9523ZY-16-Q-HRB0218 AMENDMENT A003

CONTINUATION PAGES

RFQ 9523ZY-16-Q-BMP0215 was issued on 04 August 2016.

The quotation due date of 07 September2016, as per Amendment A002, is not extended by this amendment.

Prospective Quoters submitted the following questions concerning the RFQ. The questions and the CFTC’s answers are as follows:

1. QUESTION: Who currently provides EAP services to CFTC? For how many years?

ANSWER: The incumbent Contractor is Employee Services, Inc. The current contract was for a total of five (5) years, which included one (1) base year and four (4) option years.

2. QUESTION: Are there specific areas of enhancement you are seeking at this time?

ANSWER: No.

3. QUESTION: What is the CFTC currently paying for their EAP services on a Per

Employee Per Month basis? What was the total dollar spend for the EAP in 2015?

ANSWER: The CFTC contracted for these services via a firm fixed price contract, not on a per employee basis. The firm fixed price for 2015 was $19,337.50.

4. QUESTION: Can you provide the physical address for each of the CFTC locations, including zip code?

ANSWER: The agency has four (4) offices located throughout the U.S., as outlined below:

• CFTC Headquarters office is located at 1155 21st Street, N.W., Washington, DC 20581

• CFTC Chicago Regional Office is located at 525 West Monroe Street, Suite 1100, Chicago, IL 60661

• CFTC New York Regional Office is located at 140 Broadway, New York, NY 10005

• CFTC Kansas City Regional Office is located at 4900 Main Street, Suite

500, Kansas City, MO 64112

5. QUESTION: Within the Scope of General EAP Services, are childcare and eldercare referral packets currently being sent out within 12 business hours?

9523ZY-16-Q-HRB0218 AMENDMENT A003

ANSWER: All required services are being provided as outlined in the SOW.

6. QUESTION: Often we provide more than 3 referrals, and all referrals are vetted prior to sending, would CFTC consider a window broader than 12 business hours for childcare and eldercare information to be sent to caller, and if so what would be acceptable?

ANSWER: No, the CFTC would not consider a window broader than 12 business hours for childcare and eldercare information to be sent.

7. QUESTION: Within the Scope of General EAP Services you asked that access be provided to financial planners. Is there an expectation that these financial planners would provide tactical investment advice; that is, recommend specific stocks, bonds, mutual funds or insurance policies? This would exceed the scope of EAP Financial programs.

ANSWER: There is no set expectation as to what type of advice the financial planners provide.

8. QUESTION: As a follow-up, our financial program often allows callers more than one consultation, depending on the issue, example budgeting. Also, our financial planners are centralized, and deliver service via phone, will this be acceptable in place of in-person?

ANSWER: Yes, the SOW permits for telephone access to financial planners.

9. QUESTION: Within the Scope of General EAP Services, the legal expectation falls beyond the normal benefit level of EAP Legal support, is this five (5) hour free consultation currently being provided? If yes, is the five (5) hour legal consultation with the current EAP providers own staff attorneys?

ANSWER: The required services as outlined in the SOW are currently being provided. The CFTC does not have information to respond to whether or not the five

(5) hour legal consultation is with the contractors own staff attorneys.

QUESTION: After the “free” consultation it would then appear that the employee/dependent would need to start over with a “local” attorney. Our standard model provides for a free 30 minute consultation with a local attorney (in-person or via phone) and then employee/dependent can retain that same attorney at a discounted rate. Would this be acceptable, and provide us with the same “score” as others that may deliver the service as defined.

9523ZY-16-Q-HRB0218 AMENDMENT A003

ANSWER: No.

10. QUESTION: Regarding Additional EAP Requirements – quarterly on-site workshops. Does the same requirement currently exist in the program design? How many were actually provided in the last full year of the contract?

ANSWER: Yes, 4 sessions were provided.

11. QUESTION: Regarding Administration 1. – Account Manager shall coordinate “counseling sessions”, are you referring to mandatory referrals and/or CISD support only? Or are you also referring to counseling sessions individuals may receive when using the EAP benefit?

ANSWER: The Account Manager shall be responsible for coordinating sessions outlined in the SOW under General EAP Services and also responsible for mandatory referrals and CISD support.

12. QUESTION: Why are you requesting this RFP at this time?

ANSWER: The CFTC is procuring the EAP services at this time because the current contract is expiring on 11/5/2016 and the agency wishes to continue to provide the services to its employees, without a gap in coverage.

13. QUESTION: Can you provide recent utilization reports or provide utilization statistics?

a. Specifically, can you provide total number of initial inquiry/assessment calls to the EAP in 2015? This information is not available.

b. Can you provide the total number of in-person EAP sessions provided in 2015? This information is not available.

c. Can you provide the total number of in-person EAP sessions provided in 2014? This information is not available.

9523ZY-16-Q-HRB0218 AMENDMENT A003

14. QUESTION: Regarding Additional EAP Requirements – CISD support. Do you want a set number of hours to be included in the annual program cost, and if so how many hours? Or, do you prefer that this be provided on a fee for service basis?

ANSWER: The Contractor’s quote should be based on the company standard EAP program to include the standard CISD support provided under the program.

15. QUESTION: With regard to the definition of family member – our definition is typically limited to spouse/domestic partner and dependents of the employee – this could include most of those that you have identified, but in rare occasions would it cover all, example being siblings, grandparents, foster children. We would ask that the inclusion of the word dependent be added to the definition.

ANSWER: Contractors quote should be based on their company definition of family member.

Except as specified in this Amendment A003, all other terms and conditions of this RFQ, as previously amended, remain unchanged.
