Amendment A00005 for 36C78618R0094 Great Lakes National Cemetery DB 2,000 Phase II RFI Responses Question 1: Will Licenses and Registrations included in Factor 3, Key Personnel, be included in the total page count (15) for this section?

| Question 2: | Please clarify p17 Factor 3, Key Personnel references professional licensing in the state of Alaska…, assumed certifications and licensing should be in the state of Michigan. |
Response 2: Yes, Key Staff professional licensing should be for the state of Michigan.
Question 3: Key personnel listed on p17 and p18 of solicitation may not apply to this scope of work. Please confirm all of these positions listed (ie. Mechanical Engineer, Electrical Engineer, etc.) will not need to be staffed.
Response 3: The revised key personnel list is as follows:

Question 4: Subfactors for Factor 4 are listed differently on p13 and p18. Please confirm which we should follow.
Response 4: The sub-factors listed on page 18 are correct and are repeated here:
Factor 4: Technical Solution for performance schedule: Provide project design and construction schedule. The Contractor’s understanding of project specific schedule parameters shall be evaluated. A complete and comprehensive schedule that includes design submission milestones and detailed construction task milestones per construction specifications institute (CSI) division will receive a higher rating.
Project Progress Schedule - The progress schedule will be in a time scaled bar graph format. The horizontal axis will be scaled for time beginning with the Notice to Proceed and concluding with contract completion. The vertical axis will show the milestones and major portions of the contract work. All schedule items will show a start date and a completion date. The detailed schedule will indicate specific tasks with dates for each step of the process including:
Sub Factor A: Design Period: Include pre-design site survey work, each design/construction document submittal milestone, and indicate government review periods. State all assumptions.
Sub Factor B: Construction Period: Include permitting as applicable, mobilization, site prep, staging, long lead items, start and completion of construction scope items, etc. State all assumptions.
Sub Factor C: Narrative of Schedule Parameters: Provide written commitment as to the time frame for project completion. Specify how much allowance has been made for anticipated weather delays and potential delays due to cemetery operations and events. State all assumptions.
Question 5: Are there as-built drawings available indicating the storm sewer structures invert elevations within Northwest Road, near the project location?
Response 5: Storm sewer structure as-built information is not available. Storm sewer structure design information is available.

Response 11: It is a possibility; while there are no guarantees, specifics will be discussed after project award.
Question 12: Factor 3, Key Personnel states that “Key staff professional licensing in the state of Alaska will result in higher evaluation rating.” We assume that the state listed is intended to be Michigan and not Alaska. Please confirm.
Response 12: See response to Question 2, above.
Question 13: Can we submit our Phase II Proposal Submission via the Vendor Portal (electronically) in lieu of delivering originals, hard copies and a CD to Washington DC?
Response 13: No, follow the instructions contained in the solicitation.

| Question 15: | The RFP drawings show the maintenance road being re-graded as part of this project. If achievable through the design solution, can the road be saved, or is the intent to drain the road to the west as part of this project? |
Response 15: See note 1 on RFP drawing sheet 3. The limit of construction is generally delineated by the bold dash line. The re-grading the maintenance road is not included in the scope of work.
Question 16: Will this project allow the use of precast quad pre-placed Crypts?
Response 16: Yes
Question 17: The scope of work states to include two (2) flower watering stations for this project. It appears that most of the existing burial sections at this cemetery utilize one flower watering station per section, or share one between the two sections. Is the two flower watering stations for this project to be located at the 2,000 pre-placed crypt burial section?

| Question 18: | The existing documents provided with the RFP (dwg L-305A) show the layout for the existing storm sewer located in the area of the project. However, no inverts (or profiles) are shown to determine if the connection of the crypt field underdrain system is possible. Can this information be provided? | |

| Question 19: | Existing irrigation piping is shown on the existing drawings provided with the RFP (Sheet I-7). Can we assume there is capacity in the existing 2 1/2” and 3” irrigation lines adjacent to the new crypt field to provide the water for the irrigation system? | |
| Response 19: | Assume that irrigation capacity exists; but verification will be required during the design stage. | |
| Question 20: | Glen Madderom’s e-mail to design firms several years ago stated the identification sand layer on top of the crypts was not required unless desired by the Cemetery. The bridging documents call out a 2” identification sand layer on one detail (sheet RFP-4 detail 2, end view) but not the other section details. Can you confirm the Cemetery wants an identification sand layer over the crypts? | |

| Question 21: | In the Phase II Request For Proposal Paragraph C.4, Factor 3, Key Personnel, please confirm the State of Registration (Alaska) is a typo and it should read “Michigan”. In addition , biographical data is not required for disciplines (such as Architect, Plumbing Engineer, Mechanical Engineer) that are not required for a crypt field project but a Civil Engineer should be included. | |

| Question 22: | Please provide information on the signing that will be required for the new crypt field (will the contract require modifying or replacing all directional signage within the Cemetery to add the new burial section to the signs?). | |

We can ask the question if we should assume undercutting will be required or state in our narrative that we noted the undercutting in the adjacent section but our price does not include any undercutting. | |
