About this file
This is a Declaration of Compliance form required for state and local contract opportunities in Texas, establishing legal and ethical requirements that contractors must certify before executing agreements. The document contains multiple compliance affirmations including anti-collusion statements, criminal history notifications per Texas Senate Bill No. 1, federal suspension and debarment verification under Texas Education Agency guidelines, and certifications regarding boycotts of Israel per Section 2270.002 of the Texas Government Code. Additional requirements include compliance with Senate Bill 13 regarding companies that boycott energy companies and Senate Bill 19 concerning discrimination against firearm entities, both applicable to companies with 10 or more full-time employees when contract values exceed $100,000.
The form requires contractors to provide essential business information including company details, authorized signatory information, federal identification numbers, contact information including phone, fax, and email addresses, and both order and remittance addresses. Specific pricing terms, contract amounts, or funding sources are not detailed in this compliance form, as it serves as a prerequisite certification document rather than a procurement specification. The form includes checkboxes for contractors to select applicable exemptions for energy company and firearm industry provisions, with options indicating either compliance with the requirements or verification that the statutory requirements do not apply to their specific contract situation.
