Scope of Work Lake Mead National Recreation Area

Rehab Quarters #553A, #504, and #410

Contract Requirements

The contractor shall provide all equipment, materials and labor necessary to replace building housing components throughout the three park housing units: Quarters #553A @ Callville Bay, Quarters #504 @ Las Vegas Bay, and Quarters 410 @ Boulder Beach.

(All three locations are on the Nevada side of the park)

Background and location of the Housing Units:

• Housing unit #553A is a 1,112 square foot, two (2) bedroom, one (2) bathroom, single family housing unit (duplex) that was built in 1991. Lat/Long 36.140979, - 114.729001

• Housing unit #504 is a 1,112 square foot, two (2) bedroom, one (2) bathroom, single family housing unit that was built in 1992. Lat/long 36.118491, - 114.869882

• Housing unit #410 is a 1,440 square foot, three (3) bedroom, one (1) bathroom, single family housing unit that was built in 1960. Lat/long 36.045012, - 114.814074

(copy and paste the Lat/Long into google to give you the location of where the housing units are located in the park. Housing units don’t have physical addresses that you can use)

Quarters # 553A @ Callville Bay:

The work consists of, but not limited to, supplying and installing:

1. Kitchen Counter Tops - Remove and replace the kitchen counter tops with new laminate and backsplash. Contractor will provide at least x3 examples for the COR to pick from before install.

2. Kitchen Sink/Faucet -Remove and replace kitchen sink, faucet, and garbage disposal with new stainless steel. This includes new water shut off values, supply line and drain assembly.

3. Kitchen Light – Remove light fixture and replace with new LED light fixture.

4. Bi-fold Doors – Remove (x2) bi fold doors and replace with new bi-fold doors.

This included the track system. The new bifold doors shall be painted to a semi-gloss white paint. This includes the frame around each of the doors.

5. Mixing values in Laundry Room- Replace water values with new and the casing box around the values.

6. Ceiling/Swamp Cooler Vent – Remove popcorn ceiling and retexture approximately 805 square feet interior ceiling. Retexture the interior ceiling to standard knockdown texture (beside the kitchen and both bathrooms ceilings) Ceiling shall be prepared properly before painting and the paint shell be eggshell type paint. Remove the swamp cooler vent and patch hole to match new ceiling.

7. Closet Doors – Replace (x5) siding closet doors and track system throughout the house. The new closet doors shall be painted to a semi-gloss white paint.

This includes the frame around each of the doors. The new doors shall be 6 panel wood doors.

8. Replace Bathroom, Bedroom, closet doors – Replace the doors: (x2) bathroom doors, (x2) bedroom doors, and (x2) small closet door. The new doors shall be 6 panel wood doors and shall be painted to a semi-gloss white paint.

This includes painting the frame around each of the doors and including handle hardware, hinges, doors stops that are attached to the baseboard. Handle hardware and hinges color shall be stainless steel and will be lever action. (Knob handles and Polished Brass will NOT be accepted) *Contractor will need to paint the front exterior door and the door that is leading in the garage. This includes there frames as well.

9. Interior Walls- Paint interior walls. Interior walls shall be prepared properly before painting. All holes, scratches, cracks should be patch or filled in before paint is applied. The interior walls shall be painted with a semi-gloss type paint.

Contractor will provide at least x3 examples colors for the COR to pick from before painting. (tan in color)

10. Baseboards- Remove and installed approximately 248 linear feet of new 4-inch-high baseboard throughout the house. This include the closet as well. The new baseboard shall be painted to a semi-gloss white paint before installation and the new base board shall be caulked on the top and bottom. (* Note that these measurements are estimates only. It is the contractor’s responsibility to ensure the accuracy of their measurements)

11. Flooring - Remove all carpet, old floor tile, (approximately 24 square feet by front door), and vinyl flooring. Apply/install new backing board subflooring on existing plywood. Depth of the new sub floor can range from 1/4 inch to 1/2 inch.

