About this file
This document is a Contract Amendment for the State of Arizona Department of Child Safety, specifically Amendment #1 to Contract #CTR059732. The amendment involves the Office of Procurement & Contracts located in Phoenix, Arizona, and pertains to a service contract with an unnamed contractor responsible for youth transportation services.
The amendment introduces a new "No-Show" rate of fifty dollars ($50.00) per youth per occurrence when scheduled transportation drives are missed. The contractor is required to wait 30 minutes beyond the scheduled appointment time before marking a youth as a no-show, must attempt to contact the youth or caregiver by phone, notify DCS Permanency and Youth Services staff of non-response, and send a written notification within 24 hours of the scheduled pick-up time. Payment for no-shows is contingent upon following these specified notification procedures. The amendment becomes effective upon full execution in the state's e-procurement system, with all other contract terms remaining unchanged.
