B.2 Statement of Work

Background

The Portland VA Medical Center requires numerous consumable supplies to be available for surgical procedures. These supplies are pre-packaged into kit form, known as surgical packs, and are customized for each medical center, depending on the medical/surgical procedure.

Scope

The contractor shall provide custom surgical packs to the Portland, Oregon VA Medical Center as requested by the appointed ordering official.

All items must be Sterile and in acceptable condition for use in sterile Operating Rooms procedures. The contractor will make no substitutions of product contained in the bill of materials without approval from the authorized medical center personnel. Any permanent changes to surgical pack contents, including adding or deleting surgical packs themselves, must be processed through the Contracting Officer in the form of a modification to this agreement.

0. Delivery Timeframe Surgical packs for all locations shall be delivered within five days of an order being placed. The contractor shall also offer expedited delivery for emergent need. In the event of an emergency, the contractor shall respond to emergency requests within 15-30 minutes, and deliver the requested surgical pack(s) within 24 hours.

Pack delivery considerations; standard orders will be delivered within 7 days, excluding weekends and holidays. Emergency orders will be delivered within 24 hours, excluding weekends and holidays.

Authorized individuals will place calls against this IDIQ via facsimile or telephone and will provide the VA obligation number for each call. Only authorized Government employees who have been specifically named by the Contracting Officer may order under this agreement. STRICT COMPLIANCE WITH THIS STIPULATION IS IMPERATIVE. Orders placed against this IDIQ by individuals other than those named as authorized by the Contracting Officer or for items not specifically covered by this IDIQ, or without a valid VA obligation number (format: 663-A12345) shall not be accepted by the contractor. Any and all supplies provided by the contractor in violation of this authorization will be treated as unauthorized commitments and may result in termination of this IDIQ by the Government.

“Ordering officers (OOs) will be authorized to place orders against this contract. The Contracting Officer (CO) will furnish the Contractor with the names of individuals authorized as OOs, by separate memorandum upon issuance of the contract. OOs are responsible for issuing and administering orders placed under this contract. OOs have no authority to modify any term of this basic contract. Any deviation from the terms of the basic contract must be approved in writing by the CO responsible for this contract. The Contractor shall accept orders against this contract only from the CO and/or authorized OOs. Fulfilling orders from persons other than the CO or OO may result in loss or delay in payment for supplies/services provided under such orders.”

Orders may be placed via government purchase card.

Satisfactory Performance Threshold: Failure to meet the required delivery times more than once per month will be deemed unsatisfactory.

Changes:

Changes to the pack configurations will be allowed by the user at the beginning of each quarter. Changes to the packs will be identifiable by a version number visible on the contents list. A one-month supply of packs will be available and ready for use at all times from the manufacturer. This will allow the user to request infrequent emergency (next day) delivery requests.

Changes to the custom pack due to a stock out, or the unavailability of an item by the manufacturer will be approved by the using facilities Operating Room Nurse Manager or the facilities Chief Logistics Officer prior to any change going into production. The Contracting Officer must authorize the change prior to any deliveries.

Manufacturer will meet with the facility Nurse Manager, and or the Chief Logistics Officer at the beginning of each quarter, at VA Puget Sound Health Care System for the purpose of open communications, and receive any pack changes from the facility. All changes must be reported to contracting and a modification processed before the changes can take effect.

Packaging requirements for Custom Packs:

Packs must contain the items listed in “Pack Contents.”

The outside of the pack must have a clear plastic dust cover that is thick enough to withstand multiple handlings without rips, tears or holes that might compromise the integrity of the contents. A contents list will be included on each pack, under the dust cover, and on the outside of the internal wrapper. The contents list must be clearly visible through the dust cover. The expiration date of the pack must be listed on the pack list with a minimum font size of 12.

The wrapper of the pack can be two wraps, sequentially wrapped, or a single wrap from a FDA approved wrap material. The material used must be large enough to contain the contents of the pack, withstand the sterilization process, and act as a sterile field when the pack is opened and the contents are exposed. The wrapper must also not have “bounce back” when opening the pack, as the wrapper would then contaminate the contents while attempting to create the sterile field. The contents inside of the wrapper must be sterile (free from all living organisms). Sterilization methods must follow AAMI (Association for the Advancement of Medical Instrumentation, and the FDA (Food and Drug Administration) rules and regulations governing class 1 medical devices.

The contents of the pack are user defined for each type of pack. The pack contents will be arranged as defined by the user.