Determination will be made once the old floor is removed. Install approximately 1,120 square feet new ceramic 18-inch tiles (tan in color) throughout the whole house. The contractor shall remove the two-bathroom toilets before installing the new tile and shall reinstall the toilets with new wax rings when the new tile is installed. Some of the doors may need to be adjusted due to the new floor. The exterior doors front door and the door leading into the garage may need new weather seals. These doors will need to be airtight. Carpet on the window ledge in the living will be removed and replace with the same new floor tile. Contractor will provide at least x3 examples for the COR to pick from before install.

Contractor does not need to tile the crawl space door. (Note that these measurements are estimates only. It is the contractor’s responsibility to ensure the accuracy of their measurements)

12. Bathroom Sinks, Countertop, Faucets and Drains – Replace (x2) bathroom faucets will new stainless steel. This includes new water shut off values, supply line and drain assembly. Replace x1 bathroom countertop and sink in the non- ADA bathroom.

13. Celling Fans – Replace x3 celling fans throughout the house.

14. Dining Room Light – Replace the dining light.

15. Toilets – Replace x2 new toilets in each of the bathrooms. (This included new values, water lines and wax rings.)

16. Kitchen Range Hood – Replace kitchen vent hood. (Like kind, white in color)

17. Bathroom Exhaust Vents – Replace x2 new bathroom exhausts in each of the bathrooms. Replace with like kind.

18. Smoke Alarms - Replace x1 wire smoke alarm by the front door and x2 battery operated smoke alarms in each of the bedrooms.

19. Blinds – Replace the vertical blinds with new. White in color. (Contractor can use existing track system, rail, and brackets). Need the blinds on the track system to be replace with new.

20. Upper cabinets in kitchen removed- Remove the upper cabinets that is between the living room and kitchen. Contract will need to repair any dry wall to match exist texture in the surrounding area and the same paint that will be used on the ceiling in line #6.

21. Dishwasher – Replace with new dishwasher (like kind with new - white)

22. Water Heater – Replace water heater (electric) located in the garage. This includes the shut off values, water lines, and installed an aluminum type drip pain with drain. The drain should have a line to direct water away from dry wall if it leaks in the future.

23. Replace Oven and Refrigerator Appliances: Replace the oven and refrigerator in the kitchen. (Like kind – White)

Quarters # 504 @ Las Vegas Bay:

The work consists of, but not limited to, supplying and installing:

24. Kitchen Counter Tops - Remove and replace the kitchen counter tops with new laminate and backsplash. Contractor will provide at least x3 examples for the COR to pick from before install.

26. Kitchen Light – Remove the x2 light fixture and replace with x1 new LED light fixture. (*Electrical box may need to be move over to center of opening. It is the contractor’s responsibility to look at before putting a bid in for this item line)

35. Kitchen Range Hood – Replace kitchen vent hood.

36. Bathroom Exhaust Vents – Replace x2 new bathroom exhausts in each of the bathrooms. Replace with like kind light combo and fan.

37. Smoke Alarms - Replace x1 wire smoke alarm by the front door and x2 battery operated smoke alarms in each of the bedrooms.

38. Blinds – Replace the vertical blinds with new. White in color (Contractor can use existing track system, rail, and brackets). Need the blinds on the track system to be replace with new.

39. Upper cabinets in kitchen removed- Remove the upper cabinets that is between the living room and kitchen. Contract will need to repair any dry wall to match exist texture in the surrounding area and the same paint that will be used on the ceiling in line #6.

40. Dishwasher – Replace with new dishwasher (like kind with new - white)

41. Water Heater – Replace water heater (electric) located in the garage. This include the shut off values, water lines, and installed an aluminum type drip pain with drain. The drain should have a line to direct water away from dry wall if it leaks in the future.

42. Garage Door Motor and Weather Seals – Replace the garage door motor with new. Overhead automatic door opener appropriately sized for the subject door (single car garage door). Allowable drive mechanisms include screw or chain. A wired garage door opener shall be installed near the entrance to the garage from the interior of the residence. Two battery-powered garage door opener remote control devices and a single battery-powered keypad garage door opener remote control device shall be provided. The keypad shall be mounted on the exterior of the garage door framing or other appropriate location. A ceiling mounted 120-volt duplex electrical outlet is present in the garage. Garage door will need to be seal of all grabs from the weather and outside rodents/mice. Contractor will install rodent guards which can be held up in the desert heat and UV sun. This new guard will be installed on both sides and on top. (New guards white in color)

43. Sliding Glass Patio Door - Replacing sliding patio door with new sliding, vinyl, insulated glass patio door. (White in color) Contractor is responsible with an stucco or dry wall damage that may be cause.

