1. PWS Section 1.0, 1.4 is amended as to delete the sentence and replace it with the following below:

The Contractor shall provide services and support, as defined herein, within the geographic region of the continental United States. Geographic regions include the continental United States excluding North Dakota. The contractor must also provide influenza vaccinations in Hawaii, U.S. Virgin Islands, and Puerto Rico.

The Subcontracting Commitment section does not discuss the CO contracting with a SDVOSB to oversee the contract resulting from the RFP. Utilization of a support contractor to assist with determining the contractor’s compliance with the subcontracting commitments incorporated into the contract may include review of some pricing information.

4. PWS Section 3.0, 3.11 is deleted and replaced with:

All reported Veteran safety events shall be investigated, confirmed, and resolved by the Contractor. A summary of findings and conclusions shall be included in the: ROC VHA format and immediately sent to the COR Elizabeth Tyler (Elizabeth.tyler@va.gov).

7. PWS Section 12.0, 12.5 is amended to delete and replace with the following:

The contractor shall provide to the COR a weekly status report of immunization quantities administered for the previous week in Government format. The report should provide at a minimum total number of vaccinations issued weekly and at each of the following levels: store level, Regional area levels, state level, and continuous monthly level. Offerors shall submit their written portions of their proposal in separate volumes: Executive Summary, Technical volume and Price volume. All volumes shall be provided in a 3-ring binder.

All volumes shall consist of a cover, a table of contents and the body of the proposal. Statements paraphrasing the PWS or parts thereof, and phrases such as “standard procedures will be employed” or “well known techniques will be used” will be considered insufficient. For this proposal, the term “offeror” includes all team or subcontractors.

12. E.3 proposal Submission and Contract Information is amended as to delete and replace it with the following:

Introduction. This section contains general proposal submissions instructions as well as specific proposal requirements for the Retail Pharmacy Immunization Program.

General. The proposal shall be presented in sufficient detail to allow Government evaluation of the response to the requirements of the RFP. The Government will not assume the Offeror possesses any capability, understanding, or commitment not specified in the proposal.

Offerors shall provide information by addressing each Factor and Subfactor in the format and sequence identified in the solicitation. The Offeror must provide information in sufficient detail to allow the Government to make an assessment of the Offeror’s capability to support the proposed response to the evaluation criteria. Proposals that do not contain the information requested in the solicitation risk being determined unacceptable by the Government and subsequently unawardable. The Government will not assume the Offeror possesses any capability, understanding, or commitment not specified in the proposal.

The proposal must not merely repeat the solicitation requirements, but rather must provide convincing documentary evidence in support of conclusive statements of how contract requirements will be met.

Alternate Proposals. Alternate proposals will not be accepted. The Government will not evaluate or consider for award any proposal which is based on or proposes substitute terms or conditions or proposes non-compliance with the terms and conditions of the RFP. This is not the same as an Offeror’s assumptions and predicates.

Proposals shall be valid for a minimum of 90 calendar days from the date the RFP closes. A statement regarding the validity period must be included on the first page of the proposal.

Compliance Review. The Government will review the contents of each proposal to ensure it meets the requirements of the solicitation. Proposals which do not comply with the requirements of the solicitation may be eliminated from the evaluation and will not be considered for award.

For any fill-ins that are not applicable, the Offeror must acknowledge by indicating “N/A”.

One hardcopy and one CD copy of the proposal volumes shall be compiled using the Microsoft Office for Windows suite of applications, submitted on a single-write CD-ROM to prevent accidental erasure of the data therein, and formatted for an MS Windows compatible computer. The Price Factor shall be contained on a separate CD from non-price Factors. Price breakdown, when applicable, will include MS Excel spreadsheets with formulas intact. Process flows may be delivered in hard copy if not compatible with the above software. The CD copy must mirror the submitted hard copy. If the CD copy and hardcopy differ, the CD copy supplied for each Factor will be used for the evaluation. Each CD must be clearly marked with the Offeror’s Name and the Solicitation Number. Alternatively, files may be provided in Portable Document Format (.pdf).

