1.0 EXECUTIVE SUMMARY
TTL Associates, Inc. (TTL) conducted a National Emissions Standard for Hazardous Air Pollutants (NESHAP) asbestos inspection for the Forecast Whole Health project in Buildings 1 and 14 at the George E. Whalen VA Medical Center located in Salt Lake City, Utah. The inspection was conducted by Mr. Adam Mead on September 10, 2018. Mr. Mead is certified by the State of Utah Division of Air Quality as an Asbestos Inspector.
The table below summarizes the suspected asbestos-containing materials (ACM) identified, approximate quantity, and results of the asbestos sampling. ACM is defined by the United States Environmental Protection Agency (U.S. EPA) as any material containing more than 1% asbestos.
The Occupational Safety and Health Administration (OSHA) regulates all asbestos content.
The scope of work for this project associated with 660-18-252 Forecast Whole Health was conducted in accordance with contract number 36C25918C0298.

2.0 FACILITY DESCRIPTION
A National Emissions Standard for Hazardous Air Pollutants (NESHAP) asbestos inspection was conducted by TTL Associates, Inc. (TTL) on September 10, 2018, for the Forecast Whole Health project in Buildings 1 and 14 at the George E. Whalen VA Medical Center (VAMC) located in Salt Lake City, Utah.

3.0 ASBESTOS INSPECTION METHODOLOGY
The inspection was conducted by Mr. Adam Mead on September 10, 2018. Mr. Mead is certified by the State of Utah Division of Air Quality as an Asbestos Inspector. A copy of his certification is included in Appendix A.
The asbestos inspection was performed by identifying suspect Asbestos-Containing Materials (ACM) in accessible areas of the six rooms identified in the project scope of work. The suspect ACM were then grouped into Homogeneous Sampling Areas (HSA). An HSA is a material that exhibits similar physical characteristics and was installed at the same time as observed by the licensed inspector. TTL utilized previous inspection data provided by the VA to assist in the identification of HSAs.
Upon identifying the suspect ACM, representative bulk samples were collected from each HSA following United States Occupational Safety and Health Administration (OSHA) and United States Environmental Protection Agency (U.S. EPA) NESHAP regulations. Sample locations were selected as to be representative of the material sampled based on the licensed inspector’s professional judgement. Each sample collected was given a unique sample number. Sample and material locations were documented on floorplan drawings provided by the VAMC.
At the completion of the field work, the collected bulk samples were logged onto chain-of-custody sheets and transported to TTL’s asbestos laboratory for analysis by Polarized Light Microscopy with Dispersion Staining (PLM/DS). A total of 22 samples (51 material layers) were collected and analyzed. TTL’s asbestos laboratory is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP). The bulk samples were analyzed in accordance with the U.S. EPA “Method for Determination of Asbestos in Bulk Building Materials” 600/R- 93/116. This method requires that multiple and distinct layers be analyzed separately.
Two materials (HSA 8 – Black lab countertops and HSA 9 - Cementitious lab hood panels) were assumed to be ACM because sampling would compromise the integrity of the system.

Table 1.0 below summarizes the ACM identified during this inspection. Refer to Appendix B for the NESHAP Asbestos Inspection Summary Table for a listing of all suspect ACM, locations, approximate quantities, and results of sample analysis. Floorplan drawings detailing the bulk sampling locations are included in Appendix C. Appendix D contains the laboratory analytical report for the bulk sampling analysis. Photographic documentation of each sample location is included in Appendix E.

5.0 LIMITATIONS AND CONDITIONS
The information contained within this report was prepared for the exclusive use and reliance of the VAMC and TTL. This information is based on the specific parameters of the scope of work and the regulations in force at the time of this project.
TTL has made reasonable efforts to identify and quantify suspect ACM based upon the standard of care in the environmental industry existing at the time of the survey. This survey only summarizes the potential presence and estimated quantities of visually observed ACM.
Additional material disturbed during renovation and demolition activities should be evaluated on a case-by-case basis, especially materials that were previously hidden, obscured, or inaccessible, to determine if the material is included in this survey. If a given material is not described in this survey or cannot be identified as a non-suspect material, the material should be assumed to contain asbestos and renovation and demolition activities should be halted until sampling and analysis can be accomplished. Parties conducting renovation and demolition activities should follow all applicable federal, state, and local regulations in handling identified and suspect ACM.
TTL accepts no responsibility for the use, interpretation, or reliance by other parties on the information contained herein, nor does this report represent an instrument of regulatory compliance or an asbestos abatement specification.