44. Backyard -Remove old dead grass and wooden rail wood ties in the backyard (Of what is left of grass) and install new 3/8 minus dirt material. (gold in color) Weed barrier fabric will be installed before new dirt material is installed. New material will need to be at least 2 inches deep. The new landscape dirt will be slope/graded away from the housing foundation and back patio. Once the new 3/8 minus dirt materials is installed it will need to wet down with water and a plate compacter machine will be used to compact the new material together.

Contractor may use the water spigots at each of the houses for the water supply.

Contractor will have to remove any weeds, brushes, or vegetation that "may” grow from the schedule site visit to when the contractor receives the notice to proceed from the Contracting Officer. Contractors responsibly for an damage to the irrigation system.

45. Exterior Wall – Repair the exterior stucco wall on the east side of house and paint to match existing. (See picture -color of wall SW 6114 – BAGEL)

46. Exterior Light Fixtures: Replace x3 exterior light fixtures with NON-Photocell.

Brown in color.

47. Exterior Solar Screen Windows: Replace x10 solar screens on the exterior of the house for each window. Frame shall be white, and the fabric should be black.

Quarters # 410 @ Boulder Beach:

A. Demolishing and renovating whole interior of existing housing components and replace with NEW. (Windows, exterior front door, back door slider, wooden framing, and HVAC ducting should not be touch) Contractor will need to demo and removed all flooring, carpet, baseboard, drywall, electrical wires, switches, outlets, sinks, drains, cabinets, HVAC vent registers, skylight, water lines above and below concrete foundation to water meter control box located outside, light fixtures, appliances (oven and fridge like kind – white in color), water valves, countertops, towel hooks, towel bars, exhaust fans, bathroom/kitchen fixtures such as towel and toilet paper holders, garbage disposal, all sewer lines below the concrete foundation to clean out located outside, doors, door frames, thermostat, vertical window blinds, counters tops, ceiling fans, installation, smoke alarms, mirrors, toilet, and any other fixtures etc. Every other component list should be demo and removed. If contractor is not sure, should contract the COR for verification before removal or demo.

B. It’s the contractors responsibly for any damage to the exterior of the house.

Include the windows and backdoor slider. Contractor will need to replace with new with no cost to the government.

C. It’s the contractor’s best interest to take pictures before doing any demo to ensure where all components are located to the interior of the house.

D. Materials will match existing housing unit 409 and 501 at Boulder Beach that was recently renovated. Housing unit will be available to contractor during solicitation/pre-bid site visit and/or during the performance of the contract as a reference. (*May or may not be a tenant living in the house when this happens.)

E. Field Verification: The contractor will field verify existing conditions affecting the work in this contract before ordering materials. The scheduling of field verification site visit(s) shall be coordinated with the Contracting Officer’s Representative

(COR)

F. The interior of house has been tested for Asbestos and the report is attached below to the scope of work (SOW). If the contractor wants more testing to be done, then it’s there responsible to contract vendor to conduct test and their payment for their service.

48. DEMO/REMOVE/Asbestos: Remove all asbestos and or lead base paint according to the attached environmental report. See attached. Demo and remove components according to section “A” above. All materials and items will need to remove off site. (Contractor for the removal of the asbestos needs to be Clark County, Nevada permit process included Nevada License contractor.)

49. ELECTRICAL: Includes, but is not limited to, replacing electric panel with new 200 amp, 20 circuits panel including circuit breakers; replacing branch wiring to outlets, switches, ceiling fans, lights, etc.; replacing ceiling fans with new ceiling fans including light kit; replacing ceiling light fixtures; replacing wall light fixtures;

replacing electrical outlets and covers; replacing GFCI outlets and covers;

replacing ceiling fan switches and covers; replacing light switches and covers;

replacing phone jack and cover; replacing CATV jack and cover; and replacing door chime and bathroom exhaust vent. Install wirer smoke alarms for each bedroom and in the hallway, what shall be wired into the electrical panel. Install

LAkE 225944 Page 7 of 33 new thermostat to existing HVAC unit. Work also includes associated architectural, mechanical, and electrical material, labor, and equipment.

