SECTION C: DESCRIPTION/SPECIFICATIONS/PERFORMANCE WORK

STATEMENT

C.1 PROGRAM DESCRIPTION AND BACKGROUND

C.1.1 Purpose

The purpose of this contract is to provide support in order to advance and promote the vision, mission, and goals of the United States Public Health Service Commissioned Corps (PHS). This requirement supports Medical examinations for Commissioned Corps Accessions described herein.

C.1.2 Background

The U.S. Public Health Service Commissioned Corps (PHS), within the HHS, is one of the eight uniformed services of the United States. The PHS fills essential public health leadership and service roles within Federal agencies and programs, including the Indian Health Service, Food and Drug Administration, Centers for Disease Control and Prevention, Bureau of Prisons, and Department of Homeland Security. The PHS is a critical national asset that is being modernized and strengthened to best meet the needs of our Nation. The PHS’ mission is to protect, promote, and advance the health and safety of our Nation through: (1) rapid and effective response to public health needs; (2) leadership and excellence in public health practices; and (3) advancement of public health science. Led by the Assistant Secretary for Health, the Office of the Assistant Secretary for Health (OASH) oversees the Office of the Surgeon General (OSG), and Commissioned Corps Headquarters (CCHQ). In order to effectively meet this mission, PHS leadership launched a modernization initiative and developed the following vision for the 21st century that is best aligned with today’s regional, national, and global challenges:

• A highly trained, always-ready, fully deployable national asset to preserve public health and national security during national or global public health emergencies.

• A fundamental instrument of national response for non-emergent, yet critical public health challenges within the United States and territories.

• A provider of direct healthcare, public health leadership, and scientific expertise when specifically needed to fulfill U.S. Government requirements.

• An innovation engine for public health technologies, communications, systems, and systems of systems.

• The PHS must have the right amount of manpower and adequate resources in place at the right levels and with the right skills to meet their mission. This requires an expedited accessioning approach for active duty officers and the Ready Reserve Corps component.

To support these objectives, the goal is to have the ability to complete up to 1,200 medical examinations per year to accession active and reserve Commissioned Corps to achieve the goal of significantly increasing the number of active duty officer and to achieve a Ready Reserve Corps of 2500 officers in five years.

C.2. Performance Work Statement – Severable Services

The Contractor shall furnish all of the necessary personnel, materials, services, facilities, (except as otherwise specified herein), and otherwise do all the things necessary for or incident to the performance of the work as set forth below:

C.2.1 Scope/Objective

The scope and main objective of this requirement is to conduct rapid medical and dental examinations to Commissioned Corp applicants, and to perform pre- and post-deployment health assessments on PHS officers.

C.2.2 Medical Examinations of Applicants for the U.S. Public Health Service Commissioned Corps

(PHS):

The contractor shall perform physical and dental examinations on 1200 applicants/year to the PHS in all 50 States of the United States including Puerto Rico within fifty (50) miles of the applicant’s preferred address using licensed and contractor credentialed healthcare providers at no cost to the applicant. Per the requirements in Commissioned Corps Instruction (CCI) 221.02 and Personnel Operations Memo (POM) 821.76, the physical examination shall include the performance of a complete history and physical examination by obtaining a verified height and weight, determining the applicant’s body mass index (BMI), and if indicated, obtaining a measurement of the applicant’s percent body fat by the “taping” method; the performance of screening vision and hearing examinations; and the collection of blood and urine samples for performance of required laboratory tests by a certified laboratory with results sent to the contractor. The dental examination shall consist of a general dental examination as well as panorex and bite-wing x-rays.

The contractor shall provide a secure on-line digital application, accessible to employees of Commissioned Corps Headquarters (CCHQ) Medical Affairs Branch (MAB) requiring the use of a Personal Identity Verification (PIV) card or Common Access Card (CAC) for identity validation which allows MAB personnel to securely provide applicant names and demographics to the contractor, to view applicant’s appointments and progress through the medical clearance process, to submit orders for supplemental or repeat testing, to view reports of the examinations and lab testing, to create a single PDF file of all records once completed, and to download that file to CCHQ applications.

The contractor shall provide a secure on-line digital application, accessible to PHS applicants using a unique identification number provided by PHS for identity verification which is accessible 24 hours/day and 7 days/week and allows the applicant to create an account with the contractor, digitally sign consents for examinations, and for transmittal of examination results to MAB, complete a screening medical questionnaire with “knock-out” questions provided by MAB, indicate preferences for the time and location of their examinations, to request changes in already established appointments, and to access contractor customer service. The contractor shall also provide a PHS dedicated customer service telephone number.

Once the name of an applicant is received by the contractor and examination providers are identified, the contractor shall contact the applicant within 5 days by any one or all modalities including email, text, or phone call to direct them to the applicant application. The contractor shall continue to contact the applicant every 72 hours until the applicant responds. The contractor shall inform MAB if an applicant has not responded within a 14-day period. Unless the applicant is deemed to be uncooperative after consultation with MAB, the contractor shall ensure that all examinations are completed within 30 days of the initial contact with the applicant.

Once all examinations are completed, the contractor’s clinical personnel shall, within 21 days, review all medical documentation and, based on PHS Medical Accession Standards in CCI 221.01, provide a recommendation as to whether the applicant should be medically cleared. If the contractor’s clinical personnel do not believe that the applicant should be medically cleared, they shall list the medical conditions identified as being disqualifying. The contractor shall then inform MAB that the applicant documents are available for download. The contractor shall provide project management to coordinate with MAB including regularly scheduled weekly meetings. The contractor shall provide reports documenting the status of the medical clearance process for each applicant as well as cumulative data on the total applicants processed, time for completion of examinations, and time for completion of final clinical review.

C.2.3 Deployment Health Assessments:

The contractor shall perform up to 2000 pre- and post-deployment health assessments per year on PHS officers. The assessments shall consist of an online PHS questionnaire followed by a brief telephone conference between the officer and a healthcare professional in the contractor’s call center. The purpose of the telephone conference is to clarify answers to the online questionnaire and to identify any urgent medical or mental health conditions. The online questionnaire must be accessible to PHS officers through a secure digital application 24 hours/day, 7 days/week. The call center must be available to PHS officers for at least 15 hours/day, Monday through Friday, and at least 4 hours/day, Saturday, and Sunday. After completion of the deployment assessment, the contractor’s clinical staff will, based on criteria developed with MAB and the CCHQ Readiness and Deployment Branch (RDB), designate deployment assessments as high priority or routine review by MAB. The contractor will then inform MAB that the assessment is complete and available for downloading from the contractor’s secure application.
