(a) FAR 13.106-1(b)(1)(i): Only one source reasonably available (e.g., urgency, exclusive licensing agreement, brand name or industrial mobilization); OR

(b) FAR 13.106-1(b)(1)(ii): Only one source can provide a portion of the purchase that is a particular brand-name item; OR

(e) FAR 8.405-6(a)(1)(i)(C): In the interest of economy and efficiency, the new work is a logical follow-on to an original Federal Supply Schedule order not previously issued under sole-source or limited-sources procedures.

5. Reason for Authority Cited: Alcon Vision, LLC, is the sole sourced manufacturer of an FDA approved Trifocal IOL, PanOptix IOL. This product is also doctor preferred of other ALCON Lenses manufactured by ALCON Laboratories, Inc., Fort Worth, TX 76134-2001. The resulting award will be made on SF 1449, for commercial services. The NAICS for this requirement is 339112.

In order to have access to Alcon Phaco packs and other Alcon products in related to eye surgery, these are required for the Centurion machines. Without these brand-specific packs and any other Alcon items that are crucial to the success of the surgery, we cannot perform cataract surgery. For military and retiree patients this surgery will meet the need for patient care and meet the mission for Womack Army Medical Center. ALCON toric IOLs have the best rotational stability in the market and therefore are most likely to yield consistent results for astigmatism patients. Alcon is currently the only manufacturer of an FDA-approved trifocal IOL which enables patients to see stronger and better which promotes a more-healthy life style. Currently, the Alcon trifocal IOL is the best option available for correction of vision at all distance ranges currently available in the United States. Alcon also manufacturer the preferred 3-piece IOLs which must be available as back-ups lenses for every cataract surgery case. While WAMC does not implant many of these, but we always ensure we have access to them in case they are needed in the event of a complicated cataract surgery. Additionally, the Alcon SN60WF lenses have a blue-light blocking yellow tint which some research suggests may be beneficial to the health of the eye. This product is also preferred in certain patient populations.

Also, for patients that underwent unilateral implantation of an SN60WF IOL in the past, when they require surgery in their second eye, we would like to be able to use a similarly tinted IOL. Historically, the SN60WF has been the most commonly implantation IOL at WAMC for many years.

Moreover, Alcon also provides an intraocular lens implant is an artificial replacement for the lens of your eye. It's part of the surgery to fix cataracts. An intraocular lens implant, or IOL, is made of a clear plastic, and it's about a third the size of a dime. There are several different types: Monofocal IOL: This is the most common. Unlike your natural lens, which can stretch or bend to help your eye focus, this implant stays focused at one fixed distance. If yours focuses at a distance, you might be able to see things far away but need glasses to read or see close up. Multifocal implant: Like glasses with bifocal or progressive lenses, this lens has areas that help you see things at different distances. It could take several months for your brain to adapt so your vision seems natural. Accommodating IOL: This flexible option acts more like your natural lens and focuses at more than one distance. It makes you less likely to need reading glasses. Toric IOL: You'll get this is if you have astigmatism, or a cornea that's more football-shaped than round. This can make vision blurry all over, not just close up or far away. This lens lessens astigmatism so you won't need glasses to correct it after your surgery. The surgeon has determined that these products are the most appropriate treatment to achieve the desired surgical outcome by providing relief for the patient’s chronic back pain. Failure to provide the needed supplies for these surgeries will result in diminished healthcare for the patients.