50. PLUMBING: Includes, but is not limited to, replacing all water lines, faucets, water valves to fixtures, (sinks/toilets) shower mixing valves, garbage disposal shower head, toilet all drain assembly’s, exterior water spigots, (which the water lines are feed inside the interior of house), ALL drain sewer lines (under concrete foundation to exterior clean out near water meter outside, cleans outs, and vent to kitchen, bathroom, utility, and water heater, etc. In addition, relocating water heater from living room closet to utility (laundry room) closet. Work also includes associated civil, architectural, mechanical, and electrical material, labor, and equipment.

51. FIRE SPRINKLER SYSTEM and Smoke Alarms: Includes, but is not limited to, designing and installing home fire sprinkler system (wet system). Riser room for this new system shall be located in the utility laundry room closet. The discharge line shall be run behind the washer and dryer and discharge drain line should be out into the carport area. New smoke alarms will be wirer and tied in the electrical panel. Work also includes associated architectural, mechanical, and electrical material, labor, and equipment

52. Drywall, Texture, Insulation, and Paint:

A. Includes, but is not limited to, replacing all wood framing that was broken during the demo according to code.

B. Drywall ceiling and all interior walls with new 1/2-inch thick, standard drywall sheets.

C. Level 3 drywall and knockdown texture throughout the whole house.

Work also includes associated architectural, mechanical, and electrical material, labor, and equipment.

D. Exterior wall insulation with R-13 Rockwool Work also includes associated architectural, mechanical, and electrical material, labor, and equipment.

E. Includes, but is not limited to, painting ALL walls (foyer, dining room, kitchen, living room, closets, and all bedrooms and closets) with latex primer and two (2) coats of semigloss paint. (Tan in color) Ceiling with semigloss paint (White in color) Work also includes associated architectural, mechanical, and material, labor, and equipment. Paint ALL doors and trim with two (2) coats of high-gloss paint. (White in color)

53. Generic Furnishings: Appliances, countertops, cabinets, towel holders and toilet paper holders, window blinds interior, exhaust fan in kitchen, closet shelves/poles and mirrors.

54. Install new baseboards: Includes, but is not limited to, painting baseboard and trim with alkyd primer and two (2) coats of high-gloss paint (white). Installed of new 4-inch-high baseboard throughout the house. This include the closet as well and the new base board shall be caulked on the top and bottom. Installed after

LAkE 225944 Page 8 of 33 the tile has been put in. Work also includes associated architectural, mechanical, and electrical material, labor, and equipment

55. Doors: New doors shall be 6 panel doors (This includes the closet doors as well). Hardware stain steel (No brass – gold), and door stops for each door throughout the whole house.

56. Remove Old Dog Cage, Flower Garden, and Tree in Backyard: Remove old dog cage. Including the fencing, poles and concrete foundation to the cage.

Remove the smaller tree to the right of old dog cage and the old above flower garden.

57. Exterior Doors: Replace x2 exterior doors kitchen and laundry doors that are leading to the carport. Contractor will use existing hardware on new doors and will need to paint the new doors to match existing. This includes the door sweep and weather stripping.

58. Concrete Patio (Front/back Area): Remove existing brick patio and install concrete patio. The new concrete patio should slope away from the house for drainage. It is the contractor’s responsibility to ensure the accuracy of the measurements) Contractor will need to install plastic around the lower edge of the patio so concrete does not splash into exterior paint of house. If concrete is present on the house, it’s the contractor’s responsibility to paint the area to match existing paint. The front door area, the bricks will be removed and replace with concrete.

59. NEW Flooring- Install approximately 1,440 square feet new ceramic 18-inch tiles (tan in color) throughout the whole house. COR will check for any hollow spot in the tile once the new tile is installed. Contractor will be responsible for replacing that piece of tile(s) to ensure there is a good bond from the tile to the foundation.

