DIVISION 1 - GENERAL REQUIREMENTS

SUMMARY OF WORK - 01010

1.1 - Location

The project is located on the Mesa Ranger District, of the Tonto National Forest, T7N, R8E, Section 36 as shown on the location map included in the package. The site can be accessed via Hwy 87. Exit west onto FR 627 towards the Sunflower administrative site. The project site is near the interesection with FR 25.

1.2 - Site Work

A section of FR 627 has been experiencing significant erosion on the outlet side of a box culvert near the intersection with FR 25. Several years back the Sunflower Fire created erosion issues in the area and this may have been a delayed response to the change in the drainage behaviors.

As you can see from the attached photos, the right side of the box culvert on the outlet side was not constructed with a wing wall. Our assumption is that it was stable enough that the original design didn’t call for it. The direction of flow immediately turns to the left as well.

The objective of this repair is to stabilize the slope, build it back up to the shoulder of the road, remove a portion the existing pavement and subbase and replace it with new subbase and pavement.

The project calls for removal of 100 feet of the paved roadway, with a full width of approximately 32 feet. The removal will be centered on the culvert. The exact location will be marked during the pre-con meeting. The existing pavement that is removed will be hauled off to a landfill that accepts this material that is outside the forest lands.

Construction will consist of placing large Class III rip rap at the base of the outlet slope and backfilling it with the recovered subbase from the roadway where the asphalt was removed. The rip rap will be placed, in the current void, to interlock and will continue up the slope to the midpoint of the box culvert. The remained of the slope will be placed in 12” lifts with water and compaction until the elevation reaches the existing shoulder.

The contractor will prepare the subbase to allow for a new 6” lift of aggregate base to be placed and compacted. Any additional spoils will be place somewhere adjacent to the project as designated by the COR.

01010-2

The 100 foot section of asphalt will be per MAG A standards with a 3” thickness. The transitions into the existing asphalt will be milled down by 1” to provide and overlap.

Tack coats will also be used to create a better seal.

Access to the outlet will be delineated through an existing gate just north of the culvert. A backhoe is the only anticipated equipment to be needed in the outlet area of the culvert. Forest Service will get the requisite clearances to provide this access.

1.3 - Facilities and Services

A construction work camp will not be allowed. A Contractor’s staging and storage area will be allowed where approved by the CO. Transportation to and from the work site for personnel and materials will be at Contractor’s expense.

Electricity and potable water are not available at the site. Water for construction and dust control is not available from National Forest lands. Development, hauling and placement of water shall be considered incidental to other pay items.

1.4 - Material Sources

All materials are available from commercial sources.

1.5 - Subsurface Soils and Investigations

No auger borings were made in the area. Contractor shall undertake any site investigations necessary to ascertain the subsurface conditions.

1.6 - Clean-up and Disposal

Disposal of construction debris and waste shall be off National Forest land and in compliance with local laws regarding disposal of waste material.

1.7 - Precautionary Measures

Protection of Work:

The Contractor shall take precautions necessary to protect all work during the construction period and shall maintain adequate protection during severe weather conditions to prevent damage to the work or materials.

Safety:

The Contractor is responsible for safeguarding the public, the Contractor's employees, existing facilities, and Forest resources from contract operations.

Signs, lights, barricades and other devices, as necessary, shall be erected and maintained during operations and at night and weekends as necessary.

Precautions such as lowering tractor blades and safely storing materials shall be practiced to prevent injury to others in the area.

01010-3

Security:

No security shall be provided by the government. The contractor shall take all precautions deemed necessary to secure the worksite, improvements and equipment. A security trailer with guards shall be permitted.

MEASUREMENT AND PAYMENT - 01103

1.1 – Requirements

Measurement and payment for contract work will be made only for and under those pay items included in the SCHEDULE OF ITEMS. All other work and materials will be considered incidental to and included in the payment of the pay items in the

SCHEDULE OF ITEMS.

1.2 – Payment

One of the following methods of measurement for determining final payment is DESIGNATED on the SCHEDULE OF ITEMS for each pay item.

(a) Designed Quantities (DQ): These quantities denote the final number of units to be paid for under the terms of the contract. They are based upon the original design data available prior to advertising the project. Original design data include the preliminary survey information, design assumptions, calculations, drawings, and the presentation in the contract. Changes in the number of units of the originally contemplated work will not be made unless authorized under one or more of the following conditions:

(1) As a result of changes authorized by the Contracting Officer in the originally contemplated work. Minor changes to adjust balance points, etc., will not be considered changes in quantities.

(2) As a result of the Contracting Officer determining that errors exist in the original design data used to determine designed quantities, that cause a pay item to change by 15 percent.

(3) As a result of the Contractor submitting to the Contracting Officer a written request showing evidence of errors in the original design data used to determine design quantities that cause a pay item total to change by 15 percent or more. The evidence must be verifiable and consist of calculations, drawings, or other data that show how the designed quantity is believed to be in error.

Final payment will be made for the designed quantities at the unit price shown in the SCHEDULE OF ITEMS, subject to the Variation in Quantities Provision.

(b) Actual Quantities (AQ): These quantities are determined from measurements of completed work.

01103-2

(c) Lump Sum Quantities (LSQ): These quantities denote one complete unit of work as required by or described in the contract, including necessary materials, equipment, and labor to complete the job. They will not be measured.

1.3 - Units of Measurement:

Payment will be by units defined and determined according to U.S. Standard measure.
