SECTION 01 00 00

GENERAL REQUIREMENTS

1.1 SAFETY REQUIREMENTS

A. Refer to section 01 35 26, SAFETY REQUIREMENTS f or safety and infection control requirements.

1.2 GENERAL INTENTION

A. Contractor shall completely prepare site for bui lding operations, including demolition and removal of existing struct ures, and furnish labor and materials and perform work for FCA-Replace Air Handler (AC-

S2) VA Project No. 589A4-17-107 as required by drawings and specifications.

B. One pre-bid site visit will be scheduled by the Contracting Officer for the purpose of familiarizing with the project scope . Attendance is highly recommended. Subsequent visits on an individ ual basis will not be allowed.

C. Offices of Fox Architects , as Architect-Engineers, will render certain technical services during construction. Such servic es shall be considered as advisory to the Government and shall not be construed as expressing or implying a contractual act of the Gov ernment without affirmations by Contracting Officer or his duly aut horized representative.

D. The terms Contracting Officer’s Representative ( COR), Resident Project

Engineer, and Project Engineer are synonymous when referenced within the entire project manual, drawings and specificati ons.

E. Before placement and installation of work subjec t to tests by testing laboratory retained by the General Contractor, the Contractor shall notify the COR in sufficient time to enable testing laboratory personnel to be present at the site in time for pro per taking and testing of specimens and field inspection. Such pri or notice shall be not less than three work days unless otherwise desi gnated by the COR.

F. All employees of general contractor and subcontr actors shall comply with VA security management program, be identified by project and employer, and restricted from unauthorized access.

G. Prior to commencing work, General Contractor sha ll provide proof that an OSHA certified “competent person” (CP) (29 CFR 1 926.20(b)(2) will maintain a presence at the work site whenever the g eneral or subcontractors are present.

H. Training:

1. All employees of general contractor or subcontr actors shall have the

10-hour OSHA certified construction safety course a nd or other relevant competency training. On site supervisors for the co ntractor shall have the 30-hour OSHA certified construction safety cour se.

2. Submit training records of all such employees f or approval before the start of work.

1.3 STATEMENT OF BID ITEM(S)

A. BID ITEM I – GENERAL CONSTRUCTION: Work includes ge neral construction, alterations, mechanical and electrical work, necess ary removal of existing structures and construction and certain ot her items as identified in the specifications and drawings. Peri od of performance shall be 270 days.

1.4 SPECIFICATIONS AND DRAWINGS FOR CONTRACTOR

A. Drawings and contract documents may be obtained from the website where the solicitation is posted. Additional copies will be at Contractor's expense.

1.5 CONSTRUCTION SECURITY REQUIREMENTS

A. Security Plan:

1. The security plan defines both physical and admi nistrative security procedures that will remain effective for the entir e duration of the project.

2. The General Contractor is responsible for assuri ng that all sub-contractors working on the project and their employ ees also comply with these regulations.

B. Security Procedures:

1. General Contractor’s employees shall not enter t he project site without appropriate badge. They may also be subjec t to inspection of their personal effects when entering or leaving the project site.

2. Before starting work the General Contractor shal l give one week’s notice to the Contracting Officer so that security arrangements can be provided for the employees. This notice is sepa rate from any notices required for utility shutdown described lat er in this section.

3. No photography of VA premises is allowed without written permission of the Contracting Officer.

4. VA reserves the right to close down or shut down the project site and order General Contractor’s employees off the pr emises in the event of a national emergency. The General Contrac tor may return to the site only with the written approval of the Cont racting Officer.

C. Key Control:

1. The General Contractor shall provide duplicate k eys and lock combinations to the Contracting officers representa tive (COR) for the purpose of security inspections of every area o f project including tool boxes and parked machines and take a ny emergency action.

2. The General Contractor shall turn over all perma nent lock cylinders to the VA locksmith for permanent installation. See Section 08 71

00, DOOR HARDWARE and coordinate.

D. Document Control:

1. Before starting any work, the General Contractor /Sub Contractors shall submit an electronic security memorandum desc ribing the

01 00 00 -4 approach to following goals and maintaining confide ntiality of

“sensitive information”.

2. The General Contractor is responsible for safeke eping of all drawings, project manual and other project informat ion. This information shall be shared only with those with a specific need to accomplish the project.

3. Certain documents, sketches, videos or photograp hs and drawings may be marked “Law Enforcement Sensitive” or “Sensitive Unclassified”.

Secure such information in separate containers and limit the access to only those who will need it for the project. Re turn the information to the Contracting Officer upon request .

4. These security documents shall not be removed or transmitted from the project site without the written approval of Co ntracting

Officer.

5. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the VA.

6. Notify Contracting Officer and Site Security Off icer immediately when there is a loss or compromise of “sensitive in formation”.

7. All electronic information shall be stored in sp ecified location following VA standards and procedures using an Engi neering Document

Management Software (EDMS).

a. Security, access and maintenance of all project drawings, both scanned and electronic shall be performed and track ed through the

EDMS system.

b. “Sensitive information” including drawings and o ther documents may be attached to e-mail provided all VA encryptio n procedures are followed.

E. Motor Vehicle Restrictions

1. Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitt ed 24 hours

01 00 00 -5 before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies.

2. A limited number of (2 to 5) permits shall be is sued for General

Contractor and its employees for parking in designa ted areas only.

1.6 OPERATIONS AND STORAGE AREAS

A. The Contractor shall confine all operations (inc luding storage of materials) on Government premises to area within th e defined project area. The Contractor shall hold and save the Govern ment, its officers and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.

B. The contractor will not be permitted to set up a job trailer for this project.

C. The Contractor shall, under regulations prescrib ed by the Contracting

Officer, use only established roadways, or use temp orary roadways constructed by the Contractor when and as authorize d by the Contracting

Officer. When materials are transported in prosecut ing the work, vehicles shall not be loaded beyond the loading cap acity recommended by the manufacturer of the vehicle or prescribed by an y Federal, State, or local law or regulation. When it is necessary to cr oss curbs or sidewalks, the Contractor shall protect them from d amage. The

Contractor shall repair or pay for the repair of an y damaged curbs, sidewalks, or roads.

D. Working space and space available for storing ma terials shall be limited to the defined project area.

E. Workmen are subject to rules of Medical Center a pplicable to their conduct.

F. Execute work so as to interfere as little as pos sible with normal functioning of Medical Center as a whole, including operations of utility services, fire protection systems and any e xisting equipment, and with work being done by others. Use of equipmen t and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied, during co nstruction, jointly

01 00 00 -6 by patients or medical personnel, and Contractor's personnel, except as permitted by COR where required by limited working space.

1. Do not store materials and equipment in other th an assigned areas.

2. Schedule delivery of materials and equipment to immediate construction working areas within buildings in use by Department of

Veterans Affairs in quantities sufficient for not m ore than two work days. Provide unobstructed access to Medical Center areas required to remain in operation.

3. Where access by Medical Center personnel to vaca ted portions of buildings is not required, storage of Contractor's materials and equipment will be permitted subject to fire and saf ety requirements.

G. Utilities Services: Where necessary to cut exi sting pipes, electrical wires, conduits, cables, etc., of utility services, or of fire protection systems or communications systems (excep t telephone), they shall be cut and capped at suitable places where sh own; or, in absence of such indication, where directed by COR. All such actions shall be coordinated with the COR or Utility Company involve d:

1. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the c onstruction project, for such items as water, sewer, electricit y, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.

H. Phasing:

The Medical Center must maintain its operation 24 hours a day 7 days a week. Therefore, any interruption in service must be scheduled and coordinated with the COR to ensure that no lapses i n operation occur.

It is the CONTRACTOR'S responsibility to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, the interim lif e safety measure to be used during the work, and a schedule defining th e duration of the work with milestone subtasks. The work to be outli ned shall include, but not be limited to:

To insure such executions, Contractor shall furnis h the COR with a schedule of approximate phasing dates on which the Contractor intends to accomplish work in each specific area of site, b uilding or portion thereof. In addition, Contractor shall notify the C OR two weeks in advance of the proposed date of starting work in ea ch specific area of site, building or portion thereof. Arrange such pha sing dates to insure accomplishment of this work in successive phases mu tually agreeable to

COR and Contractor.

I. Portions of this project requires work on week ends and after hours

(after 1700) for specific work. Refer to drawings for areas and specific work requiring after hours work.

J. Building No. 1, 6 th Floor will be occupied during performance of work but immediate areas of alterations will be vacated.

1. Contractor shall take all measures and provide a ll material necessary for protecting existing equipment and pro perty in affected areas of construction against dust and debris, so t hat equipment and affected areas to be used in the Medical Centers op erations will not be hindered. Contractor shall permit access to Depa rtment of

Veterans Affairs personnel and patients through oth er construction areas which serve as routes of access to such affec ted areas and equipment. These routes whether access or egress s hall be isolated from the construction area by temporary partitions and have walking surfaces, lighting etc. to facilitate patient and s taff access.

Coordinate alteration work in areas occupied by Dep artment of

Veterans Affairs so that Medical Center operations will continue during the construction period.