Contractor will provide at least x3 examples for the COR to pick from before installing.

60. HVAC Replacement (Option Line) Replace HVAC Unit with new. Match existing spec of existing one.

61. Retaining Wall (Option Line) (See example below)

a. Pavestone, Rampart BevF Tan 6 x 16 retaining wall block or equivalent

b. Liquid Nail Landscape block adhesive or equivalent

c. 5/8” crushed rock or equivalent base material

d. The retaining walls shall be constructed in strict accordance with the specifications, recommendations, and instructions of the retaining wall block manufacture.

e. The base of the retaining wall shall be no closer than 4 feet from the existing chain link fencing, but may, depending on the site conditions, be further from the fencing.

f. The retaining wall shall roughly parallel the existing fencing, in straight, as long as practical, runs.

g. Where the retaining walls change direction, all curves shall be uniform and fair.

h. All retaining walls shall be level to within + or – ½ inch from end to end.

i. The top of the walls shall be flat, in one continuous plane.

j. The height of the retaining walls shall be from the base of the slope to the projected level plane of the associated yard.

k. The walls shall be constructed on a level, compacted base of crushed 5/8-inch rock or equivalent fill, roughly 4 to 6 inches in depth and 24 to 28 inches wide.

l. First course of block shall be embedded a minimum of 4 inches below the surrounding grade.

m. The grade at the base of the wall shall not exceed ½ inch per foot of slope away from the wall base.

n. A minimum of 12” of crushed rock shall be placed and compacted immediately behind the wall and separated from the adjacent existing soil by a geotextile fabric.

o. Clean, landscape appropriate fill shall be placed and compacted behind the crushed rock/geotextile fabric, to bring the yards up to a level, smooth, grade.

p. The top course of retaining wall blocks shall be fixed in place with appropriate landscape block adhesive.

q. The top of the wall shall be topped with wall cap block, held in place with landscape adhesive.

r. The yard area behind the retaining wall shall be filled and graded to be smooth, and in plane with the top course of the retaining wall, and to be sloped away from the house for a fall of 6 to 8 inches.

s. At the conclusion of the work, all excess material, debris, trash, associated with the work shall be roved from the Park, and the site left in a neat and clean manner

62. Landscape Material Backyard (Option Line): Install above and below the new retaining wall with new 3/8 minus dirt material. (gold in color - Match what existing color at Quarters #407 and Quarters #408 in the same housing area) Weed barrier fabric will be installed before new dirt material is installed. New material will need to be at least 2 inches deep. The new landscape dirt will be slope/graded away from the housing foundation and back patio. Once the new 3/8 minus dirt materials is installed it will need to wet down with water and a plate compacter machine will be used to compact the new material together. Contractor may use the water spigots at each of the houses for the water supply. Contractor will have to remove any weeds, brushes, or vegetation that "may” grow from the schedule site visit to when the contractor receives the notice to proceed from the Contracting Officer. There is x1 tree stump in the backyard that will need to be removed and any tree roots may need to be grounded down below grade surface before new material can be installed. Contractors responsibly for any damage of utilities in the ground.

63. Irrigation System Front/backyards (Option Line) Remove and dispose of existing old and outdated irrigation systems and replacing with all new components. This includes removing the irrigation control valves, control boxes, control wirer, existing backflow (PVB) devices, irrigation water lines, bubbler heads etc. The old system will need to be remove from the project site and legally dispose of them into a landfill or incinerator outside the park. The new system will

