Statement of Work (SOW) Surgical Trays
A. GENERAL INFORMATION
1. Purchase of Surgical Trays.
2. Scope of Work: The Department of Veterans Affairs, Atlanta VA Healthcare System (VAMC), 1670 Clairmont Road, Decatur, Georgia, 30033, utilizes vendor orthopedic equipment for surgical services on a daily/weekly frequency. The vendor equipment is necessary for safe and exceptional patient outcomes for orthopedic trauma surgical procedures. These procedures are currently serviced by a combination of VAMC owned equipment and vendor loaner equipment brought in specifically for the surgeries. The VAMC also owns a significant amount of outdated and obsolete vendor equipment that can no longer be used in these procedures.
3. Background: Veterans Affairs Medical Center is requesting Surgical Trays at the Atlanta VA Medical Center SPS Department located at 1670 Clairmont rd., Decatur, GA 30033. Currently, the VAMC utilizes a combination of owned vendor inventory and loaner vendor inventory to service surgical procedures. As surgical procedures vary in nature, the loaner vendor burden is often heavy in each week. The VAMC also requires two sets of instruments for one procedure, thus the burden is often more significant at the VAMC than at other facilities. This burden requires significant time from the SPS team to process these trays in time for surgeries.
4. Performance Period: The contractor shall complete the work required under this SOW in 30 calendar days or less from date of award, unless otherwise directed by the Contracting Officer (CO). If the contractor proposes an earlier completion date, and the Government accepts the contractor’s proposal, the contractor’s proposed completion date shall prevail. Work at the Government site shall not take place on Federal holidays or weekends unless directed by the CO.
5. Type of Contract: Firm-Fixed-Price
B. GENERAL REQUIREMENTS
For every task, the contractor shall identify in writing all necessary subtasks (if any), associated costs by task, and along with associated sub-milestone dates. The contractor's subtask structure shall be reflected in the technical proposal and detailed work plan.
All written deliverables shall be phrased in layperson language. Statistical and other technical terminology shall not be used without providing a glossary of terms.
Where a written milestone deliverable is required in draft form, the VA will complete their review of the draft deliverable within 14 calendar days from the date of receipt. The contractor shall have 30 calendar days to deliver the final deliverable from date of receipt of the Government’s comments.
C. SPECIFIC MANDATORY TASKS AND ASSOCIATED DELIVERABLES
Description of Tasks and Associated Deliverables: The contractor shall provide the specific deliverables described below within the performance period stated in Section A.4 of this SOW.
Task One: The contractor shall provide Surgical Trays for Sterile Processing Services.
Deliverable One: TBA Task Two: The Contractor will provide prepaid shipping and handling and added to the invoice. However, the Contractor will utilize customer-designated third-party freight programs for shipment and payment when requested by customer.
D. CHANGES TO STATEMENT OF WORK
Any changes to this SOW shall be authorized and approved only through written correspondence from the CO. A copy of each change will be kept in a project folder along with all other products of the project. Costs incurred by the contractor through the actions of parties other than the CO shall be borne by the contractor.
E. CONFIDENTIALITY AND NONDISCLOSURE
It is agreed that:
1. The preliminary and final deliverables, and all associated working papers, application source code, and other material deemed relevant by VA which have been generated by the contractor in the performance of this task order, are the exclusive property of the U.S. Government and shall be submitted to the CO at the conclusion of the task order.
2. The CO will be the sole authorized official to release, verbally or in writing, any data, draft deliverables, final deliverables, or any other written or printed materials pertaining to this task order. No information shall be released by the contractor. Any request for information relating to this task order, presented to the contractor, shall be submitted to the CO for response.
3. Press releases, marketing material, or any other printed or electronic documentation related to this project, shall not be publicized without the written approval of the CO.
