About this file

This is a Past Performance Survey form utilized by the Philadelphia Housing Authority's Contracts and Procurement Department to evaluate contractor performance on completed projects. The survey collects comprehensive information about past contract performance through evaluations of contractors who have previously worked on government or commercial projects. The form requests detailed project information including client type and contact details, project scope and complexity, end products and results, program development requirements, staffing and planning levels, and timeline adherence. The survey is classified as source selection sensitive and not for public release, indicating its use in the competitive evaluation process for future contract awards.

The evaluation mechanism consists of five performance categories—Quality of Service, Cost Control, Timeliness of Performance, Business Relations, and Customer Satisfaction—each with specific measurable criteria rated on a six-point scale: Excellent, Good, Acceptable, Marginal, Unacceptable, and Not Applicable. Quality of Service encompasses compliance with contract requirements, accuracy of reports, personnel effectiveness, technical excellence, and workmanship standards. Cost Control assesses cost forecasting, billing accuracy, and cost efficiencies. Timeliness evaluates milestone achievement, reliability, and schedule adherence including liquidated damages assessment. Business Relations examines management effectiveness, cooperative behavior, responsiveness, problem notification, flexibility, subcontracting practices, and staff turnover. Customer Satisfaction determines overall recommendation and contractor satisfaction. Reference ratings are combined to generate a final past performance rating that directly influences contract award decisions.
