14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible.
The purpose of Amendment 1 is to add specification for mobilization, submittals, Quality control and Pit development. Due to these added specification the solicitation is hereby extended July 5, 2016 to July 14, 2016, 3:00 p.m. Estimated Start work date is July 29, 2016 and Prework July 26, 2016. Contract time remains 120 days.
Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

(a) The Contractor shall assign to this contract the following key personnel: Foreman/Supervisor, QC POC,
(b) During the first ninety (90) days of performance, the Contractor shall make no substitutions of key personnel unless the substitution is necessitated by illness, death, or termination of employment. The Contractor shall notify the Contracting Officer within 15 calendar days after the occurrence of any of these events and provide the information required by paragraph (c) below. After the initial 90-day period, the Contractor shall submit the information required by paragraph (c) to the Contracting Officer at least 15 days prior to making any permanent substitutions.
(c) The Contractor shall provide a detailed explanation of the circumstances necessitating the proposed substitutions, complete resumes for the proposed substitutes, and any additional information requested by the Contracting Officer. Proposed substitutes should have comparable qualifications to those of the persons being replaced. The Contracting Officer will notify the Contractor within 15 calendar days after receipt of all required information of the decision on substitutions. The contract will be modified to reflect any approved changes of key personnel.

QUARRY DEVELOPMENT PROJECT – STONY QUARRY
This item is intended to compensate the Contractor for operations including, but not limited to, those necessary for the movement of personnel, equipment, supplies, and incidentals to the project site; and for any other work and operations which must be performed or costs that must be incurred incident to the initiation of meaningful work at the site and for which payment is not otherwise provided for under the contract.
MEASUREMENT AND PAYMENT
The measurement shall be lump sum for mobilization. Payment shall be as follows:
1. 50% of the lump sum, not to exceed 5% of the original contract amount, will be paid following completion of 5% of the original contract amount not including mobilization and bond premiums.
2. Payment of the remaining portion of the lump sum, up to 10% of the original contract amount, will be paid following completion of 10% of the original contract amount not including mobilization.
3. Any portion of the lump sum in excess of 10% of the original contract amount will be paid after final acceptance.
4. Progress payments for mobilization and preparatory work shall be subject to retainage.
SUBMITTAL PROCEDURES
This Section includes administrative and procedural requirements for submitting Shop Drawings, Product Data, Samples, Test Reports and other miscellaneous submittals. See Table 1 for a summary of required submittals. See other specification section within this package for additional requirements on submittals.
Coordination: Coordinate preparation and processing of submittals with performance of construction activities.
Coordinate transmittal of different types of submittals for related parts of the Work so processing will not be delayed because of need to review submittals concurrently for coordination.
The Contracting Officer (CO) reserves the right to withhold action on a submittal requiring coordination with other submittals until related submittals are received.
Processing Time. Allow additional time if processing must be delayed to permit coordination with subsequent submittals. CO will advise Contractor when a submittal being processed must be delayed for coordination.
If intermediate submittal is necessary, process it in same manner as initial submittal.
No extension of the Contract Time will be authorized because of failure to transmit submittals enough in advance of the Work to permit processing.
Identification: Place a permanent label or title block on each submittal for identification.
Indicate name of firm or entity that prepared each submittal on label or title block.
Provide a space on label or beside title block to record Contractor's review and approval markings and action taken by CO.
Include the following information on label for processing and recording action taken:
Project Name & Contract Number.
Date.
Name and address of Contractor.
Any revision number.
Deviations: Highlight, encircle, or otherwise identify deviations from the Contract Documents on submittals.
Additional Copies: Unless additional copies are required for final submittal, and unless CO observes noncompliance with provisions of the Contract Documents, initial submittal may serve as final submittal.
B1. PRODUCTS
Contractor’s Project Schedule: Contractor shall submit a Proposed Project Schedule for review by the Contracting Officer at Pre-work meeting to be schedule within three days of award. Final schedule will be required 1 day prior to Start date of work.
Test and Inspection Reports: Comply with requirements in Section "Quality Control”.
Material Test Reports: Gradation Reports as work progresses.
B2. SUBMITTAL SCHEDULE – Submittals shall be made as required by and called for in the drawings and specifications. The following table is a summary of the required submittals for the project - the table is to assist the Contractor and may not be all inclusive – additional submittals may be required by specific specifications:

QUARRY DEVELOPMENT PROJECT – STONY QUARRY
QUALITY CONTROL
0. QUALITY CONTROL
The Contractor shall provide and maintain a quality control system that will ensure all services, supplies, and construction work required under this contract conforms to the contract requirements. The Contractor shall perform, or cause to be performed, the sampling, inspection, and testing required to substantiate that all services, supplies, conform to the contract requirements.

