PERFORMANCE WORK STATEMENT

GENERAL

Janitorial services for Tusquitee Ranger District

1.1.1 Contract Period - The contractor shall provide any and all labor, equipment, supplies, and services necessary to perform janitorial services at the Tusquitee Ranger District Office located at 123 Woodland Drive, Murphy North Carolina. This statement of work consists of completing the tasks/subtasks in section 5 to the performance requirements stated therein.

1.1.2 Location and Description - The Tusquitee Ranger Station is located at 123 Woodland Drive in Murphy, N.C. The office complex consists of three separate buildings that contain office spaces. The main office is a one story structure with approximately 4,000 square feet and two sets of men’s/women’s restrooms and one small kitchen area. The work center building contains three office spaces and one single uni-sex restroom (only the office spaces and restroom are to be cleaned under this contract). The Seed Orchard/Conference room building has approximately 1,200 square feet and consists of two office spaces, a conference room, and one uni-sex restroom. (Only the office spaces and restroom are to be cleaned under this contract). All three buildings have entrance walkways, sidewalks, and/or accessible ramps that are also included in this contract.

1.1.3 Contractor’s Conduct – Contractor shall be personally responsible for any breakage that occurs as a result of his/her employee’s conduct while performing cleaning activities. If cleaning is done after regular business hours, contractor shall keep all keys/security codes secure and buildings will be only accessible to contractor or his/her employees. Contractor will maintain the security of all the facilities at all times and will insure that facilities are properly secured after each work shift. The contractor shall arrange for supervision of the work at all times. The Contractor or Contractor Designated Supervisor shall be available at all times when the contract work is in progress to receive notices, reports, or requests from the COR.

1.1.4 – Inspection and Acceptance – Inspection of work performed will be accomplished weekly by the COR or designated project inspector. It is recommended that the Contractor or his/her representative attend inspections periodically, but it is not mandatory.

1.1.5 –Payment – The Contractor will submit a monthly invoice at the end of each service month to the COR or Business Manager/Support Services. Payments shall be made on a calendar month (30 days) basis in arrears. Upon receipt of the Contractor’s invoice, an IAS receipt will be prepared and faxed to the National Finance Center for payment. Payments will be made 30 calendar days from the acceptance date in accordance with the Prompt Payment Act.

Final payment will be withheld until the Contractor removes his/her equipment and returns all Forest Service keys.

1.1.6 Safety and Accident Protection – The Contractor shall comply with all applicable Federal, State, and local laws, ordinances, and regulations governing safety, health, and sanitation. The Contractor shall provide all safeguards, safety devices, and protective equipment, and shall take other needed actions, on contractor’s own responsibility that are reasonably necessary to protect the life and health of employees on the job and the safety of the public, and to protect property in connection with the performance of the work covered by the contract. All Federal buildings are smoke-free and no smoking will be allowed by contractor or their employees in any building at any time.

SECTION II

DEFINITIONS

For: Janitorial services for Tusquitee Ranger District TERM: Safe

MEANING: A condition free of hazards and not having a potential for personal injury or harm.

TERM: Clean MEANING: Free of impurities or foreign matter not normally a part of the original component and if necessary, eliminating objectionable odors.

TERM: Sanitary MEANING: Promoting healthful conditions by the elimination of dirt, agents of disease or infection through ventilation, cleaning with disinfectants, or disposal of wastes.

TERM: Scrub

MEANING: The use of brushes, stiff bristle type, with soap and clean water or other approved cleaning materials to produce a clean surface.

SECTION III

GOVERNMENT FURNISHED PROPERTY, MATERIALS, SERVICES

For: Janitorial services for TUSQUITEE RANGER DISTRICT

3.1.1 GENERAL – All basic supplies such as vacuum cleaners, mops, brooms, paper towels, toilet tissue, trash bags, bleach, soap, gloves, and sanitizing products will be provided by the government. Any additional supplies needed to meet the performance requirements will/may be provided by the contractor. Any cleaning materials provided by the contractor to be used in meeting the performance standards of this contract shall be a product specifically manufactured for the task to be performed.

The contractor shall place specified signs when cleaning. Every reasonable effort is to be made to keep restrooms open at all times. Work/caution signs etc. will be provided by the government. Contractor shall notify COR of the need for replacement of cleaning supplies and equipment as the need arises.

SECTION IV

CONTRACTOR FURNISHED CRITICAL ITEMS /SERVICES

SECTION V

PERFORMANCE WORK STATEMENT TASKS AND CRITICAL SUBTASKS

For: Janitorial services for Tusquitee Ranger District

5.1.1 – Cleaning Office Buildings – The Contractor shall perform complete custodial operations as specified to insure the office facilities are maintained in a safe, attractive, clean, and sanitary manner according to the cleaning frequency specified. All work is to be done in accordance with the Quality/performance requirements identified below.

