AGAR 452.216-73 Minimum and Maximum Contract Amounts (FEB 1988) During the period specified in FAR clause 52.216-18, ORDERING, the Government shall place orders totaling a minimum of $500 but not in excess of $30,000 per base period and option year or $120,000 over the lifetime of the contract. FAR 52.216-18 Ordering (OCT 1995)
(a) Any supplies and services to be furnished under this contract shall be ordered by issuance of delivery orders or task orders by the individuals or activities designated in the Schedule. Such orders may be issued from 1 Feb 2009 or date of contract award whichever is later through 31 Jan 2010 for the basic period. Option I: 1 February 2010 through 31 January 2011. Option II: 1 February 2011 through 31 January 2012. Option III: 1 February 2012 through 31 January 2013.
(b) All delivery orders or task orders are subject to the terms and conditions of this contract. In the event of conflict between a delivery order or task order and this contract, the contract shall control.
(c) If mailed, a delivery order or task order is considered "issued" when the Government deposits the order in the mail. Orders may be issued orally, by facsimile, or by electronic commerce methods only if authorized in the Schedule.
FAR 52.216-19 Order Limitations (OCT 1995)
(a) Minimum order. When the Government requires supplies or services covered by this contract in an amount of less than $100.00, the Government is not obligated to purchase, nor is the Contractor obligated to furnish, those supplies or services under the contract.
(b) Maximum order. The Contractor is not obligated to honor—
(1) Any order for a single item in excess of $30,000.
(2) Any order for a combination of items in excess of $30,000
(3) A series of orders from the same ordering office within 5 days that together call for quantities exceeding the limitation in subparagraph (b)(1) or (2) of this section.
(c) If this is a requirements contract (i.e., includes the Requirements clause at subsection 52.216-21 of the Federal Acquisition Regulation (FAR)), the Government is not required to order a part of any one requirement from the Contractor if that requirement exceeds the maximum-order limitations in paragraph (b) of this section.
(d) Notwithstanding paragraphs (b) and (c) of this section, the Contractor shall honor any order exceeding the maximum order limitations in paragraph (b), unless that order (or orders) is returned to the ordering office within 5 days after issuance, with written notice stating the Contractor's intent not to ship the item (or items) called for and the reasons. Upon receiving this notice, the Government may acquire the supplies or services from another source.
FAR 52.216-22 Indefinite Quantity (OCT 1995)
(a) This is an indefinite-quantity contract for the supplies or services specified, and effective for the period stated, in the Schedule. The quantities of supplies and services specified in the Schedule are estimates only and are not purchased by this contract.
(b) Delivery or performance shall be made only as authorized by orders issued in accordance with the Ordering clause. The Contractor shall furnish to the Government, when and if ordered, the supplies or services specified in the Schedule up to and including the quantity designated in the Schedule as the "maximum." The Government shall order at least the quantity of supplies or services designated in the Schedule as the "minimum."
(c) Except for any limitations on quantities in the Order Limitations clause or in the Schedule, there is no limit on the number of orders that may be issued. The Government may issue orders requiring delivery to multiple destinations or performance at multiple locations.
(d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after 31 Jan 2010 for the basic period. Option: 1 February 2010 through 31 January 2011. Option II: 1 February 2011 through 31 January 2012. Option III: 1 February 2012 through 31 January 2013.
FAR 52.217-8 Option to Extend Services (NOV 1999) The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.
FAR 52.217-9 Option to Extend the Term of the Contract (MAR 2000)
(a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension.
FAR 52.232-18 Availability of Funds (APR 1984)
AGAR 452.211-75 Effective Period of Performance (FEB 1988) The effective period of performance of this contract is from 1 February 2008 or date of contract award which is later through 31 January 2010 for the base contract period. Option I: 1 February 2010 through 31 January 2011. Option II: 1 February 2011 through 31 January 2012. Option III: 1 February 2012 through 31 January 2013.
