Period of Performance:

Base: October 1, 2021 – September 30, 2022

Option Year 1: October 1, 2022 – September 30, 2023

Option Year 2: October 1, 2023 – September 30, 2024

Option Year 3: October 1, 2024 – September 30, 2025

Option Year 4: October 1, 2025 – September 30, 2026

2. Specific Contractor Tasks include:

1. The Contractor shall fill and dispense original prescriptions issued only by prescribers assigned to VA

PCCs & CBOCs. The VA shall provide a list of preauthorized prescriber names with their NPI (and DEA numbers as necessary) to the contractor with updates as they occur.

2. Prescriptions filled must be called into Pharmacy or electronically printed on VA CONTRACT

PHARMACY PRESCRIPTION VOUCHER.

A. Prescriptions may be delivered to contract pharmacy by:

a. Veteran Beneficiary hand-carrying the VA “CONTRACT PHARMACY

PRESCRIPTION VOUCHER” containing all information required by the pharmacy for entry into the contractor’s pharmacy IT system for “on-demand” claims adjudication and dispensing.

b. Fax from PCC/CBOC to Contract Pharmacy.

c. Call-in by PCC/CBOC prescriber.

B. Prescriptions shall be filled and dispensed only for medications in formulary “Drug List” authorized at the VA Southern Nevada Healthcare System (Attachment A). A copy of the formulary Drug List will accompany the awarded contract. Updates will be provided to the

Contractor as necessary. The Contractor is required to update the Drug List within a two (2) week period from time of written notification from the VA, for both drugs and drug strengths.

The Contractor must notify and update all pharmacies of the updates within forty-eight (48) hours or by the end of the same two (2) week period. A faxed or email authorization from a VA

C. All prescriptions shall be filled for quantities up to a maximum of ten (10) days only. The prescribing provider may elect to write the prescription for less than ten (10) days, and the pharmacy may only fill for prescribed number of days. This will provide a sufficient time for the

VA to activate the monthly fill process where necessary.

D. Prescription refills are NOT authorized. The Contractor shall fill only new prescriptions.

E. The Contractor is responsible to educate the used pharmacies in consistent relaying of VA policies when questioned by a Veteran Beneficiary. It is the responsibility of the pharmacy to inform the Veteran Beneficiary and then solve the problem. The Contractor is to clarify and obtain authorization from the VA at that time. The Veteran is never to be told to return the next day when the problem can be handled; the

Contractor is responsible to solve these types of problems while the Veteran Beneficiary is waiting. The VASNHS will provide the contractor with instructions on what should be done if no

VASNHS personnel are available to assist the pharmacy or the contractor.

F. Prescriptions filled under this contract shall be generic drugs to the extent permitted by law. The least expensive drug product the pharmacy has in stock shall be dispensed. Generic drugs shall be dispensed if in the professional judgment of the pharmacist, the substitute product is available, safe, effective, and to the greatest extent possible, contains the same chemical ingredients of the same strength, quantity, and dosage of the brand name. All generic products used must be in compliance with Federal and State regulations.

G. Mailing and delivery of prescriptions is NOT authorized.

H. The Contractor shall make available to the VA all prescription records and invoices upon request during business hours. All invoice requests will be fulfilled by the contractor to the VAMC.

I. The pharmacies for filling and dispensing of prescriptions must be located within a 5 (five) mile radius from the VA location. The Contractor shall list locations for filling and dispensing of prescriptions.

K. The Pharmacist working for the contractor will evaluate each prescription for drug interactions, contraindications given recorded drug allergies, appropriate doses, and accurate directions.

Incomplete, erroneous or questionable prescriptions will be clarified with the prescriber prior to dispensing

L. The Contractor shall be responsible for contacting the PCC/CBOC prescriber to work out a solution when a “backorder” or “out-of-stock” occurs.

M. All pharmacies and pharmacists must be licensed in the state providing the services. Contractor will be required to have on file current and valid licenses for all pharmacists and provide photocopies of each to the Contracting Officer within 45 days of contract award.

N. When patient or his representative picks up prescriptions, contractor shall release such prescriptions by first verifying the identity of the veteran with the veteran’s VA identification card.

3. Documentation

A. Contractor shall maintain a written log of VA prescriptions filled under this contract. The log shall indicate the date the prescription was filled and contractor’s prescription number. The VA patient or his representative shall sign next to the prescription number at the time of prescription pick up to signify receipt of each prescription.