2. Immediate areas of alterations not mentioned in preceding

Subparagraph 1 will be temporarily vacated while al terations are performed.

K. When a building and/or construction site is turn ed over to Contractor, Contractor shall accept entire responsibility inclu ding upkeep and maintenance therefore:

1. Contractor shall maintain a minimum temperature of 4 degrees C (40 degrees F) at all times, except as otherwise specif ied.

2. Contractor shall maintain in operating condition existing fire protection and alarm equipment. In connection with fire alarm equipment, Contractor shall make arrangements for p re-inspection of site with Fire Department or Company (Department of Veterans Affairs or municipal) whichever will be required to respond to an alarm from

Contractor's employee or watchman.

L. Utilities Services: Maintain existing utility se rvices for Medical

Center at all times. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uni nterrupted services.

Where necessary to cut existing water, steam, gases , sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (incl uding telephone), they shall be cut and capped at suitable places whe re shown; or, in absence of such indication, where directed by COR.

1. No utility service such as water, gas, steam, se wers or electricity, or fire protection systems and communications syste ms may be interrupted without prior approval of COR. Electric al work shall be accomplished with all affected circuits or equipmen t de-energized.

When an electrical outage cannot be accomplished, w ork on any energized circuits or equipment shall not commence without a detailed work plan, the Medical Center Director’s p rior knowledge and written approval. Refer to specification Sectio ns 26 05 11, REQUIREMENTS FOR ELECTRICAL INSTALLATIONS, 27 05 11 REQUIREMENTS FOR

COMMUNICATIONS INSTALLATIONS for additional require ments.

2. Contractor shall submit a request to interrupt a ny such services to

COR, in writing, 7 days in advance of proposed inte rruption. Request shall state reason, date, exact time of, and approx imate duration of such interruption.

3. Contractor will be advised (in writing) of appro val of request, or of which other date and/or time such interruption w ill cause least inconvenience to operations of Medical Center. Inte rruption time

01 00 00 -9 approved by Medical Center may occur at other than Contractor's normal working hours.

4. Major interruptions of any system must be reques ted, in writing, at least 15 calendar days prior to the desired time an d shall be performed as directed by the COR.

5. In case of a contract construction emergency, se rvice will be interrupted on approval of COR. Such approval will be confirmed in writing as soon as practical.

6. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the c onstruction project, for such items as water, sewer, electricit y, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.

M. Abandoned Lines: All service lines such as wires , cables, conduits, ducts, pipes and the like, and their hangers or sup ports, which are to be abandoned but are not required to be entirely re moved, shall be sealed, capped or plugged at the main, branch or pa nel they originate from. The lines shall not be capped in finished are as, but shall be removed and sealed, capped or plugged in ceilings, within furred spaces, in unfinished areas, or within walls or par titions; so that they are completely behind the finished surfaces.

N. To minimize interference of construction activit ies with flow of

Medical Center traffic, comply with the following:

1. Keep roads, walks and entrances to grounds, to p arking and to occupied areas of buildings clear of construction m aterials, debris and standing construction equipment and vehicles.

O. Coordinate the work for this contract with other construction operations as directed by COR. This includes the sc heduling of traffic and the use of roadways, as specified in Article, U SE OF ROADWAYS.

1.7 ALTERATIONS

A. Survey: Before any work is started, the Contract or shall make a thorough survey with the COR of areas of buildings in which alterations occur and areas which are anticipated routes of acc ess, and furnish a report, signed by all, to the Contracting Officer. This report shall list by rooms and spaces:

1. Existing condition and types of resilient floori ng, doors, windows, walls and other surfaces not required to be altered throughout affected areas of building.

2. Existence and conditions of items such as plumbi ng fixtures and accessories, electrical fixtures, equipment, veneti an blinds, shades, etc., required by drawings to be either reu sed or relocated, or both.

3. Shall note any discrepancies between drawings an d existing conditions at site.

4. Shall designate areas for working space, materia ls storage and routes of access to areas within buildings where al terations occur and which have been agreed upon by Contractor and C OR.

B. Any items required by drawings to be either reus ed or relocated or both, found during this survey to be nonexistent, o r in opinion of COR, to be in such condition that their use is impossibl e or impractical, shall be furnished and/or replaced by Contractor wi th new items in accordance with specifications which will be furnis hed by Government.

Provided the contract work is changed by reason of this subparagraph B, the contract will be modified accordingly, under pr ovisions of clause entitled "DIFFERING SITE CONDITIONS" (FAR 52.236-2) and "CHANGES" (FAR

52.243-4 and VAAR 852.236-88).

