About this file

This document is a Sole Source Justification for a federal contract opportunity. It outlines the requirement to provide Accreditation Council for Graduate Medical Education (ACGME) annual accreditation fees for five Graduate Medical Education (GME) programs at Womack Army Medical Center (WAMC) for fiscal year 2024. The total value of the contract is $30,698.75, with a period of performance from January 30, 2024, to January 29, 2025.

The justification cites FAR 13.106-1(b)(1)(i) as the authority, stating that ACGME is the only accrediting body for GME programs in the United States, and there are no competitors. A market survey confirmed that ACGME is the sole provider of these services. The contract was previously awarded to ACGME under a sole source justification for fiscal years 2019-2023. The Department of the Army Medical Command is the federal agency involved in this contract opportunity.

Requirement: New Repeat Pricing: Firm-Fixed Price Time & Materials Cost Funds: OMA Other Funds: DHP Total Dollar Value: $30,698.75

Required delivery date or period of performance: 30 January 2024 – 29 January 2025

Name of Intended Contractor: Accreditation Council for Graduate Medical Education Street Address: 401 North Michigan Avenue Suite 2000 City, State, Zip: Chicago, Illinois 60611

Phone: 1-312-755-5000

3. Description of Supplies/Services: To provide for Accreditation Council for Graduate Medical Education (ACGME) Annual Accreditation fees for five GME programs at WAMC: Family Medicine, Obstetrics/Gynecology, Internal Medicine, General Surgery and Orthopedics for FY24. This service provides continued accreditation for these five programs.

4. Authority Cited:

(a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR

(b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR

Controlled by: USAHCA Controlled by: RHCO-A CUI Category: SP-PROCURE Limited Dissemination Control: FEDCON POC: Cynthia A. Richardson-Rhone / (910) 907-9310

(c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR

(d) FAR 8.405-6(a)(1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR

(e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures.

5. Reason for Authority Cited: Womack Army Medical Center, as designated by Defense Health Agency, is responsible for conducting residency and fellowship programs to train physicians in a variety of specialty programs in support of the DHA/Army mission. To conduct a residency program, all the programs must meet the accreditation standards and undergo a site review to be accredited through the ACGME. WAMC has existing residents in various program year groups that an accreditation by ACGME has already covered. All Army residency and fellowship programs are accredited by ACGME. The ACGME is the single accreditor for GME in the U.S, and there is no competitor currently that evaluates and provides accreditation to physician residency and fellowship programs. ACGME provided a sole source letter explaining they are the only accrediting body in the United States and there are no competitors. (A.02 SS Letter from ACGME) Completion of an ACGME residency or fellowship allows the physician to sit for and obtain specialty board certification. Without this program, WAMC would not be able to provide and conduct residency or fellowship programs for military physicians.

6. Actions to Increase Competition:

A sources sought was posted to the System for Award Management (SAM), the Government Point of Entry on 21 September 2023, no responses from industry were received. In the future as other accreditation services become available, research will be performed to consider other products that meet or exceed the current training requirements. All reasonable resources were used to verify and confirm that this is the only sole source for this service. The ACGME is the single accreditor for GME in the U.S., and there is no competitor currently.

This requirement will be posted on SAM.gov to increase competition for future requirements.

7. Market Survey: Market research revealed that the Accreditation Council for Graduate Medical Education (ACGME) is the only organization that can provide the services needed. Internet searches for accrediting bodies for the residency programs at WAMC resulted in the ACGME being the only organization that accredits ACGME residency programs: Family Medicine, Obstetrics/Gynecology, Internal Medicine, General Surgery and Orthopedics. There are other companies that have equivalent solutions that provide accreditation for programs internationally and in Canada, but do not meet the specific needs of this medical center. Those accreditations are only for programs housed in those countries, not the United States.

8. Procurement History:

Previous Contract or Purchase Order Number: W91YTZ19P0080 for FYs 19-23 or N/A Previously competed? N/A No Yes Previous authority for less than Full & Open Competition: IAW FAR 13.106-1(b)(1)(i) Only one source reasonably available.
