CUI//SP-PROCURE

Control Number: 22-RHCO-A-314

RFQ/RFP #: W91YTZ22Q0096

Limited Sources Justification for Acquisitions under the Simplified Acquisition Threshold

(Simplified Acquisition Threshold is $250K)

1. Contracting Activity:

Regional Health Contracting Activity – Atlantic (RHCO-A) Bragg Contracting Cell 6021 5th Street, Building 1467 Fort Belvoir, VA 22060-5505

2. Description of Action:

Requirement: New Repeat Pricing: Firm-Fixed Price Time & Materials Cost Funds: OMA Other Funds: TARA_____ Total Dollar Value:

Name of Intended Contractor: Simbionix USA (large business).

Street Address: 23500 Mercantile Road City, State, Zip: Beechwood, OH 44122

Phone: 1-800-326-9050

Period of Performance: Simulator must be delivered to Womack Army Medical Center (WAMC) Fort Bragg, NC within 60 days after award of contract. Contract will include 12 months of Platinum Service, and three additional 12 month periods of Platinum Service if the options are exercised.

3. Description of Services: One each Endoscopic (ENDO) Mentor Suite Simulator with GI Package, one each GI10108 GI Skills Training Module, three years of GI10100ADW ENDO Mentor Suite Annual Extended Service

4. Authority Cited:

(a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR

Controlled by: USAHCA CUI Category: SP-PROCURE Limited Dissemination Control: FED ONLY POC: Cynthia Richardson-Rhone (910) 904-9310

(b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR

(c) FAR 8.405-6(a)(1)(i)(A): An urgent and compelling need exists, and following the procedures would result in unacceptable delays; OR

(d) FAR 8.405-6(a)(1)(i)(B): Only one source is capable of providing the supplies or services required at the level of quality required because the supplies or services are unique or highly specialized; OR

(e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures.

5. Reason for Authority Cited: ENDO Mentor is the required simulator to be used for the skills portion of the Fundamentals of Endoscopic Surgery (FES) Certification Exam, which all General Surgery Residents must pass before they can sit for their board exams. WAMC needs to train our Residents on the equipment they will test on. In addition, the medical center intends to become a test center for the FES exam, which the Society of Gastrointestinal and Endoscopic Surgery (SAGES) specifies this simulator is the only one approved for performing the exam. This specific Virtual Reality Surgical simulator has been previously purchased by the Army and is currently is in use in several Army Military Treatment Facilities (MTFs) to include Walter Reed, Madigan and Eisenhower. If WAMC doesn’t purchase the simulator, our resident surgeons will not be prepared to take the exam, thereby causing a lack of service to patients.

6. Actions to Increase Competition: Market Research shows that the simulator can be purchased on GSA eBuy. Therefore no further competition will be used other than GSA eBuy.

7. Market Survey: Market Research conducted in accordance with FAR Part 8 and FAR Part 10.