(beside the water lines going to each tree/brush) The new pressure vacuum breaker (PVB) will be brass and will have two shut off valves on the device and union coupling on each end for replacement in the future. (See example below) This new PVB is a testable device that will need to be tested/check annual by our Utility Branch here at Lake Mead. The PVB will need to be installed 12 inches minimum above the highest sprinkler head/bubbler head in use. This new PVB will need to have a control box over it. The control box will need to removal and secure in a way that the wind does not blow it away.) All pipes before and after the PVB will be insulated for below freezing temperatures. (ONLY THE PIPES THAT ARE ABOVE GROUND) All new irrigation lines leading out of the control box will be ¾ inch PVC (polyvinyl chloride pipe –color white – “schedule 40”). This includes all the fixing that will be used to connect the lines to together. This new line will need to be trench below the ground surface approximately 5 – 6 inches below grade. The new bubbler heads going to each tree/bush will have a ½ inch plastic riser coming out of the ground. The new riser will need to be approximately 4- 5 inches above the ground surface. The new bubbler heads will need to be adjustable to change the water flow to each tree/bush. (example picture below) The new irrigation control valves will be above ground valves. (example below) These valves will be above ground anti-siphon valves. Must be installed upright and no valve can be located downstream of the antisiphon valve. The new anti-siphon must be at least 6 inches above the highest sprinkler/bubbler head. These new valves will need to have a control box over it. The control box will need to removal and secure in a way that the wind does not blow it away. The new irrigation control timer will need to be an exterior timer. This timer will need to have an exterior cover plate to protect the control bottoms and screen from the sun UV rays. (example below) Contractor will need to install new control wirer from control box to the timer. May run wirer on the edge of roof line. (Contact COR before installing wirer) The outlet that is powering the timer will need to have an exterior outlet cover installed. The existing timer does not have this cover.

64. INSTALL NEW CHICKEN WIRE MESH) (Option Line) Install all new ½ inch Hardware Cloth galvanized 19-gauge chicken wire mesh around the perimeter of new and existing fence line. The new galvanized mesh needs to be approx. 2 foot high from the ground surface. Contractor will need to use high gauge wire to secure to chain link fence (this galvanized mesh is to keep snake out of the backyards.)

1.1 PERFORMANCE PERIOD: The project schedule is based on calendar days from the period of performance (POP) start date.

The total performance period for this project is 150 calendar days. (5 Months)

1.2 FIELD VERIFICATION

A. The Contractor shall field verify all existing conditions affecting the work in this contract before ordering any materials, equipment, or parts. The scheduling of field verification site visits shall be coordinated with the COR. *(Note that these

1.3 CONTRACTOR'S USE OF PREMISES

A. Contractor shall at all times conduct his operations to ensure the least inconvenience to our park tenants which are employees that work at Lake Mead National Recreation Area.

B. Preservation of Natural Features: Confine all operations to work limits of the project. Prevent damage to natural surroundings. Restore damaged areas, repairing or replacing damaged trees and plants, at no additional expense to the Government.

C. Existing Utilities: Notify Contracting Officer and utility companies of proposed locations and times for interruption of existing utility service, if appropriate.

1. Contractor shall be responsible for locating and preventing damage to known utilities before starting any work or digging. If damage occurs, repair of the utility at no additional expense to the Government.

D. Hauling Restrictions: Comply with all legal load restrictions in the hauling of materials according to the Department of Transportation (DOT).

E. Work Limits: The Contractor shall be confined to conduct his work to job site area and work limits as shown on drawings or approved by the Contracting Officer. Contractor shall be responsible to pay for rehabilitation work for any resource damage outside authorized work limits.

F. Care shall be taken to ensure that fuel, oil and other spills do not occur. If spills occur, cleanup will be completed at no additional expense to the Government.

1.4 DESIGNATED OFFICIALS

1.5 TIME OF CONDUCTING WORK

A. All work will be completed during the hours of 7:00 AM to 5:00 PM local time (Monday – Friday) due to the tenants that are living in the housing area. If the time needs to be changed the contractor will need to get approval from the Lake Mead Housing Program Manager and the Contraction Officer.

1.6 WORK/CONSTRUCTION SCHEDULE

A. The Contractor shall provide a work/construction schedule to the Contractor Officer Representative (COR) and the Contractor Officer (CO) before any work begins. This will give the tenants a heads up on when will begin.

1.7 CLEANING JOB AFTER COMPLETED

A. All driveway, sidewalks, roads, patios, and or porches will be brushed off and/or power washed before leaving the job site. Interior of each of the housing units will be clean as well once final walk though is completed.