C. Re-Survey: Thirty days before expected partial o r final inspection date, the Contractor and COR together shall make a thorough re-survey of the areas of buildings involved. They shall furn ish a report on conditions then existing, of resilient flooring, do ors, windows, walls

01 00 00 -11 and other surfaces as compared with conditions of s ame as noted in first condition survey report:

1. Re-survey report shall also list any damage caus ed by Contractor to such flooring and other surfaces, despite protectio n measures; and, will form basis for determining extent of repair wo rk required of

Contractor to restore damage caused by Contractor's workmen in executing work of this contract.

D. Protection: Provide the following protective mea sures:

1. Wherever existing roof surfaces are disturbed th ey shall be protected against water infiltration. In case of le aks, they shall be repaired immediately upon discovery.

2. Temporary protection against damage for portions of existing structures and grounds where work is to be done, ma terials handled and equipment moved and/or relocated.

3. Protection of interior of existing structures at all times, from damage, dust and weather inclemency. Wherever work is performed, floor surfaces that are to remain in place shall be adequately protected prior to starting work, and this protecti on shall be maintained intact until all work in the area is com pleted.

1.8 DISPOSAL AND RETENTION

A. Materials and equipment accruing from work remov ed and from demolition of buildings or structures, or parts thereof, shall be disposed of as follows:

1. The contractor is permitted to use the facility construction waste dumpster(s). Coordinate with COR during demolition to have the dumpster emptied daily (Monday through Friday).

2. Reserved items which are to remain property of t he Government are identified by attached tags or noted on drawings or in specifications as items to be stored. Items that re main property of the Government shall be removed or dislodged from p resent locations

01 00 00 -12 in such a manner as to prevent damage which would b e detrimental to re-installation and reuse. Store such items where d irected by COR.

3. Items not reserved shall become property of the Contractor and be removed by Contractor from Medical Center.

4. Items of portable equipment and furnishings loca ted in rooms and spaces in which work is to be done under this contr act shall remain the property of the Government. When rooms and spac es are vacated by the Department of Veterans Affairs during the alter ation period, such items which are NOT required by drawings and s pecifications to be either relocated or reused will be removed by th e Government in advance of work to avoid interfering with Contracto r's operation.

5. PCB Transformers and Capacitors: The Contractor shall be responsible for disposal of the Polychlorinated Biphenyl (PCB) transformers and capacitors. The transformers and capacitors shall b e taken out of service and handled in accordance with the procedur es of the

Environmental Protection Agency (EPA) and the Depar tment of

Transportation (DOT) as outlined in Code of Federal Regulation

(CFR), Titled 40 and 49 respectively. The EPA's Tox ic Substance

Control Act (TSCA) Compliance Program Policy Nos. 6 -PCB-6 and 6-PCB-

7 also apply. Upon removal of PCB transformers and capacitors for disposal, the "originator" copy of the Uniform Haza rdous Waste

Manifest (EPA Form 8700-22), along with the Uniform Hazardous Waste

Manifest Continuation Sheet (EPA Form 8700-22A) sha ll be returned to the Contracting Officer who will annotate the contr act file and transmit the Manifest to the Medical Center's Chief .

1.9 PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND

IMPROVEMENTS (NOT USED)

1.10 RESTORATION

A. Remove, cut, alter, replace, patch and repair ex isting work as necessary to install new work. Except as otherwise shown or specified, do not cut, alter or remove any structural work, an d do not disturb any ducts, plumbing, steam, gas, or electric work witho ut approval of the

COR. Existing work to be altered or extended and th at is found to be defective in any way, shall be reported to the COR before it is disturbed. Materials and workmanship used in restor ing work, shall conform in type and quality to that of original exi sting construction, except as otherwise shown or specified.

B. Upon completion of contract, deliver work comple te and undamaged.

Existing work (walls, ceilings, partitions, floors, mechanical and electrical work, lawns, paving, roads, walks, etc.) disturbed or removed as a result of performing required new work , shall be patched, repaired, reinstalled, or replaced with new work, a nd refinished and left in as good condition as existed before commenc ing work.

C. At Contractor's own expense, Contractor shall im mediately restore to service and repair any damage caused by Contractor' s workmen to existing piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications sy stems (including telephone) which are not scheduled for discontinuan ce or abandonment.

D. Expense of repairs to such utilities and systems not shown on drawings or locations of which are unknown will be covered b y adjustment to contract time and price in accordance with clause e ntitled "CHANGES"

(FAR 52.243-4 and VAAR 852.236-88) and "DIFFERING S ITE CONDITIONS" (FAR

52.236-2).