5.1.2 - Frequency/time of Services - Perform janitorial services three days per week (Monday, Wednesday, and Friday) for the main office building, and once a week (Wednesday) for the work center offices and seed orchard offices and conference room. All restrooms located in these buildings are to be cleaned three days per week. All work is to be done during normal office open hours, which are typically 8:00 a.m. until 4:30 p.m. Monday through Friday. No work is to be done on designated Federal Holidays.

5.1.3 – Altering Time of Services - Contractor may request permission to perform work after normal business hours but there is no guarantee that this request will be granted. Granting this request will only be upon the concurrence and approval of the Contracting Officer and the District Line Officer based on security considerations for these Federal facilities. If approval is granted for working and being in the facilities outside normal hours the contractor shall:

1) Protect Government property, buildings, materials, equipment, supplies, records, and data against unauthorized access, loss, or damage.

2) Ensure that all lights are turned off upon completion of work and heating/cooling thermostats remain as set by the government.

3) Check all doors, windows, and gates to be sure they are secure and locked before leaving. Report any unusual or suspicious activity in the parking lots to the COR or law enforcement personnel.

4) Contractor shall perform services to provide for the safety and protection of personnel and property against injury, molestation, loss, and/or damage from any preventable cause, including but not limited to fire, theft, espionage, and sabotage.

5) Make a daily tour on each workday to insure than all machines and appliances are shut off or disconnected.

6) Establish a system for custodial personnel to report potentially hazardous conditions and/or maintenance and repair needs of the facilities.

7) In the event of emergency or unusual occurrences adversely affecting the interest of the Government, summon appropriate assistance such as the fire/police department, and immediately thereafter notify the COR.

8) Keys to gates and outside doors, as well as security codes for alarm/security systems will only be issued to the Contractor or the Contractor’s designated employees and these keys/security codes will be safeguarded at all times.

Quality/performance standards:

Restrooms:

All restrooms must be thoroughly cleaned so that they present a clean, sanitary appearance, and are virtually odorless.

A. Sweep floor and dust walls/ceilings for cobwebs.

B. Empty waste receptacles, replace with new liner as needed, and place bagged trash in the provided dumpster.

C. Clean toilet partitions, dispensers, waste receptacles and doors.

D. Sanitize/clean toilet bowls, seats, and urinals.

E. Clean/sanitize lavatories, and counter tops.

F. Replenish toilet paper, paper towels, and soap dispensers.

G. Clean hardware (push/pull plates, bars, kick plates, etc.)

H. Clean mirrors.

I. Mop floors (includes scrubbing of floors as needed).

J. Sanitize/clean water fountains.

K. Report any facility problems (such as electrical or plumbing problems) as soon as possible to Forest Service personnel.

Office Spaces/Common Spaces: All office spaces and common spaces must be cleaned so that they present a clean, sanitary appearance. Work papers, supplies, equipment etc. on individual desktops are not to be disturbed and if individual office spaces are occupied during the hours contractor is working, the contractor shall coordinate cleaning with the occupant or by-pass that office for the day.

A. Vacuum carpets weekly or more often if needed.

B. Sweep/mop non-carpeted floors as needed.

C. Dust all available surface areas such as counters, table tops, desk tops etc.

D. Dust window sills weekly.

E. Clean kitchen areas each scheduled work day. .

F. Empty all waste receptacles each scheduled work day, replace bags as needed, and put all trash into provided dumpster.

Outside sidewalks, ramps, and walkways:

A. Sweep in accordance with the general cleaning schedule for that particular building.

5.2 Documentation

5.2.1 Contractor Inspection Records – The contractor shall perform inspections and reports daily for all tasks and shall keep a daily logbook of completed work items for each building. Copies of the daily log entries will be submitted at the end of each month.

Quality/performance standards Inspection reports are submitted monthly by keeping the daily logbooks current.

SECTION VII

QUALITY ASSURANCE PLAN

1. Introduction

Purpose of this contact is to provide procurement and contracting assistance in the pre-award, award and post award functions at Southern Research Station/EAZ AQM.

Bill Champion is the designated Contracting Officer’s Representative. He will have overall responsibility for the day to day administration of the contract including but not limited to inspection. He has the authority to designate Government Inspectors to aid in inspection of the work accomplished during the life of this contract.

2. How to Use This Plan

a. Scheduling – Work to begin within 5 days after receipt of the purchase order and will be accomplished between the hours of 8:00 A.M. to 4:30 P.M. or as agreed upon by the Contracting Officer’s Representative and the Contractor’s Representative and completed within the periods of time specified on the purchase order (see section 5.1.3).

b. Sampling procedures – random and regular inspections and review of work in progress and completed work.

c. Inspection procedures – review of work.

d. Documentation – Daily diaries for contract file; Notice of Non-compliance to contractor for any work less than 100% in compliance with the objectives and cure notices to be issued by Contracting Officer if work is not performed in a timely manner or if work does not comply with contract work statement.