452.215-73 Postaward Conference (NOV 1996) A post award conference with the successful offeror is required. It will be scheduled within 30 days after the date of contract award. The conference will be held at: Bldg 760-15G, Savannah River Site

SAVANNAH RIVER SPECIAL CONTRACT REQUIREMENTS
SITE ACCESS AUTHORIZATION FOR CONTRACTOR AND EMPLOYEES
Contractor, Contractor’s employees, subcontractors and subcontractors’ employees will be required to attend Category II General Employee Training (GET) prior to obtaining a Savannah River Site Access Badge if the personnel have not previously attended this training. Category II GET training is given every Thursday morning at the Jackson, SC Town Hall. GET training consists of watching a one and one-half hour video and taking a test with questions concerning material shown on the video. GET training is an orientation for personnel who will work at the Savannah River Site for more than 10 (ten) consecutive days in a one year time period. After award of the contract coordination for scheduling of the GET training will be performed by the Contracting Officer’s Representative (COR). Normally personnel will go from the GET training to Central Badging (Bldg.703-46A) to obtain their SRS access badge. At the Central Badging, personnel will be required to have two forms of identification (including one form of current picture identification) and a card that will be given to each person upon successful completion of the GET training. The Contractor must complete Section 2 of the OSR form 10-302 for each employee, subcontractor and subcontractor’s employee and submit forms to the COR no later than Monday of the week the GET training is scheduled. The COR will provide the OSR 10-302 forms to the Contractor and will answer questions concerning the completing of the forms and the GET training.
However, a screening process will be performed and those individuals meeting the following conditions will be denied future access to SRS.
1. Those who are currently on probation or parole for a felony (as determined by the state where the conviction occurred).
2. Any applicant for employment at SRS who received a positive drug screen.
3. Any employee who was assigned by his employer to work at the SRS and was fired for possession, trafficking or use of illegal drugs or for being under the influence of drugs while at SRS.
4. Any employee who has fired or quit to avoid being fired from any employment for violation of a contractor or subcontractor policy on the use or abuse of illegal drugs.
5. Any employee who has been denied a DOE access authorization (security clearance) pursuant to the procedures of 10 CFR 710 administrative review.
6. Individuals who have been involuntarily terminated or have voluntarily terminated to avoid involuntary termination from any employment for theft of Government property at the SRS or for a willful violation of contractor policies or security regulations which may jeopardize the security of site operations or the health and safety of other employees or the general public.
7. Any employee who has been fired or quit to avoid being fired from any employment at the SRS for theft of property.
Contractor or their representative shall be notified of employees that will be denied access to SRS and immediately upon notification, Contractor shall remove the employee(s) from SRS and return the site access badge(s) to the Contracting Officer or COR. Contractor shall be responsible for replacing those employee(s) denied access to SRS and denial of access for Contractor’s employee(s) shall not be construed as a suitable excuse for nonperformance of the contract. If the contractor is denied access to SRS the contract may be terminated for default.
Failure of the Contractor to promptly remove employee(s) denied access and to surrender their site access badges to the Contracting Officer or COR may result in termination for default.
SECURITY AND ACCESS INFORMATION FOR DELIVERIES MADE TO THE SITE: All deliveries must enter through S.C. State Highway 19 Aiken/New Ellenton Savannah River Site security entrance barricade. Vehicle operators might be asked to present proof of insurance, a valid state driver’s license, and vehicle registration before entering the site. Moreover, vehicles over a certain size restriction must enter through the Aiken barricade; however, the vehicles may exit through any exit point. Delivery personnel must have access to all compartments of the delivery vehicle and allow security personnel to search the vehicle. All Contractors will require a security badge for entry to the Site. To obtain a badge, the contractor will receive a general visitor/contractor safety/security briefing at the 703-46A badge office. Allow at least 1-2 hours for badge processing. The badge office hours are Monday through Thursday 6:30 a.m. to 4:00 p.m. On Fridays, the badge office closes at 3:00 p.m. Contractors should arrive at least 1 hour prior to closing time to ensure adequate processing time. This briefing will be delivered using the touch screen computer stations in the badge office. Furthermore, contractors could receive a safety/security briefing via a hard copy checklist and manual log at the Aiken/New Ellenton barricade. Also, if applicable, hazard-specific checklists for high and medium hazard work will be utilized and will be subject to focused observations, as appropriate. Contractor must provide current vehicle registration and proof of insurance along with a valid driver’s license to gain entry to the site.