1.13 LAYOUT OF WORK

A. The Contractor shall lay out the work from exist ing interior/exterior duct and building structures, walls, etc. as indica ted on the drawings.

1.14 AS-BUILT DRAWINGS

A. The contractor shall maintain two full size sets of as-built drawings which will be kept current during construction of t he project, to include all contract changes, modifications and cla rifications.

B. All variations shall be shown in the same genera l detail as used in the contract drawings. To insure compliance, as-built d rawings shall be made available for the COR review, as often as requ ested.

C. Contractor shall deliver two approved completed sets of as-built drawings in the electronic version (scanned PDF) to the COR within 15 calendar days after each completed phase and after the acceptance of the project by the COR.

D. Paragraphs A, B, & C shall also apply to all sho p drawings.

1.15 USE OF ROADWAYS

A. For hauling, use only established public roads a nd roads on Medical

Center property and, when authorized by the COR, su ch temporary roads which are necessary in the performance of contract work. Temporary

01 00 00 -15 roads shall be constructed and restoration performe d by the Contractor at Contractor's expense. When necessary to cross cu rbing, sidewalks, or similar construction, they must be protected by wel l-constructed bridges.

1.17 TEMPORARY USE OF MECHANICAL AND ELECTRICAL EQUIPMENT

A. Use of new installed mechanical and electrical e quipment to provide heat, ventilation, plumbing, light and power will b e permitted subject to written approval and compliance with the followi ng provisions:

1. Permission to use each unit or system must be gi ven by COR in writing. If the equipment is not installed and main tained in accordance with the written agreement and following provisions, the

COR will withdraw permission for use of the equipme nt.

2. Electrical installations used by the equipment s hall be completed in accordance with the drawings and specifications to prevent damage to the equipment and the electrical systems, i.e. tran sformers, relays, circuit breakers, fuses, conductors, motor controll ers and their overload elements shall be properly sized, coordina ted and adjusted.

Installation of temporary electrical equipment or d evices shall be in accordance with NFPA 70, National Electrical Cod e, (2014

Edition), Article 590, Temporary Installations. Voltage supplied to each item of equipment shall be verified to be corr ect and it shall be determined that motors are not overloaded. The e lectrical equipment shall be thoroughly cleaned before using it and again immediately before final inspection including vacuu m cleaning and wiping clean interior and exterior surfaces.

3. Units shall be properly lubricated, balanced, an d aligned.

Vibrations must be eliminated.

4. Automatic temperature control systems for prehea t coils shall function properly and all safety controls shall fun ction to prevent coil freeze-up damage.

5. The air filtering system utilized shall be that which is designed for the system when complete, and all filter elemen ts shall be replaced at completion of construction and prior to testing and balancing of system.

6. All components of heat production and distributi on system, metering equipment, condensate returns, and other auxiliary facilities used in temporary service shall be cleaned prior to use; maintained to prevent corrosion internally and externally during use; and cleaned, maintained and inspected prior to acceptance by the Government.

Boilers, pumps, feedwater heaters and auxiliary equ ipment must be operated as a complete system and be fully maintain ed by operating personnel. Boiler water must be given complete and continuous chemical treatment.

B. Prior to final inspection, the equipment or part s used which show wear and tear beyond normal, shall be replaced with iden tical replacements, at no additional cost to the Government.

C. This paragraph shall not reduce the requirements of the mechanical and electrical specifications sections.

D. Any damage to the equipment or excessive wear du e to prolonged use will be repaired replaced by the contractor at the contr actor’s expense.

1.18 TEMPORARY USE OF EXISTING ELEVATORS

A. Use of existing elevators for handling buildin g materials and

Contractor's personnel will be permitted subject to following provisions:

1. Contractor makes all arrangements with the COR f or use of elevators.

The COR will ascertain that elevators are in proper condition.

2. Contractor covers and provides maximum protectio n of following elevator components:

a. Entrance jambs, heads soffits and threshold plat es.

b. Entrance columns, canopy, return panels and insi de surfaces of car enclosure walls.

c. Finish flooring.

3. Government will accept hoisting ropes of elevato r and rope of each speed governor if they are worn under normal operat ion. However, if these ropes are damaged by action of foreign matter such as sand, lime, grit, stones, etc., during temporary use, the y shall be removed and replaced by new hoisting ropes at the c ontractor’s expense.

4. If brake lining of elevators are excessively wor n or damaged during temporary use, they shall be removed and replaced b y new brake lining at the contractors expense.

5. All parts of main controller, starter, relay pan el, selector, etc., worn or damaged during temporary use shall be remov ed and replaced with new parts at the contractor’s expense, if reco mmended by elevator inspector after elevator is released by Co ntractor.

