About this file
This is a Diversity Practices Questionnaire document issued by the New York Office of Contractor and Supplier Diversity for the SSBCI Technical Assistance to Capital Programs (Round 2) procurement. The questionnaire is required from respondents seeking selection as Technical Assistance (TA) providers to deliver legal, accounting, and financial services to eligible beneficiaries. Round 2 targets very small businesses (VSBs) and businesses owned and controlled by socially and economically disadvantaged individuals (SEDI-owned businesses) that are applying for, preparing to apply for, or have previously applied for SSBCI capital programs or other federal or state small business programs. The questionnaire must be completed, sworn under penalty of perjury, and notarized as part of the procurement response.
The questionnaire evaluates supplier diversity commitments through eight key questions assessing the respondent's diversity infrastructure and practices. Respondents must disclose whether they employ a Chief Diversity Officer, the percentage of gross revenues and overhead paid to New York State-certified minority- and women-owned business enterprises (MWBEs), technical training provided to MWBEs, participation in government mentor-protégé programs, quantitative utilization goals for MWBEs in non-government procurements, formal MWBE supplier diversity programs, and plans to partner or subcontract with New York State-certified MWBEs if selected. The MWBE contracting goal for this procurement is 0.00%, and the DBE contracting goal is 0.00%. All information is subject to audit, and fraudulent statements may result in criminal prosecution and debarment.
