Sol # 693JF718Q000025 Off-Campus Laundry Services Questions & Responses v.1
Question 1) If I have a written proposal, do I submit both Form 1449 completed as instructed and a written proposal too?
Response: Interested vendors can submit either the written proposal and the Form 1449 or the written proposal and their own pricing form.
Question 2) In completing form 1449: block 12, discount terms, what is this referring to?
Response: If a vendor decides to give the government a discount on the product or services, block 12 is where the vendor would write or type the information.
Question 3) On form 1449, block 17, contractor code and facility code, what are these codes and where are they found?
Response: On the Form 1449, Block 17 Contractor code and Facility code are filled in with the contractor who is awarded the contract.
Question 4) On the Solicitation form, Section 4-Solicitation Provisions, Pg. 16 under Acceptable Offerors, where it states if the vendor is using its own proposal letter it may be considered only if, (1) the offeror accepts all terms and conditions of the Request for Quote (RFQ) , and (2) this acceptance is indicated in writing, where is this RFQ and what are the terms and conditions and how to accept them in writing?
Response: Section 4 Solicitation Provisions (under Acceptable Offerors) ensures interested vendors that submit proposals accept all terms and conditions in the solicitation. The terms and conditions can be accepted by signing Form 1449 or the offeror can submit their own form (with acceptance in writing).
Question 5) In completing Form 1449 can it be handwritten or must it be typed?
Response: The Form 1449 can either be handwritten or typed. (Note: If the SF 1449 form is handwritten, please ensure it’s legible).
Question 6) Can the Form 1449 and written proposal be submitted via email only or do I need to also send (2) copies of proposal packages to the office designated in block 9? What are the Representations and Certifications to be enclosed?
Response: The Form 1449 and written proposal can be sent via e-mail. Vendors that submit proposals for this procurement need to ensure the Reps and Certs have been updated. See FAR 52.212-3 and access http://www.sam.gov/portal.