6. Place elevator in condition equal, less normal w ear, to that existing at time it was placed in service of Contra ctor as approved by Contracting Officer.

1.19 TEMPORARY USE OF NEW ELEVATORS (NOT USED)

1.20 TEMPORARY TOILETS

A. Contractor may have for use of Contractor's wo rkmen, such toilet accommodations as may be assigned to Contractor by Medical Center.

Contractor shall keep such places clean and be resp onsible for any damage done thereto by Contractor's workmen. Failur e to maintain satisfactory condition in toilets will deprive Cont ractor of the privilege to use such toilets.

1.21 AVAILABILITY AND USE OF UTILITY SERVICES

A. The Government shall make all reasonably require d amounts of utilities available to the Contractor from existing outlets a nd supplies, as specified in the contract. The amount to be paid by the Contractor for

01 00 00 -18 chargeable electrical services shall be the prevail ing rates charged to the Government. The Contractor shall carefully cons erve any utilities furnished without charge.

B. The Contractor, at Contractor's expense and in a workmanlike manner, in compliance with code and as satisfactory to the Con tracting Officer, shall install and maintain all necessary temporary connections and distribution lines, and all meters required to meas ure the amount of electricity used for the purpose of determining cha rges. Before final acceptance of the work by the Government, the Contr actor shall remove all the temporary connections, distribution lines, meters, and associated paraphernalia and repair restore the inf rastructure as required.

C. Heat: Furnish temporary heat necessary to preven t injury to work and materials through dampness and cold. Use of open sa lamanders or any temporary heating devices which may be fire hazards or may smoke and damage finished work, will not be permitted. Mainta in minimum temperatures as specified for various materials:

1. Obtain heat by connecting to Medical Center heat ing distribution system.

a. Steam is available at no cost to Contractor.

E. Electricity (for Construction and Testing): Furn ish all temporary electric services.

1. Obtain electricity by connecting to the Medical Center electrical distribution system. The Contractor shall meter and pay for electricity required for electric cranes and hoisti ng devices, electrical welding devices and any electrical heati ng devices providing temporary heat. Electricity for all other uses is available at no cost to the Contractor.

F. Water (for Construction and Testing): Furnish te mporary water service.

1. Obtain water by connecting to the Medical Center water distribution system. Provide reduced pressure backflow preventer at each

01 00 00 -19 connection as per code. Water is available at no co st to the

Contractor.

2. Maintain connections, pipe, fittings and fixture s and conserve water-use so none is wasted. Failure to stop leakag e or other wastes will be cause for revocation (at COR discretion) of use of water from Medical Center's system.

G. Fuel: Natural and LP gas and burner fuel oil req uired for boiler cleaning, normal initial boiler-burner setup and ad justing, and for performing the specified boiler tests will be furni shed by the

Government. Fuel required for prolonged boiler-burn er setup, adjustments, or modifications due to improper desig n or operation of boiler, burner, or control devices shall be furnish ed and paid by the

Contractor at Contractor's expense.

1.22 NEW TELEPHONE EQUIPMENT

A. The contractor shall coordinate with the work of installation or relocation of minor telephone equipment by others. This work shall be completed before the building is turned over to VA.

1.23 TESTS

A. As per specification section 23 05 93 the contra ctor shall provide a written testing and commissioning plan complete wit h component level, equipment level, sub-system level and system level breakdowns. The plan will provide a schedule and a written sequence of what will be tested, how and what the expected outcome will be. This document will be submitted for approval prior to commencing work. The contractor shall document the results of the approved plan and submit for approval with the as built documentation.

B. Pre-test mechanical and electrical equipment and systems and make corrections required for proper operation of such s ystems before requesting final tests. Final test will not be cond ucted unless pre-tested.

C. Conduct final tests required in various sections of specifications in presence of an authorized representative of the Con tracting Officer.

Contractor shall furnish all labor, materials, equi pment, instruments, and forms, to conduct and record such tests.

D. Mechanical and electrical systems shall be balan ced, controlled and coordinated. A system is defined as the entire syst em which must be coordinated to work together during normal operatio n to produce results for which the system is designed. For example, air conditioning supply air is only one part of entire system which provide s comfort conditions for a building. Other related components are return air, exhaust air, steam, chilled water, refrigerant, hot water, contr ols and electricity, etc. Another example of a system which involves sev eral components of different disciplines is a boiler installation. Eff icient and acceptable boiler operation depends upon the coordi nation and proper operation of fuel, combustion air, controls, steam, feedwater, condensate and other related components.

