide construction staking. The Contractor shall provide all equipment, labor, supervision, transportation, materials, tools and other incidentals and execute all work necessary to perform the milling and overlay of the Munising Work Center in accordance with specification and exhibits in this contract, and current industry standards. Contractor is responsible for ensuring all equipment meets State, Federal, and Local Laws.
The Contractor shall perform all services necessary to produce a complete and usable project in accordance with the project goals and design intent stated herein.
1.2 Location
The project is located at the Hiawatha National Forest Munising District Work Center at 314 E Varnum St. From the Munising District Office turn left onto E. Munising Ave., at the traffic circle take the 2nd exit onto Cedar Street, travel 0.2 miles along Cedar Street, continue Cedar Street for 0.2 miles to East Varnum Street, turn right onto East Varnum Street, travel on Varnum street for 250 feet to reach 314 East Varnum Street Munising, MI 49862. 2.0 GENERAL REQUIREMENTS
2.1 Scope of Work Summary
The project will consist of improving the parking lot/ roadway at the facility based on the attached drawings set. The work covered by these plans shall conform to the FP-14 Standard Specifications for Construction Roads and Bridge on Federal Highway Projects and the Forest Service Supplemental Specifications. The project will include these major tasks, but not be limited to, the construction of the following major items of work:
1. Mobilize for the project.
2. Install temporary traffic control signs before starting construction. See Section 635 – Temporary Traffic Control. This shall be maintained for the duration of the project. Signage shall comply with Manual on Uniform Traffic Control Devices (MUTCD).
3. Mill asphalt from the entire paved area. See Section 413 - Asphalt Pavement Milling.
4. Compact roadbed surface to comply with Section 403- Asphalt Concrete.
5. After the milling is complete, clean the existing surface of loose material, dirt, and other materials. Apply tack coat to the cleaned surface. See Section 412 - Asphalt Tack Coat.
6. Apply two 1.5” compacted thickness lifts of Type II asphalt concrete. The mix shall be placed according to Section 403 – Asphalt Concrete.
7. All bituminous mixtures shall be transported by a truck which shall have tight, clean, smooth metal beds. Each truck shall be covered with a canvas or other suitable material of such size that will protect the mixture from the weather. See Section 401.11- Hauling.
8. Bituminous materials shall be placed, finished, and compacted according to Section 403 Asphalt Concrete. See Section 401.07- Weather Limitations.
9. Seal joints at the interface between the new asphalt the existing drainage structures, sidewalks, and pavement. See Section 414- Asphalt Pavement Crack Sealing and Filling.
10. On all disturbed areas, spread approved seed mix. See Section 625- Turf Establishment.
Verification of Dimensions The contractor shall be responsible for the conditions and proper relation of their work to the site conditions and will familiarize themselves with all details of the work and working conditions. The contractor shall verify all dimensions in the field and generate their own quantities for work provided. Any and all incidental items not specifically listed in this contract are to be included by the contractor, to provide the government a complete and usable end product.
2.2 Project Meetings
2.2.1 Pre-Construction Meeting
The COR shall setup a pre-construction meeting with the contractor after award. Payrolls and other construction administration documentation shall be required to be prepared by the Contractor and submitted to the COR.
2.3 Contractor’s Project Schedule
General: The Contractor shall provide a Project schedule for the entire contract period within 2 weeks after award.
2.4 Submittals
2.4.1 Submittal Register
Submittals for this contract shall be emailed to the COR.
Manufacturer’s recommended installation procedures and Manufacturer’s Industry Standard shall be the basis of how products and materials are installed in addition to building and accessibility codes. It is strongly encouraged that the Contractor provide the manufacturer’s recommended installation instruction or manual for each submittal and process. If the proper installation criteria are not provided, then the CO/COR will rely on the manufacturer’s web-based material or mfr. provided documentation as the source of installation validation. No construction product, material or system shall be installed without approved documentation by the CO. They shall hold the Contractor to these installation standards in construction if there is not procedural standard for this work.
2.4.2 Submittals Distribution and Format
B. Construction Submittals:
1. All Construction submittals shall be e-mailed to the COR. Other formats may be approved by the COR.
2.4.3 Submittal Procedures
The COR shall have 10 days to review and return submittals to the Contractor. The Contractor can request a faster turnaround on specific submittals in writing if warranted.
