About this file

This is a Texas Ethics Commission Conflict of Interest Questionnaire (Form CIQ) for vendors doing business with local governmental entities in Texas. The form is required under Chapter 176 of the Local Government Code and must be filed by vendors who have specific business relationships with local government officers or their family members. The questionnaire must be completed and filed with the records administrator of the local governmental entity within seven business days of when the vendor becomes aware of facts requiring disclosure, such as when beginning contract discussions, submitting bids or proposals, or becoming aware of relevant relationships.

The form requires vendors to disclose employment or business relationships with local government officers that result in taxable income exceeding $2,500 during a 12-month period, gifts totaling more than $100 given to officers or family members, and any family relationships with local government officers. Vendors must also identify any business relationships they maintain with corporations where the local government officer serves as an officer, director, or holds ownership interest of one percent or more. Filing this questionnaire is mandatory for qualifying vendors, and knowingly violating the filing requirements constitutes a misdemeanor offense under Texas law.
