About this file

This document is a Program Plan Application Form for Electronic Value Transfer Administration (EVTA) from the New York State Office of General Services (OGS). The form is a detailed template for state and non-state agencies to submit a comprehensive plan for implementing electronic payment processing services. It requires organizations to provide extensive information about their existing payment methods, proposed payment program, transaction details, projected costs, and potential benefits of implementing electronic payment services.

The application covers multiple aspects of payment processing, including transaction types (face-to-face, internet, phone, mobile), payment methods (credit/debit cards, ACH), and implementation strategies. Agencies must outline their plan for transaction processing, customer benefits, security measures, and financial projections. The document mandates agencies provide specifics on transaction volumes, projected costs, potential savings, and net financial impact. The ultimate goal is to obtain approval from OGS EVTA (and the Division of Budget for state agencies) to utilize state-wide payment processing service contracts, with the approved application serving as authorization for implementing electronic payment programs.