ACCIDENT PREVENTION

1.8 DESCRIPTION

A. The work of this section consists of establishing an effective accident prevention program and providing a safe environment for all personnel and visitors.

B. The Contractor shall be responsible for establishing an effective accident prevention program that accounts for all phases of the work and the methods to ensure property protection and safety of the public, National Park Service personnel, and Contractor's employees.

1.9 QUALITY ASSURANCE

A. Qualifications of Employees

1. Contractor shall ensure that employees are physically qualified to perform their assigned duties in a safe manner.

2. Contractor shall not allow employees to work whose ability or alertness is impaired because of drugs, fatigue, illness, intoxication, or other conditions that may expose themselves or others to injury.

3. Coordination procedures. Contractor shall ensure that operators of vehicles, mobile equipment, hoisting equipment, and hazardous plant

LAkE 225944 Page 13 of 33 equipment shall be able to understand signs, signals, and operating instructions, and be capable of operating such equipment safely.

1.10 ACCIDENT REPORTING

A. Reportable Accidents: A reportable accident is defined as death, occupational disease, traumatic injury to employees or the public, property damage by accident in excess of $100, and fires. Within 7 days of a reportable accident, fill out and forward to the Contracting Officer a DI 134 form, which may be obtained from the Contracting Officer.

B. All Other Accidents: The Contractor shall report all other accidents to the Contracting Officer as soon as possible and assist the Contracting Officer and other officials as required in the investigation of the accident.

C. FIRST AID FACILITIES: Provide adequate facilities for the number of employees and the type of work being performed at the site.

D. PERSONNEL PROTECTIVE EQUIPMENT: Meet requirements of NIOSH and MSHA, where applicable, as well as ANSI.

1.11 EXECUTION

A. EMERGENCY INSTRUCTIONS: Post telephone numbers and reporting instructions for ambulance, physician, hospital, fire department, and police in conspicuous locations at the work site.

B. Protective equipment

1. Contractor shall ensure that personal protective equipment is maintained in a serviceable condition.

2. Inspect and maintain other protective equipment and devices before use and on a periodic basis to ensure safe operation.

C. Hard hats and protective equipment areas

1. Hard hats shall be worn at all times in locations and conditions as required by OSHA.

2. It is the Contractor's responsibility to require all those working on or visiting the site to wear appropriate protective equipment at all times as required by OSHA.

D. Training

1. First Aid: Provide adequate training to ensure prompt and efficient first aid.

2. Hazardous Material: Train and instruct each employee exposed to hazardous material in safe and approved methods of handling and storage. Hazardous materials are defined as explosive, flammable, poisonous, corrosive, oxidizing, irritating, or otherwise harmful substances that could cause death or injury.

PROJECT MANAGEMENT AND COORDINATION

1.12 SUMMARY

A. This Section includes administrative provisions for coordinating operations on this project including, but not limited to the following:

1. Supervisory and administrative personnel.

2. Project meetings.

3. Coordination procedures.

1.13 SUPERVISORY AND ADMINISTRATIVE PERSONNEL

A. General: In addition to the project superintendent, the Contractor shall provide other supervisory and administrative personnel as required for proper work performance. This may include special personnel required for coordination of operations with subcontractors and material suppliers.

1.14 PROJECT MEETINGS

A. Submittals required prior to the Pre-performance meeting.

1. Letter designating the Project Superintendent.

2. Proposed work schedule.

3. Satisfactory evidence of liability insurance coverage and workman’s compensation for the Contactor.

B. Pre-construction meeting: Once the contract is awarded, an initial orientation meeting will be scheduled for a time and date that is agreeable to the Government and Contractor and will be conducted over the phone. Lake Mead personnel and Contracting Officer will be present to go over any specific

1. Agenda: Items to be discussed will include but not be limited to:

1. Park Rules and Regulations.

2. General correspondence procedures.

3. Payments to the Contractor.

5. Subcontractors.

6. Documents required under contract.

7. Local emergency phone number.

2. Once the start date has been established, the Contractor shall submit the proposed work schedule to the Government for approval.

