1. So does the SSHO and the CP have to be on site at all times? Yes, when work is going on this person must be on site. Can the SSHO and the CP be the same person? Yes
2. Will there have to be negative air machines while construction is being done? Yes If so how many? One per area. And where will we be able to vent them to? Contractor may vent into hallway or use outside window, if available. All negative air machines shall have HEPA filters. If venting into hallway, proper site security shall be maintained.
3. Does the area need to have a construction barrier built around it? The rooms in Whole Health are self-contained. If so does it need to be built to the deck? The project calls out what walls need to be built to the deck.
4. Can the mastic be mechanically removed versus chemical? . The vendor will need to provide the “Alternative Work Practice” that has been approved by Utah DEQ for using mechanical methods to remove mastic.
5. Can the V/A provide a map for a staging area for equipment trucks and dump trailer for asbestos debris? See attached map. Contractor will be given two parking spots, for the duration of abatement activities.
6. Can the V/A Clarify if only the 12 inch floor tile is asbestos and not the yellow adhesive? Or are we to assume that both the floor tile and yellow adhesive is asbestos containing material. No Lab results were posted that separate the two materials. The glue is not asbestos.
7. Can the HVAC supply and return ducts be isolated for this project? No
8. Will the V-A hire the third party consultant for over sight and area/clearance air sampling?
Inside the containment is on the contractor , the VA will hire a third party of outside clearance sampling.
9. It is sometimes difficult to attain new PPQ’s for bid submissions, will the VA allow submission of previously submitted PPQ’s for this RFP if it is recent and relevant? No. As stated in the Instructions to offerors: If the offeror cannot provide a CPARS for Projects Listed in Factor 1, then the offeror shall Submit Past Performance Questionnaires for each project included in Factor 1. Offerors shall not incorporate by reference into their proposal PPQs PREVIOUSLY SUBMITTED for other RFPs.
