• Conduct sampling and analyses of floors and walls for chemical residue left after initial cleaning. Three to four (3-4) samples per room shall be taken dependent upon the number of visible indicators (remaining stains on the floor in front of sinks or fume hoods or remaining stains on walls above sinks.). If the samples indicate contamination remains, but is below the action level, then the area shall be considered clear. If the samples indicate that contamination remains and is above the action level for the chemical tested, then additional cleaning and sampling is required until the space is below the action level.
• Each sample shall be marked with a unique identifier and the sample location. For example, Sample W1 – SPA, west wall, 6 ft. north and 4 ft. up from the SW corner. The floor plan of the USGS space shall be marked with the unique identifier and the information included in the final report. An Adobe pdf file is included at the end of this statement of work, and if necessary, the pdf file can be adjusted to allow for more clear viewing of the sample locations.
• Samples analysis shall be presented in a table format showing the criteria used and a list of the sample analytes against the specified criteria. A table of the waste disposal results shall also be included. Suggested examples of the data to be presented are shown below. The contractor may add additional information if it will enhance or otherwise explain the sample results.
• The contractor shall code each chemical as to its waste characteristics in accordance with EPA Resource Conservation and Recovery Act (RCRA) applicable regulations and segregate into proper compatibility groups for shipping according to applicable Department of Transportation (DOT) regulations and Transportation, Storage, and Disposal Facility (TSDF) guidelines. The carrier shall have the appropriate Hazardous Materials Shipping Permit and provide a copy of the Permit to the Contracting Officer.
• Mere acceptance of the materials and waste at a properly permitted TSDF does not meet the definition of disposal under this contract. It is the contractor’s responsibility to obtain all necessary documentation verifying that the disposal, i.e., incineration, neutralization, destruction, sequestering, etc., of all items has been accomplished. If the TSDF offers documentation verifying disposal of items, the contractor must also obtain that documentation.
140G0321Q0217 Page 11 of 37
• All manifests, bills of lading, certificates of destruction (if a TSDF issues them), sample analysis, and chain-of-custody forms shall be included in the final report. Those documents such as certificates of destruction that are not issued immediately by the TSDF, may be sent to the USGS COR upon arrival.
• The contractor shall retain the services of an independent or third-party industrial hygiene service to collect the required samples. An industrial hygiene technician may collect the samples provided a certified industrial hygienist oversees the technician.
Task 6. The contractor shall provide a draft final report of all work activities within 20 days after the completion of the project.
• The contractor shall include a complete copy of the National Renewal Energy Laboratory Worksheet for Determination of Actionable Surface Contamination Levels and create a table which shows the results of all sampling results compared with the ASCLs. See above for an example of the table.
