CONSULTATION AND ANALYSIS OF THYROID FIND NEEDLE ASPIRATIONS AND OTHER SPECIALTY TESTING SERVICES

G. V. (SONNY) MONTGOMERY, VA MEDICAL CENTER

JACKSON, MS

STATEMENT OF WORK

The G.V. (Sonny) Montgomery, VA Medical Center (VAMC), Pathology & Laboratory Medicine Service (P&LMS), Jackson, MS, has a requirement for consultation and analysis of Thyroid Fine Needle Aspirations and other specialty testing. P&LMS is a clinical laboratory that does a wide menu of laboratory testing to include Anatomic Pathology services and Clinical Laboratory services. Throughout the fiscal year, P&LMS is in need of highly specialized testing not routinely offered by our lab or any other commercial reference lab on an FSS schedule. The requirements and performance of this contract shall be in accordance with contract requirements.

This requirement addresses those patients who have had a thyroid biopsy and tissue that need to be sent for further testing to determine if the patient has cancer. These patients are poor candidates for surgery due to multiple comorbidities. The initial cytology results were indeterminate. In order to determine if the thyroid has a malignancy, there is a test called Afirma that measures the expression of 167 genes. This test is a genetic classifier and it is the only one of its kind currently available in the U.S. The results of this test will help the clinicians determine if the risk of surgery is warranted in these patients.

P&LMS Jackson, MS, is estimating they will send 10 patient samples per year throughout the contract period requested.

The tissue is extracted during a fine needle aspiration. The sample is sent to this laboratory where a gene Expression Classifier determines the expression profile of RNA isolated from the nodule to determine if benign or suspicious for malignancy.