The following are prohibited items on the SRS: Weapons; firearms and simulated firearms; ammunition or incendiary devices with the exception of emergency road flares; explosive materials and devices such as firecrackers; narcotics, illegal drugs, and drug paraphernalia; alcoholic beverages to include non-alcoholic beer; chemical irritants with the exception of small containers of mace carried for personal protection; and stun guns. External fuel tanks and fuel containers shall not be brought onto the site unless required for contract performance.
Due to the point of entry procedures at SRS, the contractor will be required to give a 3 day notice of delivery and provide to designated Forest Service personnel, the cargo delivery driver’s name and company’s name, social security number and a fax copy of his/her driver’s license. (applies if security badge is needed) Two forms of identification (one must be a picture ID) will be required by the driver. The next page lists the acceptable documents for identification purposes. Drivers must be United States Citizens and cannot be parolees or convicted felons. Former SRS employees removed for cause are denied entry to SRS. Delays and/or costs associated with the use of drivers failing to meet these qualifications or for failure to follow the procedures indicated will be born by the contractor.
RECEIVING INFORMATION: Normal receiving hours for the USDA, Forest Service—Savannah River located 1 mile inside the Aiken/New Ellenton barricade are Monday through Friday, 9:00 a.m. to 3:00 p.m., except for Federally recognized holidays. Trucks arriving after the hours indicated will be turned away unless prior arrangements have been made with the point of contact. The Contractor shall pack and mark the shipment in conformance with the carrier requirements, deliver the shipment in good order and condition to the point of delivery specified in the purchase order, be responsible for loss of and/or damage to the goods occurring before receipt and acceptance of the shipment by the consignee and ay all charges to the specified point of delivery. Failure to pass receipt inspection may result in material being returned to contractor. Note: The Contractor must be capable of off-loading material. No loading dock is available.
COMPLIANCE WITH AREA REGULATIONS
The Contractor agrees to comply with all security and safety regulations and requirements of the Government. The Contractor shall be responsible for maintaining satisfactory standards of employee competency, conduct, and integrity and shall be responsible for taking disciplinary action with respect to his/her employees as may be necessary. In the event the Contractor fails to remove any employee whose continued employment is deemed by the Government to be contrary to the public interest, the Government reserves the right to require the Contractor to remove the employee. The Contractor agrees to designate a representative who shall be authorized to receive on behalf of the Contractor any notices and instructions given by the Contracting Officer in regard to performance under this contract and to take such action thereon as is required by the terms of this contract.
The Contractor further agrees that only persons possessing a valid state driver’s license will be permitted to operate motor vehicles engaged in the performance of this contract upon the roads of the SRS. Failure to furnish evidence of such license for an employee driver will result in such employee being denied access to the SRS. In addition, Contractor and all employee drivers shall comply with all applicable state laws regulating traffic on SRS.
SAFETY AND HEALTH
Contractor shall take all reasonable precautions in the performance of the work under this contract to protect the safety and health of employees, subcontractors, and members of the public and shall comply with all applicable safety and health regulations and requirements of the Department of Energy, OSHA (including reporting), and the USDA Forest Service.
Contractor agrees to cooperate in making it possible for the Contracting Officer, Forest Service, and DOE personnel to safely and economically inspect activities under this contract and to conduct their official duties on the contract site and such other places on the SRS where the Contractor, his/her employees, and subcontractors conduct operations under this contract. The Contracting Officer shall notify the Contractor, in writing, of any noncompliance with the provisions of this clause and the corrective action to be taken. After receipt of such notice, the Contractor shall immediately take such corrective action. In the event that the Contractor fails to comply with said regulations or requirements, the Contracting Officer may, without prejudice to any other legal or contractual rights, issue an order stopping all or any part of the work; thereafter, a start order for resumption of the work may be issued at the discretion of the Contracting Officer. The Contractor shall make no claim for an extension of time or for compensation of damage by reason of or in connection with such work stoppage. In the absence of the Contracting Officer, the COR, the inspector, or any DOE or Government employee has the authority to stop all or any part of the work being conducted by the Contractor if it is determined that any unsafe condition is occurring that threatens life or limb. If such action is taken, the Government employee must notify the Contracting Officer or COR immediately at 725-0239. If the COR or inspector is at the work site, the Government employee must promptly notify the COR or inspector of the action taken and the unsafe condition observed. The resulting formal contract action will be directed by the Contracting Officer.
Operating supplies and maintenance responsibilities: Contractor shall furnish all fuel, oil, grease and repairs. Contractor shall pay any other expenses incidental to operation of equipment.
Protection of streams, lakes and reservoirs: Contractor shall take sufficient precautions to prevent pollutions of streams, lakes and reservoirs by fuels, oils and other chemicals, silt or other harmful materials. Contractor shall schedule and conduct contract in such a manner so as to avoid silting of streams, lakes and reservoirs, and loss of embankment materials during high flow periods. All streams shall be kept free of contractor- generated debris.
Erosion: Contractor shall take every precaution necessary to prevent soil erosion on contract areas and access facilities including but not limited to installing and/or maintaining drainage dips and water bars on roads and trails, out-sloping and re-vegetation of areas disturbed by Contractor’s operations.
Final cleanup: Before final acceptance all trash, rubbish, spills, and debris generated by Contractor’s operation shall be properly disposed of from DOE land.
Area cleanup: Contractor shall be responsible for the housekeeping of Contractor’s employees and any approved subcontractors working under this contract. Roads, right-of-ways, timber stands and general areas shall be kept free of disabled or abandoned vehicles and equipment, part of vehicles or equipment, fuel, oil, beverage and lunch containers, and other debris resulting from Contractor’s operations. Contractor shall be responsible for the cleanup of all spills of any amount of petroleum products (i.e. diesel, oil, and coolant). This includes excavation and proper disposal of contaminated soils in a certified landfill or contaminated soil disposal facility. Contractor shall be required to provide documented proof of such disposal.
Liabilities: Contractor shall assume responsibilities and be liable for all damages to persons or property incurred in or resulting from Contractor’s negligence and/or willful acts in conducting the operations required under this contract. Contractor shall hold the Government harmless for any damages to property and/or injury to persons which may arise or result from the operations required hereof.
Fire protection: Contractor and all approved subcontractors under Contractor shall exercise due diligence, and take all reasonable and practicable action, both independently and in cooperation with the Government to prevent and suppress forest fires on the contract areas and vicinity. A minimum of one axe and one shovel shall be available in the immediate vicinity of the work areas. Gasoline or diesel powered equipment shall be equipped with satisfactory mufflers or spark arrestors. Contractor will not be allowed to start any warming fires on the SRS and agrees to reimburse the Government for the cost of fire suppression incurred as the result of Contractor’s negligence and/or willful acts. All operations on the SRS may be suspended by the Contracting Officer during periods of extreme fire danger as determined by the Forest Service-Savannah River’s Fire Department or when ongoing fires warrant such suspension. Such suspension shall be effected by written notice to the Contractor except in cases of sudden emergency when such notice may be given verbally. In the event of such suspension, the Contractor agrees to remove all personnel and equipment immediately as requested by the Contracting Officer. The Government will not be liable for any damages or claims of any nature resulting from suspension.
QUARANTINE RESTRICTIONS
The Department of Energy’s Savannah River Site is infected with imported fire ants and is under quarantine; therefore, the Contractor shall comply with U.S. Department of Agriculture (USDA) and South Carolina Plant Pest Regulatory Service restrictions to prevent the spread of the fire ants until they can be eliminated.
Quarantine restrictions apply to movement of articles that may carry fire ants; such as, used soil moving equipment, soil plants with roots, grass sod, and hay and straw except hay and straw used for bedding and packing. The restrictions are imposed on the movement of the regulated articles from an infested area to non-infested areas. Movement of the regulated articles within the infested areas is not restricted. Geographical areas under quarantine for imported fire ants are shown on an Attachment in Section J.
Cleaning and processing procedures are required before moving restricted material to a non-infested area. Earth moving equipment shall be cleaned of all loose non-compacted soil before moving. Facilities are available for this cleaning at the Savannah River Site.
Restricted material shall contain a certification from the USDA Animal and Plant Inspection Service (APHIS) that the material is pest free before moving from an infested to a non-infested area.

