About this file
This is Addendum 1 to a Colorado Request for Proposal for a Healthcare Organization to Operate Skilled Nursing Home Placement Beds (GovTribe ID 2027000018). The addendum provides clarifications on contractor responsibilities and operational requirements for the nursing home placement program. The contractor is expected to provide or arrange mental health and psychiatry support services, including counselors, therapists, and psychiatrists, either through in-house staffing or contracted agreements with community providers. The program will serve adults over age 25 with primarily neurocognitive disorders, mental illness, intellectual and developmental disabilities, traumatic brain injuries, and dementia, with nursing needs varying based on primary diagnosis. Referrals are made directly to the Office of Community and Family Mental Health (OCFMH), which serves as the primary decision-maker for resident placement based on negotiated contracting criteria, though exceptions may be made for critical circumstances involving safety concerns or active restraining orders. Residents may also be required to undergo the PASSR (Person-Centered Assessment and Service System Review) process.
The addendum clarifies that contractors may subcontract for substance use disorder support services and that Momentum Program involvement is not required for all residents. Regarding facility configuration, OCFMH expressed a preference for securing and non-secured units under the same roof but indicated openness to placement in two separate communities; similarly, the 12 available beds are preferred to be in a single community, though placement in two communities is acceptable, with placement across more than two communities being less favorable. The addendum does not specify pricing, set-asides, funding amounts, or contract term details.
