About this file

This document outlines requirements for external accreditation services for U.S. Coast Guard ambulatory health care clinics. Services are required for clinics located across the USCG enterprise in the United States and applicable territories. The requirement is set aside for professional organizations with an NAICS code of 813920 and small business size standard of $16.5 million. The Government intends to award a firm-fixed-price contract for one base year and four option years. Proposals are due no later than 1400 Eastern Time on 08 September 2020. The appendices provide templates and guidance for quality improvement processes but do not specify requirements for the external accreditation services.

Safety Assessment Code (SAC) Matrix

Adapted from Coast Guard Medical Manual, COMDINST M6000.1 (series), Chapter 13, Quality Improvement, the SAC is a risk assessment tool that con-siders the severity of an adverse or near miss event together with the probability of the events recurrence. The score, or Safety Assessment Code, assigned to the event determines the type of action that should be taken, e.g., Root Cause Analysis (score 3).

Severity Categories:

Severity is divided into four categories:

1. Catastrophic

2. Major

3. Moderate

4. Minor

The examples in the four categories below apply to events which resulted in harm. For near-misses, where the event did not reach the patient, assign severity based on the potential for harm. However, many cases that reach the patient also have the potential for greater degrees of harm than that experienced and should be considered in determining the final severity.

Catastrophic Major Patients with Actual:

-Death or major permanent loss of function (sensory, motor, physiological, or intellectual) not related to the natural course of the patient’s illness or underlying condition (i.e., acts of commission or omission) -Suicide -Rape -Hemolytic transfusion reaction -Surgery/procedure on the wrong patient or wrong body part -Patient abduction, any age -Unintentionally retained foreign body -Prolonged Fluoroscopy (>1500 rads to single field), or radiotherapy to wrong body region or >25% above the planned radiotherapy dose.

-Permanent lessening of bodily functioning (sensory, motor, physiologic, or intellectual) not related to the natural course of the patient’s illness or under-lying conditions (i.e., acts of commission or omission) -Disfigurement -Surgical intervention required -Increased length of stay or level of care of 3days or more

Moderate Minor

-Increased length of stay or higher level of care for less than 3 days

-No increased length of stay or increased level of care

Probability of Recurrence:

Like the severity categories, the probability recurrence applies to actual adverse events and near misses. In order to assign a probability rating for an adverse event or near miss, it is ideal to know how often it occurs at your facility. Sometimes, the data will be easily available because it is tracked. Sometimes, getting a feel for the probability of events which are not routinely tracked will mean asking for a quick or informal opinion from staff most familiar with those events. Sometimes it will have to be your best educated guess.

High-Likely to occur immediately or within a short period of time

Medium-Likely to occur several times in 1 to 2 years Low-May happen, but with an interval of two or more years between occurrences

The SAC Matrix (Severity versus Probability):

Probability of Recurrence Severity Severity Severity Severity Catastrophic Major Moderate Minor High 3 3 2 1 Medium 3 2 1 1 Low 3 2 1 1

How the SAC Matrix Works:

When you pair a severity category with a probability category for either an actual or near miss, you will get a ranked matrix (3=highest risk, 2- intermediate risk, 1 = lowest risk). These ranks, or Safety Assessment Codes (SACs), can then be used for doing comparative analysis and for deciding who needs to be notified about an event. All SAC-3 scored events must have a corresponding RCA as outlined in Coast Guard Medical Manual, COMDINST M6000.1 (se-ries), Chapter 13, Quality Improvement. Cases where events reached the patient but did not result in harm should be scored as SAC-1 no-harm events.

Notes:

1. All known reporters of events, regardless of SAC score, should receive appropriate and timely feedback.

2. Refer all adverse events or close calls solely related to staff, visitors or equipment/facility damage to relevant facility experts or services on a timely basis, for assessment and resolution of those situations.