Representations through System for Award Management. The purpose of electronic Representations and Certifications is to provide all Offerors with a portal in which to submit Reps/Certs in a publicly accessible format, nullifying the requirement to submit identical information in response to each and every Federal contract solicitation. As a requirement under this solicitation, all Offerors must submit Reps/Certs data into SAM at https://www.sam.gov/. Any Offeror that has previously submitted Reps/Certs data into SAM is reminded to update the data prior to submitting with the proposal.

13. E.4 FORMAT FOR PROPOSALS, is amended as to delete and replace it with the following below:

The offeror’s proposal shall be submitted in severable volumes based on the Factors as set forth below and all information specific to each Factor shall be confined to that volume. Each proposal must include all volumes and shall be submitted by the closing date and time of the solicitation. Failure to provide any volume may result in the proposal being eliminated from the evaluation and not being considered for award. Information provided shall be specific to each Factor.

The proposal shall include the following volumes and page limitation:

Volume I: Technical Factor (Maximum of 50 pages) Volume II: Past Performance Factor (Maximum of 25 pages) Volume III: Price Factor (No Maximum) Volume IV: Solicitation, Offer and Award Documents (No Maximum)

Each volume shall be clearly identified and shall begin at the top of a page. All pages of each volume shall be appropriately numbered. A Table of Contents should be included at the beginning of the proposal.

Each volume shall be as brief as possible, consistent with complete submission. The page count specified in Section E.4 will be made by counting the pages from left to right, consecutively. Pages that exceed the page limitation will not be evaluated. Each paragraph shall be separated by at least one blank line. Annexes, documentation and attachments that are submitted by the Offeror, which are not required as part of the RFP, will count against the page limitations. If pages are printed on both sides, each side will count as a separate page. The following will not count against page limitations: volume title pages, table of contents pages, cross-referencing pages, indices, acronym lists and page dividers (used to separate proposal sections). Foldout pages depicting such items as diagrams, charts, graphs, etc. may be used, with each fold counted as one additional page.

The Offeror is responsible for including sufficient details to permit a complete and accurate evaluation of the proposal. Offerors shall provide a proposal that, at a minimum, addresses those evaluation Factors required in Section E.8 Evaluation Commercial Items.

File Packaging. All of the proposal files shall be compressed (zipped) into one file using WinZip version 20 or later, or as separate uploads in their native format, i.e. .doc or .docx, .xls or.xlsx, .ppt or .pptx, etc. Please note – Self extracting .exe files will not be accepted.

Volume I: Technical Factor All Offerors must provide a Technical Factor Volume to be considered for award. The proposal shall not simply rephrase or restate the Government's requirements, but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Offerors shall assume that the Government has no prior knowledge of their capabilities, and experience.

Offerors are responsible for including enough details (i.e. process flows, diagrams, graphs) to permit a complete and accurate evaluation strictly from a technical standpoint. The proposal shall be detailed and clearly stated to allow an assessment by the Government without the need for additional clarifications. All information and data provided shall be specific to retail pharmacy immunizations, except for the area of Past Performance. In response to the RFP, the Offeror must address the following subfactors:

1: The offeror Shall demonstrates its capability to use the Sequoia Project VLER eHealth Exchange program with PUSH technology capabilities; and Change Health upfront eligibility check.

3: The offeror shall demonstrate the capability of planning and executing an advertising campaign to promote Veteran influenza immunizations.

