Maximum mowing height shall be four inches above ground or standing water. Routine operations shall consist of mowing all growth out from centerline to a point at least eight feet beyond shoulder on each side of the road with a heavy duty rotary mower. Roads with ditches will require mowing the front slope and at least one four foot strip on the backslope of the ditch opposite the traveled way, to a maximum width of fourteen feet from shoulder. All mowing heavy equipment shall be operated only within the traveled way. Any additional mowing for safe sight distance at road intersections or blind curves is considered a part of this activity. This activity also includes brushing around road signs, culverts, culvert markers, barrier posts, gates, bridges, and other appurtenances. Brushing shall be to a point one foot beyond appurtenance and then tapered back to the normal mowing width. Contractor shall perform plumbing (vertical alignment) of all culvert marker posts.
All rocks, debris or slash (windfall trees or branches) larger than than three inches in diameter within mowing limits shall be removed. Slash may be scattered outside mowing and ditch limits. No slash shall be placed in streams or ditches. All woody material larger than one inch in diameter or two feet in length and any sharp object shall be removed from the traveled way.
Unless approved by the COR, all work shall be completed on each road before moving on to the next road.
Road signs, culverts, culvert markers, barrier posts, gates, and other appurtenances damaged by the contractor's operations shall be repaired or replaced by the contractor at his/her expense.
1.2 – Heavy Brushimg Requirements:
Heavy brushing shall consist of machine cutting of all brush and trees (up to 5 inches dbh) from road shoulder to the original clearing limits or a maximum of 12 feet on both sides of the road. Brushing shall also consist of removing all dead or down trees that lie across the road regardless of dbh. Any vegetation up to 15 feet high that is hanging over the road shall also be removed. Brush and trees shall be cut to within 3 inches of ground level or mower height above standing water in ditches.
Contactor shall include brushing of all plant growth around signs and culverts within designated cutting areas.
All heavy brush and trees shall be cut to fall away from the roadway as much as possible.
Debris cut or thrown by machinery into road ditches shall be removed to outside of ditch and placed in such a way as to not fall or wash back into ditch.
Debris cut or thrown onto the riding surface of roads through the Contractor’s operations shall be removed. This includes all woody vegetation larger than 3 inches in diameter and any other debris that could cause tire damage.
Contractor shall inform the Contracting Officer’s Representative (COR) of his proposed operating schedule on a daily basis to facilitate timely inspections.

SECTION VII
Quality Assurance Surveillance Plan
This Quality Assurance Surveillance Plan (QASP) has been developed pursuant to the requirements of the Performance Work Statement (PWS). This plan sets forth procedures and guidelines that will be used in evaluating the technical performance of the contractor, PURPOSE OF THE QASP
The QASP is intended to accomplish the following:
Define the roles and responsibilities of participating government officials and the contractor;
Define the types of work to be performed;
Describe the evaluation methods that will be employed by the contractor and the government in assessing the contractor’s inspection program and performance; of the tasks
Provide copies of the contractor’s inspection records and Government quality assurance monitoring forms that will be used by the government in documenting and evaluating the contractor’s performance;
Describe the process of performance documentation
The contractor is responsible for performing inspection services pursuant to the inspection clause and for providing and maintaining the inspection forms approved by the government to perform the contractors inspection services. The contractor has represented themselves, to be responsible for meeting the prescribed standards in the statement of work
ROLES AND RESPONSIBILITIES OF GOVERNMENT OFFICIALS
The following government officials will participate in assessing the quality of the contractor’s performance. Their roles and responsibilities are described as follows:
Contracting Officers’ Representative (COR)/Inspector will be responsible for monitoring, assessing, recording, and reporting on the technical performance of the contractor on the schedule of inspections submitted by the contractor and approved by the Government COR/Inspector, if the latter is assigned to the project will either witness the contractors performance of the inspections performed by the contractor or will evaluate the contractors inspection results as to documented findings relative to meeting the standards for each task/subtask within the statement of work. The COR will have primary responsibility for completing “Quality Assurance Monitoring Forms” which will be used to document the inspection and evaluation of the contractor’s work performance.
The contracting officer (CO) will serve as and have overall responsibility for overseeing the contractor’s performance. The CO will also be responsible for the day-to-day monitoring of the contractor’s performance in the areas of contract compliance, contract administration, cost control, and property control; reviewing the COR’s assessment of the contractor’s performance; and resolving all differences between the COR’s version and the contractor’s version.
TYPES OF WORK TO BE PERFORMED/SCHEDULE OF INSPECTIONS
This contract calls for the contractor to perform the following tasks and subtasks and to perform inspections at the completion of the tasks/subtasks as a minimum and or for intermediate inspections as determined at contract award by the COR/CO.
5.1 – Roadside mowing - periodic
5.2 – Equipment – periodic
5.3 – Documentation
5.3.1 Work Plan and Schedule - periodic
5.3.2 Quality Control Plan and Schedule - periodic
5.3.3 Job Completion Report - periodic
5.3.4 Contractor Inspection Records - periodic
METHODS OF SURVEILLANCE
1.The government, through its COR, will be monitoring the contractor’s performance on a continuous basis. The method the government will use is 100% inspection of the contractor’s inspection results. The government COR will perform periodic independent inspections after the contractor has performed their inspection if questions develop regarding the results of the contractor’s inspections. Results of the independent inspections that differ from the results noted by the contractor on their inspections will be assessed against the contractor. The contractor shall be required to rework the effort until the performance standard is achieved. And the added costs of re-inspection charged to the contractor.
2. Inspections will be performed to the schedule submitted by the contractor and approved by the government. Variances from the schedule must be pre-approved by the CO.
QUALITY ASSURANCE FORMS AND REPORT
The COR will judge each event in accordance with the performance standards and performance requirements stated in the Quality/Performance Requirements stated in the contract either in Section 5 of the Statement of Work (SOW) or in Section 7 of the SOW.
The COR must substantiate all tasks which he judges to be indicative of “unacceptable” performance. Performance at the “acceptable” level is expected and need not be substantiated
The contractor shall furnish to the COR copies of all inspection forms generated witnessing inspections performed. The COR will forward copies of all completed QA monitoring forms to the CO and contractor inspection records by close of business on the day the forms were prepared. The contractor is required to respond in writing to any negative QA monitoring form(s) within 5 working days after receipt of the form(s)
ANALYSIS OF SURVEILLANCE RESULTS
The CO will review each QA monitoring form prepared by the COR. When appropriate, the CO may investigate the event further to determine if all the facts and circumstances surrounding the event were considered in the COR’s opinions outlined on the forms. The CO will immediately discuss every event receiving a substandard rating with the contractor to assure that corrective action is promptly initiated.
At the end of each month, the COR will prepare a written report for the CO summarizing the overall result of his surveillance of the contractor’s performance during the previous month. This report will become part of the formal QA documentation.