1.15 PAYMENTS

A. All payments will be submitted online (IPP system)

SUBMITTALS

1.16 DESCRIPTION

A. The intent of this section is to explain submittal requirements before and during construction.

B. This section also includes the requirements for properly handling

"Substitutions" and "Comparable Products" the Contractor submits as being "equal" to the product that has been specified.

C. The contractor will submit submittals to the COR on the following materials for review before install:

1. x3 Tile Samples

2. x3 Paint Colors Samples

3. x3 Counter tops Samples for color

4. x3 Baseboard Samples

1.17 MODIFICATIONS

A. Any changes to this contract shall be in writing in accordance with FAR clause 52.212-4(c), Contract Terms and Conditions. The contractor should notify the Contracting Officer immediately.

1.18 TEMPORARY CONTROLS

2. HOUSEKEEPING

A. Keep project site neat, orderly, and in a safe condition at all times.

B. Provide enough containers for collecting debris and materials to be recycled.

C. Keep volatile wastes in covered containers.

3. DISPOSAL

A. Unless otherwise specified, all removed material becomes the property of the Contractor and shall be disposed of outside the park. It is the contractor responsivity to provide there or dumpsters. They shall NOT us government dumpsters within the park.

B. Immediately remove hazardous rubbish from project site. Remove all debris from project site and legally dispose of them into a landfill or incinerator, private dumping areas outside the park. Do not burn or bury refuse inside the park.

4. FIRE PROTECTION EQUIPMENT

A. Extinguisher shall have a minimum UL rating of 2-A: 10-B: C. (Should be on the job site at all times for each of the three areas)

5. AIR AND WATER POLLUTION CONTROL

A. Take all necessary reasonable measures to reduce air and water pollution by any material or equipment used during construction.

B. Do not dispose of volatile wastes or oils in storm or sanitary drains.

C. Do not allow waste materials to be washed into streams or bodies of water.

6. WATER/RESTROOMS

A. Potable water is available on site. Make connections to existing facilities as needed. Facilities must be cleaned and maintained in a condition acceptable to the NPS.

C. Toilets/Restrooms: The contractor can use the facilities in each of the housing units they are working on. Once work is complete, it is the contractor’s responsibility to ensure toilets are clean before leaving the site.

A. Traffic Controls: Erect and maintain barricades, lights, danger signals, and warning signs as required for a safe work area.

1. Protect existing site improvements, such as curbs, sidewalks, pavement, and utilities.

2. Maintain access for fire-fighting equipment and access to fire hydrants.

3. Illuminate barricades and obstructions at night.

4. Protect pedestrian traffic when needed.

5. Install Barrier Tape were directed by Contracting Officer.

6. Fence, barricade, or otherwise block off the immediate work area to prevent unauthorized entry.

B. Perform work in a manner that assures the safety and convenience of the public and protects the workers and property adjacent to the project.

1.20 CONTRACTOR USE OF SITE

A. General: Limit use of the site during the construction period to the project area limits as outlined on the attached site map. Do not disturb portions of the site beyond the project area limits.

B. Park Entry: Vehicles associated with the contractor shall be waived entrance fee upon entering in a marked company vehicle or by providing the names of employees entering in personal vehicles. The entrance fee is waived only for the purpose of reporting directly to a specific work location. Any deviation for recreational purposes is subject to payment of required entrance fees

C: Site Access: The contractor shall access the project site as outlined on the attached site map. Do not drive vehicles off road or access non-project areas without permission from the contracting officer or park superintendent.

D. Park Regulations: Ensure all personnel (contractors, subcontractors, and vendors) entering or working in the park are familiar with and obey all park regulations. Law enforcement rangers will intervene if they perceive any unsafe or unlawful activity.

E. No Smoking Policy: Smoking is prohibited inside or outside within 25 feet of any buildings in the park. This applies to existing buildings, buildings under construction, and buildings under renovation. Discarding of tobacco products or debris anywhere other than into designated tobacco receptacles is considered littering and subject to fines.

F. Parking: The contractor shall park vehicles and heavy equipment in designated areas for contractor use or as established at the preconstruction meeting. Vehicles are not permitted within long-term or overnight parking along roads or in associated overlooks / pull-outs.