C2. QUALITY CONTROL SYSTEM
General: Perform required testing, inspections, sampling, and similar services per direction specified in the contract drawings and specifications and in accordance with established industry standards.
C3. CONTRACTOR QUALITY CONTROL PLAN
At the time of the pre-work conference, the Contractor shall submit for approval a written Contractor Quality Control Plan.
If the plan requires any revisions or corrections, the Contractor shall resubmit the plan prior to starting work on the ground.
The Government reserves the right to require changes in the plan during the contract period as necessary.
No change in the approved plan may be made without written concurrence by the Contracting Officer.
At a minimum, the plan shall include the following:
A list of personnel responsible for quality control and assigned duties. Include each person's qualifications.
A copy of a letter of direction to the Contractor's Quality Control Supervisor outlining assigned duties.
Methods of performing, documenting, and enforcing quality control of all work.
Methods of monitoring and controlling environmental pollution and contamination as required by all applicable regulations and laws.
C4. TEST AND INSPECTION REPORTS
Submit a copy of complete test results no later than three calendar days after the test was performed.
Submit failing test results and proposed remedial actions within 24 hours of noted deficiency.
Testing and Inspection Reports shall include the following:
Date of issue.
Project title and number.
Name, address, and telephone number of testing agency.
Dates and locations of samples, tests, or inspections.
Names of individuals performing tests and inspections.
Reference Specification Section(s).
Complete test or inspection data.
Retesting report if required by CO/COR.
C5. PERMITS, LICENSES, AND CERTIFICATES
For Contracting Officer’s records, submit copies of permits, licenses, certifications, inspection reports, releases, for compliance with standards and regulations relevant to the on performance of the work.
C6. SAMPLING, TESTING, AND CERTIFICATION REQUIREMENTS
Sampling, testing, and Certification requirements and frequency for specific items shall be as specified in the drawings and specification. The following table is a summary of the required sampling, testing, and certification for the project - the table is to assist the Contractor, but may not be all inclusive – additional submittals may be required by specific specification section:

SECTION C. Supplemental Specification
QUARRY DEVELOPMENT PROJECT – STONY QUARRY
PIT DEVELOPMENT PROCEDURES
PART 1 - GENERAL
1.1 SUMMARY
2. This work consists of excavating material from a hillside either by mechanical ripping or blasting, and crushing the material to a required surface course gradation. This work includes clearing and grubbing, on-site hauling, stockpiling, sloping/shaping for drainage and stability, compacting, dewatering/erosion control prevention, waste disposal and finish grading within a rock quarry.
1.2 SUBMITTALS
2. PIT DEVELOPMENT PLAN: A detailed pit development plan-plan of operations shall be prepared by the Contractor prior to starting extraction of material from the existing pit. The development plan shall include, at a minimum, the following details:
1. SITE LAYOUT
Indicate boundaries showing all proposed activities inside the pit Equipment List Stockpile Locations Dust Abatement Procedures (in accordance to local air quality management office) Waste Management Storm Water Management Structures (Storm Water Pollution and Prevention Plan) & Dewatering Procedures Sequence of Operation Site Operating Procedures (including Safety Plan) Spill Response Procedures
2. SITE MONITORING
a. Pit Wall Stability
b. Wildlife Interactions or sightings
c. Contingencies if changes to the original development scenario are required
3. SITE RECLAMATION
a. Overburden and topsoil replacement for site grading and re-contouring
b. Reclamation of natural drainage
c. Slope Reconstruction
d. Removal of trash and temporary storage/structures/equipment/roads
B. QUALITY CONTROL FIELD REPORTS
1. Gradation Sampling: Sampling frequency indicated in Part 3 of this section.
2. Surveyor’s Grade Control field notes
C. SITE SPECIFIC BLASTING PLAN: If the Contractor determines that blasting is the most effective means for material extraction, a blasting plan shall be submitted for review and approval, before drilling operations begin. Include the following information:
1. Designated Blaster-In-Charge and submit evidence of valid state blaster’s license or other license recognized by the state where the project is located.
2. Working procedures and safety precautions for storing, transporting, handling, and detonating procedures.
3. Proposed product selection for both dry and wet holes. Furnish manufacturer’s Material Safety Data Sheets for all explosives, primers, initiators, and other blasting devices.
4. Site Drawings showing a scaled map of the blast area and cross sectional views which indicate beginning and ending stations, free face location, hole spacing, hole diameter, hole depth. Burden, hole inclination, and subdrill depth. Incorporate any known geological information into the drawings.
5. Loading pattern diagram showing the location and amount of each type of explosive to be used in the holes, including primer and initiators and the location, type, and depth of stemming, column heights, and overall powder factor for each type of loading.
6. Delay and initiation diagram showing delay pattern, sequence, and delay times.
1.3 MEASUREMENT AND PAYMENT
A. Measurement shall be in Cubic Yards (CY), measured by cross section of the completed stockpile(s) to the nearest 1.0 cubic yard. Only basaltic material shall be processed and accepted; other materials such as decomposed granite, shall be excavated and will not be accepted for payment. There will be no allowance made for settlement or shrinkage.
B. Any crushed aggregate not meeting the gradation requirements in this section shall be rejected by the Forest Service, or accepted at a reduced unit rate.
C. All other work: There will be no separate measurement or payment for other work (incidental work) outlined in this section and as shown on the drawings. The Contractor shall program those costs into work items specifically listed in the schedule of items.
PART 2 – PRODUCTS
2.1 CRUSHED SURFACE COURSE AGGREGATE
1. Furnish hard, durable particles or fragments of crushed stone, crushed slag, or crushed gravel conforming to the following:

1. The Contractor shall crush all suitable material extracted from the hillside conforming to the gradation below, as determined by AASHTO T-27 & AASHTO T-11.

PART 3 – EXECUTION
3.1 LOCATION, ALIGNMENT AND GRADE
A. The limits of excavation, and cut and fill reference shall be staked out by the contractor. Staking shall be approved by the contracting officer prior to the start of material extraction.
B. A copy of the AutoCAD files shall be made available to the Contractor upon request, for the purposes of developing stakeout points.
3.2 MATERIAL EXTRACTION
A. Clear and Grub all vegetation within the limits of excavation and utilize any organic debris at the end of the project for site reclamation. The removal of soil contaminated with invasive weed seed is required; remove the top 12 inches of overburden within on-site flagged perimeter, area also shown on the drawings. Dispose of contaminated soil at Designated Quarry Disposal Site, shown on the project drawings.
B. Prior to the start of work, the Contractor shall have a surveyor provide stake out points, to delineate the limits of excavation. The stakeout points will be provided by the Forest Service.
C. The Contractor shall determine the most effective means for extracting material from the hillside. If it is determined that the most effective means for material extraction is blasting instead of mechanical ripping, refer to Part 3.4 for details.
D. Excavate into the hillside and harvest all material to the subgrade elevations and grades provided in the drawings. Monitor the excavation to verify that the material being extracted is basaltic in nature; if material consistency changes to decomposed granite or another material, the Contractor shall cease excavation and inform Contracting Officer immediately. Only extracted and processed basalt will be accepted by the Forest Service.
E. The Contractor shall strip any topsoil or overburden deemed not suitable for use in the crusher, where indicated on the ground by Forest Service personnel; stockpile this material for re-use in site reclamation at the end of the project. Other deleterious material extracted from the hillside shall also be stockpiled away from the crushed aggregate stockpiles.
F. Maintain the excavations to guard against and prevent injury to employees and the public, and to reduce the amount of material lost to surface runoff erosion. Cut and fill areas will be stabilized at the end of each day by constructing gentle slopes of 1:1 or flatter.
3.3 PROCESSING & STORAGE
A. Prior to crushing, take the following actions:
1. All organic matter, lumps or balls of clay, and other deleterious matter shall be removed. All unsuitable inorganic material shall be stockpiled onsite and not utilized for crushing.
2. Consolidate existing stockpiles to locations shown on the drawings.
3. At stockpile locations, the existing ground shall be leveled (but still matching the existing grade) of rocky and organic debris.
B. Contractor shall provide for crushing all equipment suitable for crushing material to the required gradation. Rock crushing equipment shall be erected within the borrow pit, and in a manner which will not disrupt overall operations.
C. The contractor shall take a gradation sample, in accordance to ASTM D-422, and have a State Certified Laboratory verify that the material being processed is meeting contract requirements. The Contractor shall sample at the following intervals:
1. At start of production, obtain one sample after the first 100 cubic yards has been processed through the crusher, to verify the crusher is properly functioning. If the gradation of the initial sample is out of specification, the Contractor shall process another 100 cubic yards of aggregate and take another sample to verify compliance. If necessary, the Contractor shall continue taking a sample every 100 cubic yards until the crusher is producing aggregates at the required gradation.
2. After initial sampling has verified crusher is producing aggregates to specification, the Contractor shall take one sample every 600 cubic yards of material processed (verification sample). If verification sample fails, adjust crusher to correct the gradation; process 100 cubic yards of material and take sample to verify gradation; repeat this process until material is being processed to the correct gradation.
3. If Contractor processes less than 600 cubic yards of crushed aggregate per day, the Contractor shall take a minimum of one gradation sample per day.
D. Consolidate processed material into stockpiles, over prepared surfaces (reference Part 3.3) at designated locations inside the quarry. Maintain slopes of the stockpiles to be no steeper than 1.5:1 and 20 feet maximum height. Older stockpiles that were consolidated prior to the start of excavation shall remain segregated from the newer stockpiles, for the purpose of accurately determining the amounts of new material processed.
