About this file

This document is an Affirmation and Disclosure Form associated with a state contract involving the Ohio Department of Transportation (ODOT), executed by a contractor located in Muncie, Indiana. The form specifically addresses compliance with Executive Order 2022-02D, which prohibits state agencies from purchasing services from Russian companies or institutions, and extends previous restrictions on offshore services. The contractor, identified as being from Muncie, Indiana, affirms that all services will be performed within the United States and provides details about their business and service locations.

The contract appears to involve services related to snow and ice management, with the contractor's service location listed at 2400 N. Priority Way in Yorktown, Indiana.
