14. DESCRIPTION OF AMENDMENT/MODIFICATION (Organized by UCF section headings, including solicitation/contract subject matter where feasible) John Day Helibase Type I Construction and Site Work—Malheur National Forest This amendment captures the notes from the site visit and also makes some changes to the initial solicitation and specifications. The Davis-Bacon wage determination has been updated and this solicitation reflects the new wage determination (dated 8/29/2014). Please replace original solicitation, bidder’s packet, specifications, and wage determination. All changes to existing documents have vertical lines in the left margin.
The date that offers are due has been changed to September 11, 2014 by 1200 noon PST at the location listed in block 6 of this form.
By signature in Block 15B Contractor releases the Government from all claims relating to this modification. This settlement is complete accord and satisfaction. Except as provided herein, all terms and conditions of the document referenced in Item 9A or 10A, as heretofore changed, remains unchanged and in full force and effect.

NOTES FROM SITE VISIT
Date and Time of Site Visit: August 20, 2014 at 1000
Location: Drove from Malheur National Forest Supervisor’s Office to Helipad site
Attendance:

Eric Bush—Forest Service Airport Manager
Contractors: Rise & Run Construction; Iron Triangle; McKenzie Cascade; Evergreen Remodeling; Moreno & Nelson; Palmer Exc, Inc.
Discussions:
1) We are deleting the supply and installation of lights from this project because the lights that were specified did not meet safety standards.
2) The newly constructed pad needs to be flat and have no lips. Contractor shall adjust the location of the new helipad to be in line with other two existing helipads. The new helipad shall be at least 110’ from center-to-center to the nearest helipad. Government will stake out the center of the new location.
3) There will need to be some grading work done. All three pads shall be graded per specifications. There shall be a 5% slope out from each pad in all directions unless otherwise approved. Contractor is responsible for furnishing all necessary aggregate to adequately rock all of the regraded sections away from helipads and transitions. All Contractor provided rock shall be similar to the rock that is already at the site. There shall be no lips between pads and regraded sections.
4) The sidewalks need to be at least 3’ minimum at the 9” depth around the entire perimeter before the Contractor has the option to decrease to a minimum depth of 6 inches.
5) Contractor shall be responsible for painting and grading all three pads per specifications. Paint shall be US Technical Coatings (USTC), TTp-1952B, Regular Dry Waterborne Traffic Marking Paint, White (or equivalent). Contractor shall apply at least two coats or as many as necessary until complete coverage is achieved from all directions with a minimum of 15 mils wet thickness. Contractor shall apply per manufacturer’s recommentation.
6) The Contractor’s work shall be completed no later than May 1, 2015 regardless of start time.
7) There are old modular footings and stem walls remains under the ground near where the new helipad will be constructed. These footings will make the area unsuitable for compaction. These footings and other remains shall be removed at the Contractor’s expense. Any extra fill material necessary shall be provided by the Contractor.
8) We will delete the reference to Montana in the specifications. This was an oversight.
Questions and Answers:
1) Question: Will the compaction testing be required for all grading and who is responsible for testing?
Answer: No testing is called out for final grading for regrading away from slopes, but there is compaction testing called out for embankment construction. Contractor shall compact regraded sections outside of helipads by adjusting the moisture content of the backfill material to a moisture content suitable for compaction. Contractor shall compact each 6 inch maximum depth layer with a minimum of three complete passes until there is no visible evidence of further consolidation. Contractor’s compaction equipment shall be approved by the CO or a designated representative. Contractor shall be responsible for and shall bear all costs associated with testing.
2) Question: Does the existing pad have any footings?
Answer: The Government is not aware of any, but the understructure is unknown and the Contractor shall prepare its proposal accordingly.
3) Question: There is reference to dipping rebar under the Minor Concrete specifications. Are Contractors required to dip the rebar?
Answer: There shall be no coating of any rebar required on this project. The specification that references this is in Part 2.1 E in the “Minor Concrete” specifications. The specification is actually talking about dowels of which there are none in this project. No zinc or tar coating shall be required on any rebar for this project.
