STATEMENT OF WORK

1. Introduction/Overview

1.1 Project title

Cardiothoracic Back-Up Coverage

1.2 Background

The Cardiovascular Intervention Program of the National Heart, Lung, and Blood Institute (NHLBI) of the National Institutes of Health performs seven (7) to fourteen (14) cases annually of Percutaneous Coronary Intervention (PCI) – a form of treatment for stenotic coronary arteries commonly associated with coronary heart disease – on patients in the NIH Clinical Center (CC). Although uncommon, major procedural complications associated with PCI can be life-threatening and require immediate, invasive surgery. Neither the NHLBI nor the CC has a cardiac surgery program capable of responding to such emergencies. For this reason, the CC requires the NHLBI to have an agreement with a local or reginal medical center, which ensures round-the-clock access to a qualified advanced catheterization and cardiothoracic surgery program to perform emergency rescue on NHLBI’s PCI patients.

1.3 Scope

Contractor shall provide round-the-clock catheterization and cardiothoracic surgery back-up coverage for NHLBI patients undergoing PCI treatment at the CC.

1.4 Purpose and objectives

In order to comply with CC policy, the NHLBI requires round-the-clock bailout catheterization and cardiothoracic surgical back-up coverage for its patients undergoing PCI treatment. Should an emergency occur with a patient, the NIH will transport the patient from the CC to Contractor’s facilities where the patient will receive complete medical attention from Contractor’s medical staff, including any emergency surgery necessary to resuscitate the patient.

2. Requirements

2.1 Contractor requirements

Contract shall provide full-time, round-the-clock cardiothoracic surgical back-up coverage for NHLBI’s patients undergoing PCI treatment at the CC. Specifically, Contractor shall ensure a team of qualified medical staff is on-call 24 hours a day, seven days a week, to perform emergency cardiothoracic surgery on NHLBI’s patients at Contractor’s location. The medical staff shall consist of certified physicians including, but not limited to, a cardiothoracic surgeon capable of performing highly complex and invasive surgical and percutaneous procedures necessary to complete medical attention from Contractor’s medical staff including any emergency surgery necessary to resuscitate the patient.

- Applicable percutaneous services include (1) Advanced percutaneous coronary intervention devices, including coronary rotational atherectomy, coronary laser atherectomy, intracoronary ultrasound, coronary optical coherence tomography.

(2) Advanced coronary intervention services, including left main bifurcation stenting, and antegrade and retrograde recanalization of chronic total coronary occlusion. (3) Mechanical circulatory support: Percutaneous left ventricular assist device (LVAD), percutaneous right ventricular assist device (RVAD), both simultaneously, including non-surgical transcaval LVAD. Percutaneous mechanical support must have oxygenator capability (ECMO). (4) Percutaneous large-bore extraction thrombectomy for acute pulmonary thromboembolism.

- Applicable surgical services include (1) Emergency surgical coronary artery bypass grafting after failed percutaneous coronary artery intervention. (2) Emergency surgical repair of cardiac and coronary perforations. (3) Emergency surgical retrieval of embolized cardiac devices and repair of attendant cardiac devices. (4) Emergency surgical pulmonary thromboendarterectomy. (5) Emergency surgical repair of acute valvular insufficiency

2.2 Deliverables/Reporting requirements

The Contractor shall email a written report in PDF format to the Contracting Officer’s Representative (COR) within 48 hours of receiving each case from NIH detailing the events of the case. The report shall contain the following information:

• Current status of the patient;

• Date and time Contractor received the patient from NIH

• Date and time patient was admitted to the treatment/operating room

• Date and time patient was discharged from the medical center

• Contractor’s assessment of the condition of the patient upon receipt of the patient from NIH

• Explanation of all measures taken by the Contractor to resuscitate the patient and the patient’s response to each measure

• Recommendations for the patient’s future medical care (as necessary)

2.3 Schedule

The ordering period of performance for this contract will be for one year from the date of award.

2.4 Government responsibilities

The NIH shall be responsible for coordinating the transportation of the patient(s) from the CC to Contractor’s facilities. The NIH shall alert Contractor immediately of its decisions to transport the patient to Contractor’s facilities for emergency medical attention.

The COR and/or his/her Technical Advisors will also hold a meeting (in person or via teleconference) with Contractor’s surgical staff within 24 hours of each emergency performed to: (1) assess the condition of the patient(s); (2) review the measures taken to resuscitate the patient(s); and (3) discuss recommendations for future medical care (as necessary). Follow-up meetings with the COR and/or his/her Technical Advisors will be held as necessary.

3. Additional considerations

3.1 Reimbursement for invasive and surgical procedures with applicable health insurance

• Insured patients; have their health care insurance charged directly by Contractor for applicable medical, surgical, and transportation fees.

3.2 Reimbursement for invasive and surgical procedures WITHOUT applicable health insurance

• All services shall be provided at Medicare rates (See attached CMS proposed rates)

• Excess LOS shall be charged at average daily ICU rate with a 70% discount (i.e., $9,289 minus 70 percent equals a daily rate of $2,786)

• Services provided are limited to emergent cardiac transfers

• Services shall be billed directly to NHLBI

3.3 Reimbursement for emergency medical transportation services

• NIH shall reimburse Contractor for use of ambulance/helicopter transport if needed.

TECHNICAL EVALUATION CRITERIA

“Cardiothoracic Back-Up Coverage”

1. Technical Evaluation Criteria

The back-up center must possess services not available at NIH Clinical Center to rescue and treat patients with advanced cardiovascular disease. The center must offer breadth of service; continuity of service; and geographic and temporal proximity.

1.1. Breadth of service. Services must include all of the following:

- Percutaneous

- Advanced percutaneous coronary intervention devices, including coronary rotational atherectomy, coronary laser atherectomy, intracoronary ultrasound, coronary optical coherence tomography

- Advanced coronary intervention services, including left main bifurcation stenting, and antegrade and retrograde recanalization of chronic total coronary occlusion

- Mechanical circulatory support: Percutaneous left ventricular assist device (LVAD), percutaneous right ventricular assist device (RVAD), both simultaneously, including non-surgical transcaval LVAD. Percutaneous mechanical support must have oxygenator capability (ECMO).

- Percutaneous large-bore extraction thrombectomy for acute pulmonary thromboembolism.

- Surgical

- Emergency surgical coronary artery bypass grafting after failed percutaneous coronary artery intervention.

- Emergency surgical repair of cardiac and coronary perforations

- Emergency surgical retrieval of embolized cardiac devices and repair of attendant cardiac devices

- Emergency surgical pulmonary thromboendarterectomy

- Emergency surgical repair of acute valvular insufficiency

1.2. Continuity of service

- Due to immediate life-threatening nature of applicable medical and surgical problems, services must be available on all days and at all hours

1.3. Geographic and temporal proximity

- Due to immediate life-threatening nature of applicable medical and surgical problems, and due to the low priority public health systems assign to interhospital transfer, the contracted center must have its own advanced cardiac life support (ACLS) transfer/transportation system.

- The center must allow transport by ground and/or air, from the NIH Clinical Center (CC) located in Bethesda Maryland, to the center within 30 minutes from first contact.

1.4. Personnel

- The proposal includes a list of proposed staff to perform the work referenced in the Statement of Work (SOW). Additionally, the proposal demonstrates that the proposed staff is capable and qualified to perform all of the work referenced in the SOW according to applicable local, state and federal licensure laws and regulations. Personnel evaluation includes experience with specified procedures.

1.5. Facilities and Equipment

- The proposal demonstrates that the offeror possesses and has immediate access to all of the equipment and facilities required to perform the work referenced in the SOW.