E. All related components as defined above shall be functioning when any system component is tested. Tests shall be complete d within a reasonably period of time during which operating an d environmental conditions remain reasonably constant and are typic al of the design conditions.

F. Individual test result of any component, where r equired, will only be accepted when submitted with the test results of re lated components and of the entire system.

1.24 INSTRUCTIONS

A. Contractor shall furnish Maintenance and Operati ng manuals (hard copies and electronic) and verbal instructions when requir ed by the various sections of the specifications and as hereinafter s pecified.

B. Manuals: Maintenance and operating manuals and o ne compact disc (four hard copies and one electronic copy each) for each separate piece of equipment shall be delivered to the COR coincidenta l with the delivery of the equipment to the job site. Manuals shall be complete, detailed guides for the maintenance and operation of equipme nt. They shall include complete information necessary for starting , adjusting, maintaining in continuous operation for long period s of time and

01 00 00 -21 dismantling and reassembling of the complete units and sub-assembly components. Manuals shall include an index covering all component parts clearly cross-referenced to diagrams and illustrati ons. Illustrations shall include "exploded" views showing and identify ing each separate item. Emphasis shall be placed on the use of specia l tools and instruments. The function of each piece of equipmen t, component, accessory and control shall be clearly and thorough ly explained. All necessary precautions for the operation of the equi pment and the reason for each precaution shall be clearly set forth. Man uals must reference the exact model, style and size of the piece of equ ipment and system being furnished. Manuals referencing equipment simi lar to but of a different model, style, and size than that furnishe d will not be accepted.

C. Instructions: Contractor shall provide qualified , factory-trained manufacturers' representatives to give detailed tra ining to assigned

Department of Veterans Affairs personnel in the ope ration and complete maintenance for each piece of equipment. All such t raining will be at the job site. These requirements are more specifica lly detailed in the various technical sections. Instructions for differ ent items of equipment that are component parts of a complete sy stem, shall be given in an integrated, progressive manner. All instructo rs for every piece of component equipment in a system shall be availab le until instructions for all items included in the system h ave been completed.

This is to assure proper instruction in the operati on of inter-related systems. All instruction periods shall be at such t imes as scheduled by the COR and shall be considered concluded only when the COR is satisfied in regard to complete and thorough covera ge. The contractor shall submit a course outline with associated mater ial to the COR for review and approval prior to scheduling training to ensure the subject matter covers the expectations of the VA and the co ntractual requirements. The Department of Veterans Affairs r eserves the right to request the removal of, and substitution for, any i nstructor who, in the opinion of the COR, does not demonstrate suffic ient qualifications in accordance with requirements for instructors abo ve.

1.25 GOVERNMENT-FURNISHED PROPERTY

A. The Government shall deliver to the Contractor, the

Government-furnished property shown on the drawings .

B. Equipment furnished by Government to be installe d by Contractor will be furnished to Contractor at the Medical Center.

C. Contractor shall be prepared to receive this e quipment from Government and store or place such equipment not less than 90 days before

Completion Date of project.

D. Notify Contracting Officer in writing, 60 days i n advance, of date on which Contractor will be prepared to receive equipm ent furnished by

Government. Arrangements will then be made by the G overnment for delivery of equipment.

1. Immediately upon delivery of equipment, Contract or shall arrange for a joint inspection thereof with a representative of the Government.

At such time the Contractor shall acknowledge recei pt of equipment described, make notations, and immediately furnish the Government representative with a written statement as to its c ondition or shortages.

2. Contractor thereafter is responsible for such eq uipment until such time as acceptance of contract work is made by the Government.

E. Equipment furnished by the Government will be de livered in a partially assembled (knock down) condition in accordance with existing standard commercial practices, complete with all fittings, f astenings, and appliances necessary for connections to respective services installed under contract. All fittings and appliances (i.e., couplings, ells, tees, nipples, piping, conduits, cables, and the li ke) necessary to make the connection between the Government furnishe d equipment item and the utility stub-up shall be furnished and installe d by the contractor at no additional cost to the Government.

F. Completely assemble and install the Government f urnished equipment in place ready for proper operation in accordance with specifications and drawings.

G. Furnish supervision of installation of equipment at construction site by qualified factory trained technicians regularly employed by the equipment manufacturer.

