Solicitation Summary: 36C25526Q0524

This is a Request for Quotation (RFQ) issued by the Department of Veterans Affairs Network Contracting Office 15 for ophthalmic equipment required by the Harry S. Truman VA Medical Center in Columbia, Missouri. The solicitation is set-aside for small businesses (100% of contract value) with NAICS code 339112 and a size standard of 1,000 employees. The offer due date is July 31, 2026, at 3:30 PM CDT, with a solicitation issue date of July 21, 2026. The contract will be a firm-fixed-price purchase order with delivery required 90 days after receipt of order, with installation completed by January 31, 2027, and training within 45 days of final installation.

The solicitation requires three primary medical devices: a photographic slit lamp with specific magnification (6x/10x/16x/25x/40x) and optical specifications; an automated ophthalmic perimeter with powered adjustable height table and comprehensive testing capabilities including SITA strategies and progression analysis; and an optical biometer with corneal topography, full-length OCT imaging, multiple imaging modes, and IOL calculation formulas. All equipment must include two-year manufacturer warranties, two sets of user and technical manuals, and be FORUM-compatible with current and future EHR systems while meeting all federal cybersecurity requirements. The contract includes six line items with separate pricing for equipment (CLINs 0001-0004), white-glove installation service (CLIN 0005), and training (CLIN 0006). Evaluation criteria emphasize technical capability, past performance (from the last three years), and price, with technical and past performance weighted significantly more heavily than price. Offerors must be registered in SAM and verified in the SBA Small Business Search at time of offer submission and contract award, with unverified firms considered non-responsive and ineligible for award. Vendors must submit an OEM authorized distributor letter with their quote or be deemed non-responsive.

The Contractor shall proceed diligently with performance of this contract, pending final resolution of any dispute arising under the contract.

(f) Excusable delays. The Contractor shall be liable for default unless nonperformance is caused by an occurrence beyond the reasonable control of the Contractor and without its fault or negligence. Examples of occurrences include acts of God or the public enemy, acts of the Government in either its sovereign or contractual capacity, fires, floods, epidemics, quarantine restrictions, strikes, unusually severe weather, and delays of common carriers. (h) Patent indemnity. The Contractor shall indemnify the Government and its officers, employees, and agents against liability, including costs, for actual or alleged direct or contributory infringement of, or inducement to infringe, any United States or foreign patent, trademark, or copyright, arising out of the performance of this contract, provided the Contractor is reasonably notified of such claims and proceedings.
