PACKAGE 1: EXPERIENCE QUESTIONNAIRE

OFFERORS must complete and submit this Experience Questionnaire to receive full consideration for award. Complete this Questionnaire in its entirety or provide a separate proposal that addresses these questions. Copies of company literature, brochures, or other information may be attached to the questionnaire, if appropriate. Extensive, elaborate and specially prepared attachments are neither required nor desired.

The intent of this contract is for licensed and bonded General Contractors to perform road construction projects. The Contractor shall be able to perform all phases of work which may include, but is not be limited to: ability to read and understand architectural and engineering plans; perform new road construction, reconstruction, and restoration; decommission roads;

construct steel, concrete, and timber road bridges; construct trail bridges (at trailheads accessible by road building equipment); Burned Area Emergency Response (BAER) requirements;

Emergency Relief for Federally Owned (ERFO) roads requirements; culverts, aquatic passage structures, and ancillary structures; trailhead construction, trail construction, or other roadbed construction; erosion work for roads, stream banks, and lake shore erosion work; and other requirements cited under Divisions 100 through 700 of the FP-03 and FP-14 (Section C-5).

Offerors may qualify their proposal to limit the types/sizes/values of work they are capable of providing. Please be certain you have accurately assessed your BONDING capacity and FINANCING capabilities when completing these qualifications (see Factor 1a).

Attention: Before answering this Questionnaire refer to Solicitation Section L-11 Request for Proposal Requirements, L-12 Experience Questionnaire (Package #1), M-1 Evaluation of Proposals, Tables M-1a and M-1b, M-2 Evaluation Procedures / Award Determination as well as all solicitation Sections to ensure your answers capture the elements that will be evaluated by the Forest Service’s Technical Evaluation Board (TEB) during the source selection process.

FACTOR 1 - TECHNICAL CAPABILITY

General Technical Capability for Road Construction Activities: Briefly, explain your understanding of the work as described by the specifications, past experience, and production rates for the work areas/ disciplines listed below. Also, indicate if you typically perform this type of work with your own workforce and equipment or if you typically subcontract this type of work:

Subfactor 1b – Key Personnel: Identify key project managers by name and provide their experience with road construction projects and related License(s). Failure to submit résumés shall result in the offeror receiving a lower rating (attach resumes to this Questionnaire):

Subfactor 1c – Quality Control: Explain your company’s capabilities to provide project management, on-site supervision, conflict resolution, analysis techniques, and quality control procedures:

FACTOR 2 – Relevant Past Performance: Provide a listing of your work for the past three (3) years that is most closely related to road construction. Include the dates of performance, who you worked for, the name of their contact person and their phone number (refer to Section M for additional information). If your firm has no relevant past performance over the past three years it will receive a “NEUTRAL” rating for this Factor (refer to Table M-2):

FACTOR 3 – Cost/Price Evaluation: Response not required on Questionnaire. This factor will be evaluated based on your company’s Individual Project Task Order proposal(s).

FACTOR 4 – Use of Biobased Items: List your company’s experience in complying with USDA’s BioPreferred Program and cite the related contract numbers, products used, and dollar amounts. Also explain your intended use of biopreferred products if you are selected for this procurement:
