B.4 STATEMENT OF WORK
Shockwave Treatment Machine; Brand Name or Equal - Statement of Work (SOW)
1. Project Description: Acquisition and delivery of the following: Shockwave Treatment Machine (2), carts for machine (2) and probe attachments to change penetration depth (2).
2. Background: The device emits acoustic waves whose energy can propagate through tissue and benefit patients through pain relief, vascularization, tissue regeneration and destruction of calcium deposits. It is becoming a standard of care throughout other VAs as it offers clinicians a non-invasive treatment of multiple Musculoskeletal (MSK) conditions. The service is looking to follow the lead and expand their treatment capabilities through this procurement.
This procurement has been requested to be brand name or equal to the Intelect Focus Shockwave machine as it has the capabilities needed of the request, with the probe attachments that expand the care options for providers.
SALIENT CHARACTERISTICS – Intelect Focus Shockwave System
The Intelect Focus Shockwave system must meet or exceed the following minimum salient characteristics to ensure compatibility with clinical treatment protocols and therapeutic outcomes:
1. Shockwave Generation Technology o Electromagnetically generated focused extracorporeal shock waves designed for precise, non-invasive therapeutic applications.
2. Energy Output Requirements o Adjustable energy range from 0.01 to 0.55 mJ/mm² to accommodate varying treatment intensities.
3. Frequency Range o Operational frequency settings between 1–8 Hz for customizable treatment protocols.
4. Therapeutic Penetration Depth o Ability to deliver focused shockwave energy to a minimum depth of 4.7 inches (12 cm) for deep tissue treatments.
5. Target Focus & Standoff Applicators o System must include two standoff variations to adjust focal depth and optimize targeting (Short and Long stand-offs).
6. User Interface o Integrated LCD touchscreen interface displaying frequency, energy level, pulse count, and treatment parameters; capable of storing and recalling custom treatment protocols.
7. Operational Efficiency o Device must support short treatment durations (typically a few minutes) and provide precise, targeted application with consistent output.
8. Non-Invasive, Clinical-Grade Design o Must provide a non-invasive alternative to medication or surgical intervention with no known significant adverse effects.
9. Accessories & Configuration o System must include:
▪ Focus Shockwave handpiece
▪ Water bag
▪ Silicone oil
▪ Closing ring (transparent)
▪ Shockwave power cord
▪ Stand-off I and II
▪ Handpiece holder
10. Clinical Indications Supported o Must be capable of treating:
▪ Chronic plantar fasciitis
▪ Tendinopathies (Achilles, patellar, epicondylitis)
▪ Trigger points
▪ Calcific tendinitis
▪ Tibial stress syndrome
▪ Other specified musculoskeletal pain conditions
3. Equipment Listing. The following equipment shall be delivered and installed as agreed upon within this statement of work:

SECTION C - CONTRACT CLAUSES
C.1 FSS RFQ INTRODUCTORY LANGUAGE
The terms and conditions of the contractor's FSS contract (including any contract modifications) apply to all Blanket Purchase Agreements (BPA) and task or delivery orders issued under the contract as a result of this RFQ. When a lower price has been established, or when the delivery terms, FOB terms, or ordering requirements have been modified by the BPA or task/delivery order, those modified terms will apply to all purchases made pursuant to it and take precedence over the FSS contract. Any unique terms and conditions of a BPA or order issued under the contract that are not a part of the applicable FSS contract will govern. In the event of an inconsistency between the terms and conditions of a BPA or task/delivery order and the Contractor's FSS terms, other than those identified above, the terms of the FSS contract will take precedence.