SECTION 01 00 10

MEDICAL CENTER REQUIREMENTS

1.0 GENERAL INTENTION: This document pertains to station policy for construction projects performed at the Harry S Truman Memorial Veterans' Hospital (HSTMVH) in Columbia, MO. Safety and health concerns are taken seriously at this facility. Both our staff and yours are expected to strictly adhere to the regulations and requirements. This is exceedingly important, since we must be primarily concerned for the safety of our patients. In this regard, OSHA Standards may protect worker safety and health, but they have minimal benefit for protecting the safety and health of our patients, due primarily to their differing medical conditions. Review this information as orientation with your personnel performing work on site. Where the requirements as outlined in this and Section 01 00 00 are differing, the more stringent shall apply.

2.0 REQUIREMENTS:

A. Security:

1. Secure all construction areas, especially mechanical and electrical rooms against entry of unauthorized individuals including patients.

2. Notify the Contracting Officer’s Representative (COR) for permission to work after hours and weekends. Standard work hours for this project will be Monday through Friday from 7:30 am to 4:00 pm.

B. Key Security:

1. Only a limited number of keys will be issued to the contractor.

2. See section 01 00 00 for procedures in case the Contractor loses a key.

3. Ensure all doors leading to and from construction are either monitored or locked to prevent access to the area from unauthorized persons.

C. General Safety:

1. Follow all federal, state and local safety and health regulations.

2. Maintain safety in the construction site/area in accordance with the provisions of the contract that includes the Occupational Safety and Health Administration (OSHA) Regulations; National Electrical Codes; National Fire Protection Association (NFPA) 70, National Electric Code; and NFPA 101, Life Safety Code. Work in a safe manner and take all proper precautions while performing your work. Extra precautions shall be taken when working around persons occupying the building during construction.

3. Provide Personal Protective Equipment (PPE) for your employees.

4. Post appropriate signs in specific hazardous areas.

5. Keep tools, ladders, materials, etc., secure and away from patients to prevent injuries.

D. Safety Inspections: The professional Occupational Safety and Health staff at this facility will perform safety inspections of all contract operations. Written reports of unsafe practices or conditions will be reported to the COR and Contracting Officer for immediate attention and resolution.

E. Fire Alarms:

1. The fire alarm system connects all buildings at this facility, and is activated by various heat, duct, manual pull stations and smoke sensors. Manual pull stations are provided at each entrance. Please survey the area in which you are working to locate the manual pull stations.

2. In the event of a fire alarm sounding, you are to remain in your area, unless medical center personnel (Safety, Nursing or Engineering) instruct otherwise, or unless a fire situation is in your area, in which case you should immediately evacuate.

3. Any work involving the fire protection systems will require written permission to proceed from the COR. Do not tamper with or otherwise disturb any fire alarm system components without prior written permission. To do so without written permission will result in an adverse action.

F. Hazardous Materials:

1. Many of the operations you are scheduled to perform may involve the use of hazardous materials. Prior to locating hazardous materials on site, all Material Safety Data Sheets (MSDS) will be submitted through the COR for evaluation by the facility Safety Officer.

2. Storage of hazardous materials within buildings will be minimal with only enough on hand to perform daily work tasks. Flammable materials will either be removed from buildings at the end of the work shift or stored in approved flammable storage containers.

3. Care must be taken to ensure adequate ventilation to remove vapors of hazardous materials in use. Many of the patients being cared for in the facility are susceptible to environmental contaminants, even when odors seem minimal. You will isolate those areas where vapors are produced, and ventilate to the most extent possible to reduce the number of complaints.

G. Airborne Dust Control During Construction:

1. Generation of dust is of major concern within staff and especially in patient occupied buildings. Where operations involve the generation of dust, all efforts will be directed at reducing airborne generated dust to the lowest level feasible.

This may be accomplished by a number of methods. These include misting the area with water, or use of tools attached to high efficiency particulate air (HEPA) filtering vacuums. Where large amounts of materials may be disturbed, resulting in airborne dust, establishment of full ceiling-to-floor plastic barriers may be required.

2. Classification of Jobs: see Attachment A

H. Infection Control Procedures: see Attachment A

1. Implement the requirements of VAMC’s Infection Control Risk Assessment (ICRA) team. ICRA Group may monitor dust in the vicinity of the construction work and require the Contractor to take corrective action immediately if the safe levels are exceeded.

2. Establish and maintain a dust control program as part of the contractor’s infection preventive measures in accordance with the guidelines provided by ICRA Group. Prior to start of work, prepare a plan detailing project-specific dust protection measures, including periodic status reports, and submit to COR for review for compliance with contract requirements in accordance with Section 01 33 23, SAMPLES AND SHOP DRAWINGS. All personnel involved in the construction or

01 00 10 - 3 renovation activity shall be educated and trained in infection prevention measures established by the medical center.

3. Medical center Infection Control personnel shall monitor for airborne disease (e.g. aspergillosis) as appropriate during construction.