Offerors shall submit a list of all recent Commercial and Government contracts (prime and major subcontracts) in performance or awarded to the Offeror within the past three years, or awarded earlier than three years ago, but for which performance occurred or was scheduled to occur within the past three years, which are relevant to the effort required by the solicitation. Data concerning the prime Offeror shall be provided first, followed by each proposed major subcontractor, in alphabetical order. Offerors are reminded that both independent data and data provided by the Offerors in their proposals may be used to evaluate the Offerors’ past performance. The Government plans to rely on existing documentation from Federal databases to the maximum extent practicable. Sources of existing information available to the Government include, but are not limited to, CPARS, FAPIIS, and Electronic Subcontract Reporting System (eSRS). The Government may use past performance information obtained from sources other than those identified by the Offeror. The Government reserves the right to use or not use past performance information submitted by the Offeror based on whether or not adequate recorded past performance information is available in its Federal databases. Since the Government may not necessarily interview any or all of the sources provided by the Offerors, it is incumbent upon the Offeror to explain the relevance of the data provided. The burden of providing thorough and complete past performance information remains with the Offeror. Proposals that do not contain the information requested by this paragraph may result in rejection of the offer. This volume shall be organized into the following sections:

(1) Section 1 – Contract Descriptions. This section shall include the following information in the following format, if applicable:

The following factors shall be used to evaluate offers:

Technical Factor: The subfactors below will be used to determine technical acceptability. Failure to meet any criteria will result in rejection of the offer. All subfactors are minimum requirements that must be met to be found technically acceptable. These will be rated as technically acceptable or technically non-acceptable.

Subfactor 1: The Government will verify the Offeror demonstrated its capability to use the Sequoia Project VLER eHealth Exchange program with PUSH technology capabilities;

Subfactor 2: The Government will evaluate the offeror’s ability to demonstrate its capability to send and receive secure encrypted email messages;

SF 1449, Section 8

What time is the RFP due on May 13?

The RFP is due at 3:30 PM Mountain time.

2
B.3–Subcontracting Commitments
Retail chains generally do not subcontract. Does this mean that the CO may contract with an SDVOSB to oversee this contract and that we would have to provide our records to the subcontractor of the VA? Does that include pricing information?
VAAR 852.215-70 is required in RFPs that are not set aside for SDVOSBs or VOSBs such as this. VAAR 852.215-71 is required in RFPs where VAAR 852.215-70 is included.
3
Section 1.0, 1.4

Can the contractor offer shots at locations in Alaska, Hawaii, Puerto Rico, and the U.S. Virgin Islands The Contractor shall provide services and support, as defined herein, within the geographic region of the continental United States. This sentence is incomplete. Please provide the finished sentence.

All reported Veteran safety events shall be investigated, confirmed, and resolved by the Contractor. A summary of findings and conclusions shall be included in the: ROC VHA format and immediately sent to the COR Elizabeth Tyler (Elizabeth.tyler@va.gov)

5
Section 8.0
Please explain what the VHA approved encrypted email means?
Once the contractor is selected, the VA must approve the system the contractor uses to send encrypted messages to the VHA. The COR shall be contacted to approve the system used for encryption.
6
Section 11.0
Can you clarify the use of the expression “Authorized VA Flu Shot Centers”? The locations whereby the VA contractor is authorized to provide influenza shots.

Is there a logo or style guide that accompanies this phrase?

If not, are there rules to consider how to use this phrase?

No, contracting officer must approve all signage advertising messages for this contract; see PWS paragraph 3.4. The contracting officer may work with the COR, communication Director, and VHA staff to determine acceptability of the advertising.

You do not have to use this phrase in all advertising, again the advertising language must be approved by the contracting officer.

As well as report the number of veterans who visit a contractor retail location and did not receive a vaccination because they demand the High Dose or Quadrivalent versions or went to the referred nearest VHA facility.
The contractor shall provide to the COR a weekly status report of immunization quantities administered for the previous week in Government format. The report should provide at a minimum total number of vaccinations issued weekly and at each of the following levels: store level, Regional area levels, state level, and continuous monthly level. 8
C.12 52.227-14 RIGHTS IN DATA—GENERAL (MAY 2014)

Please explain the rationale behind including this provision to the RFP.