REQUIRED INSURANCE (If Contract award amount over $100,000) In accordance with FAR 52.228-05 Insurance – Work on a Government Installation, the following information on required insurance levels is provided.
Workmen’s Compensation and Employer’s Liability Insurance that provides coverage for employees working in the State of South Carolina (if required by law) $100,000.00

LOSS OF TIMBER SALE LOCK KEY OR LOCK OR SECURITY BADGES
When necessary Forest Service personnel will place Timber Sale locks on exterior and/or interior gates for areas where Contractor will be working. Contractor will be issued one or more keys for these Timber Sale locks to allow the Contractor access to the areas. When a key(s) and/or locks are lost or misplaced, DOE Security Regulations require the Forest Service to remove all current Timber Sale locks and replace these locks immediately.
If the Contractor or contractor’s employee(s) loses a Timber Sale key(s) and/or lock the loss shall be reported to the COR or Contracting Officer immediately. When the loss is due to the Contractor’s or contractor’s employee(s) fault or negligence, the Contractor shall be charged the costs that the Government incurs in removing all current Timber Sale locks, placing new locks on all required gates, collecting all keys for the locks that are removed, and issuing keys for the new locks. This charge will be approximately $1,935.00 and will be deducted from any payments due the Contractor including final payment.
A security badge is issued to the Contractor and each of the contractor’s employees to allow them access to SRS. When a security badge is lost or misplaced the Contractor or his/her representative shall report the loss to the COR or Contracting Officer immediately and obtain a replacement badge for the lost or misplaced badge. At the completion of the contract, all security badges must be returned to the Government. A deduction of $100 will be made on the final payment for each security badge that is not returned.
Final payments will not be processed until all Timber Sale keys and security badges are either returned to the Government or determined to be lost. When a Timber Sale key(s) and security badge(s) are determined to be lost prior to final payment appropriate deductions will be made from the final payment for this contract.
MOTORCYCLE HELMETS
Any person operating a motorcycle within the boundaries of Savannah River Site is required to wear a DOT approved helmet. This includes any 2, 3, or 4 wheeled type motorcycles. Motorcyclists can still operate their motorcycles without a helmet on the three publically accessible hi ways, like 125. Motorcyclists will be turned away at the barricades.
ARCHAEOLOGICAL OR HISTORIC SITES (AGAR 452.236-73)(FEB 1988)
If a previously unidentified archaeological or historic site(s) is encountered, the Contractor shall discontinue work in the general area of the site(s) and notify the Contracting Officer immediately.
CRIMINAL LIABILITY
It is understood that disclosure of any classified information relating to the work or services ordered hereunder to any person not entitled to receive it or failure to safeguard any classified information that may come to the USFS or any person under the USFS control in connection with work under this Agreement, may subject USFS, its agents, employees, or subcontractors to criminal liability under the laws of the United States. (See the Atomic Energy Act of 1954, as amended, 42 U.S.C. 2011 et seq.; 18 U.S.C. 793 and 794; and Executive Order 12356)
SUSPENSION OF WORK
The Department of Energy operates the Savannah River Site. Sometimes due to safety, security and other reasons, the Contracting Officer may order the Contractor to suspend or delay work. FAR Clause 52.242-14 terms and conditions will apply if a work suspension/delay is ordered by the Contracting Officer.
HOURS OF OPERATION.
Normal operating hours are 0700 to 1700, Monday through Friday. The contractor shall perform services required under this contract during normal operating hours excluding the following recognized Federal Holidays

If work outside the normal operating hours is necessary, approval from the COR is required with at least 48 hours notice.
SAFETY REQUIREMENTS
The Contractor shall furnish all necessary personnel, material, equipment, services, supervision, transportation, and supplies (except as otherwise specified) to perform the attached statement of work/specifications.
The Contractor shall supply the following safety items (not an all inclusive list) and ensure the use of them as required for the duration of this contract. The Contractor shall be responsible for complying with OSHA 29 CFR 1910 General Industry and 29 CFR 1926 Construction, as applicable for work being performed.