3.4 BLASTING
A. Before drilling, remove overburden soil and loose rock along the top of the excavation for at least 30 feet beyond the hole drilling limits or to the end of the cut.
B. Cap all holes to prevent unwanted backfill from falling into the holes, and place a stake next to each hole with the hole number and total depth drilled.
C. Use the types of explosives and accessories in accordance to the blasting plan.
D. Free blast holes of obstructions for their entire depth. Place charges without caving in the blast hole wall. Provide stemming for the upper portion of each blast hole with dry sand passing the 3/8” sieve.
E. Follow each blast, stop all work on site and check for misfires before allowing workers to return to the site.
F. Repair or stabilize any fracturing of the final rock face in an approved manner and at no additional costs to the Forest Service.
G. Halt blasting if slopes become unstable, exceed tolerance and creates overhangs, backslope damage occurs, safety to workers and public is jeopardized, property or natural features are endangered, fly rock is generated, or excessive ground or airblast vibrations occur. The Forest Service reserves the right to implement test blasting if any of these conditions occur, and at no additional costs to the Forest Service.
H. Submit drill logs, indicating the following details:
1. Blast map showing designated hole numbers.
2. Individual hole logs completed and signed by the driller that show total depth drilled, depths and descriptions of significant conditions encountered during drilling operations, and the date of drilling.
3.5 STORMWATER MANAGEMENT / DEWATERING
A. Site Development must ensure positive drainage to prevent water from pooling or flooding the pit. The following measures shall be implemented to enhance re-establishment of equilibrium and minimize erosion and ponding water onsite:
1. Minimize the amount of area disturbed during material extraction, processing, and stockpiling.
2. Areas of unexpected settlement shall be filled to re-establish natural contours and eliminate ponding water.
3. Swales and other erosion control measures shall be implemented to re-direct surface runoff inside the excavation and to reduce the amount of erosion throughout the site.
4. Cut and fill slopes shall be laid back at no steeper than 1:1 at the end of each work day.
B. As part of Pit Development plan, a section shall be dedicated to Storm Water Management/ Dewater Procedures. The following items shall be included in the management plan:
1. Proposed locations of drainage and erosion control structures.
2. Design details for proposed drainage structures.
3. Material list of drainage and erosion control materials.
4. Equipment to be used for dewatering pit.
3.6 SITE RECLAMATION
A. After the contracted amount of material has been crushed and accepted by the Forest Service, proceed with reclaiming the site by conducting the following procedures:
1. Dismantle and transport all crushing plant equipment off Forest.
2. Final grade the side slopes and bottom of the excavation to the lines and grades shown on the drawings; re-use any overburden or topsoil stockpiled on the top layer for the finished ground surface. Any rock fractures remaining in the final slopes shall be excavated and the back slopes re-set, if determined to be unsafe by the Contracting Officer.
3. To maintain positive drainage, it may be necessary for additional ground outside of the limits of excavation (as shown on the drawings) to be graded. The Contracting Officer shall make the determination of where additional grading is required to maintain positive drainage.
4. Any areas of disturbance from pit activities shall be treated to the satisfaction of the Forest Service.
5. Install additional drainage structures around stockpiles, as shown on the drawings and determined in the field by the Forest Service. Other drainage structures constructed during the contract may be left in place, as determined by the Forest Service.
B. WASTE DISPOSAL
1. Any material not consisting off basalt (and thus not used for crushed aggregates) that was extracted shall remain onsite and be placed in stockpiles at designated locations in the pit.
2. All weed-contaminated soils shall be stockpiled at the Designated Quarry Site, shown on the project drawings
3. Organic materials, such as stumps, logs, and slash and all other unsuitable material shall be hauled to an approved landfill, or scattered onsite under the coordination with the Contracting Officer.
4. All hazardous materials (contaminated soils caused by fuel or other fluid spillage), garbage, and other refuse generated shall be removed from the project site and legally disposed of Government property in an approved landfill.