The CO/COR oversight is for quality assurance only and compliance with contract requirements. The Contractor is fully responsible and liable for construction applications, technical considerations, and use of products for the intended application.
3.0 GENERAL REQUIREMENTS BY WORK ITEM
3.1 Asphalt Replacement
Mill and overlay approximately 11354 square feet of asphalt as shown on the attached plans. Contractor shall regrade the base to ensure drainage to the drainage ditch. Ensure that the final base is sufficient in compaction and quality to support delivery traffic. See Section 413 - Asphalt Pavement Milling. Slope to drain and provide a smooth transition between the parking lot and driveway. If base is determined unsuitable the Government will provide suitable aggregate to the site.
3.2 Seal Coat
After the milling is complete, clean the existing surface of loose material, dirt, and other materials. Apply tack coat to the cleaned surface and curb faces. See Section 412 - Asphalt Tack
4.0 SPECIFIC CONSTRUCTION REQUIREMENTS
4.1 Safety
The Contractor shall comply with all Federal and State safety requirements for the type of work to be performed. Refer to the Occupation Safety and Health Standards (OSHA) for details of these and other safety requirements.
4.2 Closure of Site
The contractor shall be responsible for closing the parking lot work area with barriers 1 day prior to commencement of work. The COR will be responsible for ensuring vehicles are removed from work area by end of day closing occurs. If vehicles are located within a work area, contractor shall leave vehicle in its place and not work within 10’ of the vehicles location.
Upon commencement of work, it shall be their responsible to supply and maintain traffic control devices during the duration of work and all associated dry times. One Type 2 or Type 3 barricade is required at each entrance of work location. The contractor may then utilize type 1, 2, or 3 barricades and/or any other method to ensure no vehicles can enter the work location.
4.3 Advertisement/Company Logo
The Contractor shall not place any form of advertisement or company logo, in any form, on the pavement or anywhere on Federal Lands.
4.4 Project Limits along Roadways
The limits of work along Federal, State, County, or Local roads include all pavement “drive apron” or deceleration/ acceleration lanes associated with that road or site.
4.5 Construction specifications (FP-14)
The Forest Service, US Department of Agriculture has adopted the Standard Specifications for Construction of Roads and Bridges on Federal Highway Projects, FP-14, U.S. Customary Units (FP-14) for construction of National Forest System Roads and other projects.
The full provisions of FP-14 shall apply as applicable on this contract, unless modified by the Forest Service Supplemental Specifications, attached.
5.0 ADDITIONAL REQUIREMENTS
5.1 Temporary Facilities
The contractor shall provide bathroom facilities (i.e port-a-potty).
5.2 Work Hour Restrictions
All work and deliveries shall be limited to the weekday hours of 8:00 am to 5:00 pm unless otherwise approved by the COR. No work shall occur on Federal holidays or weekends without prior approval.
5.3 Quality Control
The quality of all work shall be the responsibility of the Contractor. Inspect and test all work as needed to ensure that the quality of materials, workmanship, construction, finish, and functional performance is in compliance with applicable specifications and drawings. Failure to provide adequate quality control will result in retention and other contract actions.
5.4 Hazard Control
Take all necessary precautions to prevent fire during construction. Do not store flammable or combustible liquids in existing structures. Provide adequate ventilation during use of volatile or noxious substances.
5.4.1 Smoking
Smoking inside Forest Service buildings is prohibited, cigarette butts shall not be discarded on the ground.
5.5 Minimizing Construction Impacts
The Contractor shall take all precautions necessary to minimize and restore all construction impacts. All existing work and conditions impacted or modified by the contractor shall be replaced with like kind at the contractor’s expense.
5.6 Project Closeout
Before submitting a request for final inspection, submit the following:
Payment:
The quantities shall be paid for as lump sum quantities or by other methods of measure as listed in the SCHEDULE OF ITEMS. Such payment shall be considered full compensation for constructing the complete project in accordance with the plans and specifications. The availability of progress payments shall be determined by the CO. The invoice must be inputted into the IPP System.
Additional Documents required to be prior to final invoice processing are:
· Certified Payroll
· Contract Release Form Final payment will not be processed until all documents have been received by the CO/ COR.
5.7 Final Inspection
Before scheduling the final inspection, remove all tools, equipment, surplus materials, and rubbish. Restore or refinish surfaces that are damaged due to work of this contract to original condition. Pick up and remove all construction debris from the site. At time of final inspection, project shall be thoroughly clean and ready for use.