A. Rolling stock equipment – All equipment in use with this contract must meet OSHA standards. Rollover protection shall be labeled as follows:
1. Manufacturer’s or fabricator’s name and address
2. ROPS model number, if any
3. Dozer make, model, or series number that the structure is designed to fit
4. ROPS model was tested in accordance with the requirements of OSHA subpart 1928.52.
B. Personal Protective Equipment (PPE) – Contractor shall provide the following PPE, as needed, to include documented training on the use and care of:
1. Hardhats
2. Eye protection
3. Hearing protection
4. safety boots (over the ankle—suitable for the work to be performed)
5. Respirators (as needed) and document complying with OSHA requirements
6. Chainsaw chaps (when using chainsaws)
C. Other safety equipment/supplies
1. Fire extinguisher (5 lb. ABC type) on each piece of equipment.
2. Seatbelts on equipment
3. Communication site (2-way radio, CB or cellular phone with site alphanumeric pager) at each worksite at all times
4. First aid kit (clean and serviceable) at each worksite at all times
5. Back-up alarms (on heavy equipment with rear vision obstructed)
D. Portable Fuel Tank
1. Proper venting
2. Self-closing dispenser
3. “NO SMOKING” and “DIESEL” or “GASOLINE” signs affixed and clearly visible
4. Safety fuel cans labeled and serviceable
E. Contractor shall have a written comprehensive Safety Plan as well as a Job Hazard Analysis (JHA) that will address the specific hazards and controls for each phase of the work. A copy of both the Safety Plan and JHA’s shall be provided to the COR prior to starting work, and be available at each work site.
F. Contractor shall have a written Hazard Communications, and Bloodborne Pathogen Plans available at each worksite. Plans should include documented training for each crew member. The Hazard Communication Plan shall include current material safety data sheets (MSDS) for each chemical in use. In addition, as required by OSHA regulations, one supervising crew member shall have up to date certified First Aid and CPR training. It is strongly recommended to have an individual trained in First Aid and CPR even if not required by OSHA due to the remote duty locations sometimes involved when working at the Savannah River Site. Be prepared to bring a copy of the Safety Plan, JHA, Hazard Communications, and Bloodborne Pathogen Plan to any post award conference.
G. Contractor shall provide documented training for all equipment operators.
H. The Contractor shall not use power line Right-of-Ways (ROW) for staging, loading/unloading, parking or turn around for equipment and trucks. The COR shall be present for any work required within or adjacent to power line ROW.
The contractor shall inform all employees and sub-contractors of the requirements concerning power line ROW.
When using dump trucks to place material on roadways that cross under power lines, within 25 feet laterally of lines, trucks will end dump moving away from lines. Trucks shall not, in any circumstance, operate lift beds within 25 feet of lines.
Vehicles with lift bodies or booms shall not have the bodies/booms operated within ROW. Drivers need to know the total height of vehicles to ensure safe passage under power lines. Minimum distances are 20 feet from ground supports (including poles, guys, towers and other structures) and 12 feet vertical clearance from over head power lines.
I. REMOTE WORKER POLICY: The Contractor shall be required to comply with the USDA Forest Service – Savannah River’s “Field Worker and Contractor Tracking System.” This system aids in the location of personnel in the case of an emergency; such as, severe weather, fire, personal injury, or other threats. The system will be explained at the Pre-Work Meeting. Contractor will be issued a Site pager that will provide emergency information to the contractor, such as, severe weather alerts, etc. The Site pager will be issued at the Pre-construction meeting and must remain with the crew working on the contract at all times that the crew is on SRS. It is the contractor’s responsibility to request additional pagers to ensure employees working alone or away from the work crew are equipped with a site pager.
A Remote Worker is defined as - any worker within the SRS boundaries who is beyond range of an installed Safety Alarm System. This includes persons in transit via any site roadway to and from buildings, personnel involved in outside exercise activities, and field personnel.
All remote workers will check out and in with either Forest Service - Savannah River Operations Center (FS-SROC) or the Savannah River Site Operations Center (SRSOC) as appropriate. The FS-SROC is responsible for monitoring remote employees checked out during normal work hours. The work hours will be provided after contract award. The SRSOC will monitor remote employees checked out before or after FS-SROC regular work hours. Example—Contractors will initiate remote work notification procedures prior to departing the USFS-SR compound and prior to opening an outer perimeter site security gate.
SRS may dictate that the remote worker forfeits their badge if not in compliance with SRS Remote Worker policies.
All affected personnel changing location to include facilities, road way work areas, or field sites will check in with dispatch immediately and update location or travel information.
J.
All Terrain Vehicles/Off Highway Vehicles
The utilization of standard 4 wheel All Terrain Vehicles (ATV) is prohibited USFS-SR policy. The use of an Off Highway Vehicle (OHV) is permitted. However, the contractor must comply with Forest Service safety guidelines during the use of this equipment. (PPE, JHA, tailgate safety briefings, documented training, etc.). OHVs will not be used for cross country travel. All riders will wear a seatbelt, wear safety glasses for non windshield equipped OHV, wear hard hat, riders are not permitted to ride except in seatbelt provided seat.
POWER LINE TWO BARRIER PROCEDURES:
· Job Hazard Analysis (JHA) currently establish site requirements while working near energized electrical power lines, and include the following:
· No parking under power lines
· Do not turn vehicles around under power lines.
· No staging, loading of equipment or vehicles in concerned area.
· Dump trucks working near power lines will only dump while moving away from power lines.
· Dump trucks should not in any circumstance operate lift beds within 20 feet of power lines. However if road work in this area is required, maintain 20 foot clearance laterally of power lines.
· Identify the requirement to utilize at least two of the listed barrier options.
· Documented tailgate site specific safety briefings detailing each task associated hazards with mitigation requirements are conducted.
The following work principles near overhead powerlines are currently used.
· Work areas are walked down by planners and workers to identify overhead line hazards.
· Consistent with OSHA requirements, the first preference shall always be to de-energize the line.
· If there are circumstances that preclude de-energizing the line, those reasons are documented in the work planning process and approved at the appropriate management level.
· Equipment controls used to raise and lower truck beds, excavator booms, etc. will be tagged-out while traversing under effected work site power lines. The vehicle operator is responsible to attach and remove the tag; the spotter is to verify tagged-out compliance.
· At least two of the following two barriers options will be utilized:
· Trained spotters – spotters trained in the techniques of visually determining standoff distance and in communication with the operator.
· Physical boundaries – barriers that prevent the equipment from intruding within the standoff distance.
· Demarcation lines – stakes or pained lines that provide constant reminders to equipment operators of the proximity.
· Measurement of the overhead line and equipment clearance – utilizing remote, not direct, measurement techniques to determine the actual clearance distance.
· Use of reflective materials – enhancing visual identification of spotters of equipment operators.
· Equipment clearance-a horizontal distance of 20 feet from the powerline for equipment operations.
· Documentation of the work planning process and approved at the appropriate management level.
PERSONAL PROTECTIVE EQUIPMENT (FOREST SERVICE)
1. The contractor will train in the safe operation and use of equipment to all workers using such equipment.
2. Personal protective equipment, including personal protective equipment for eyes, face, head, and extremities shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards or processes encountered in a manner capable of causing injury or impairment in the function of any part of the body.
3. Defective or damaged personal protective equipment shall not be used.
4. The contractor shall provide, at no cost to the employee, to each employee who operates a chain saw, chain saw chaps and assure the chaps are worn during saw operations. These chaps must be approved by Underwriters Laboratory or meet Forest Service specification 6170-47. The chaps shall cover the full length of the thigh to the top of the boot on each leg.
5. The contractor shall assure that each employee wears foot protection that provides adequate traction and ankle support. Employees operating chain saws shall wear foot protection that is constructed with cut-resistant material which will protect the employee against contact with a running chain saw.
6. The contractor shall provide, at no cost to the employee, and assure that each employee who works in an area where there is potential for head injury from falling objects or flying objects wears a hard hat that meets the minimum requirements of American National Standards Institute (ANSI) standard Z89.1-1997.
7. The contractor shall provide, at no cost to the employee, eye protection where there is potential for eye injury due to flying objects. This eye protection must meet the minimum requirements of ANSI standard Z87.1-2003. (Added by USFS-SR—Eye protection will be worn during all chainsaw operations)
8. The contractor shall provide, at no cost to the employee, hearing protection where there is a potential for hearing loss due to high intensity noise e.g., chain saw operation.
(If conflict between SRS and Forest Service Safety standards, the stricter standards will apply)
