PRE-PROPOSAL CONFERENCE-

Project: Replace OR 5 Equipment, 589A4-17-113

a. Introduction:

· Welcome to the Harry S Truman VAMC and the pre-proposal conference.

· I would appreciate minimal side-bar conversations as the purpose of this conference is to ensure that everyone has the same information about the procurement.

Project Title/Project NO.: Replace OR 5 Equipment, 589A4-17-113

· Solicitation No: 36C25518R0411

· Location: Columbia VAMC.

· This meeting will discuss contractual issues, project requirements; entertain questions and a walk-thru of the project area. We will reconvene in this room after the walk-thru to entertain any further questions and allow you time to network with subcontractors that may be here.

· Before we begin, let me introduce myself, other VA staff and A&E staff.

· My name is Jeanette Mathena and I am a Contracting Officer at NCO 15. I am the Contracting Officer assigned to this procurement. I am the point of contact for questions.

· Ryan Brown will serve as the Contracting Officer’s Representative (COR) and is responsible for the day-to-day technical oversight of this project. He is not here to be Quality Control – Quality Control is the prime contractor’s responsibility.

b. Contractual /Administrative issues I’d like to cover are:

· Please ensure that you have signed the attendance roster – this will be published along with any applicable amendment to the solicitation on www.fbo.gov.

· You are reminded that this procurement is 100% set-aside for Service Disabled Veteran-Owned Small Businesses only and firms that do not meet this criteria will have their proposals rejected.

· The North Atlantic Industry Classification System (NAICS) code is 236220 and the code must be included on your SAM Registration as well as your VIP registration (www.va.gov/osdbu). Make sure your company is certified in VIP. Ensure that your company’s SAM Records / Representations and Certifications Application are active – they are usually good for one year only.

· Make sure you are current with your Vets 4212 reporting.

· Cost Range: $500,000.00 to $1,000,000.00

· PERIOD OF PERFORMANCE: Period of Performance 180 calendar days.

· The prime contractor (or employees of other service-disabled veteran-owned small business concerns) is required to perform 15% of the labor for contract performance.

· The facilities will be in operation during the construction requiring that you keep all areas clean of debris and/or obstructions.

· Proposals are due by 2:00 p.m. CDT on June 15, 2018 and need to be submitted in person or by mail, fedex, UPS, etc. All proposals are required to have a 120-day acceptance period.

· Fax or electronically sent bids are NOT acceptable.

· Please pay close attention to the bid items starting on page 2.

The selection process for this procurement is Best Value Lowest Price Technically Acceptable (LPTA) there are 5 Technical factors for this project so please ensure that you comply with the Solicitation Instructions to Offerors listed in the solicitation.

2.Technical Construction Experience;
3.Technical Qualifications of Proposed Key Staffing;
4.Technical Construction Experience of the Structural Steel (Fabricator and Erector) subcontractor;
5.Contractor Safety Plan;

You will be considered nonresponsive if required items are not submitted with your initial proposal. The government plans to award without discussions but retains the right to open discussions, if deemed necessary.

· Please Pay close proposal submittal instructions in the solicitation. The Government plans to award without discussions.

c. Bonds

· A bid guarantee in the amount of 20% of the bid price is required and must be submitted with your proposal submission.

· After award of the contract a Payment Bond and a Performance Bond in the amount of 100% of the contract value are due prior to a Notice to Proceed Letter being issued.

d. Other Contracting Topics:

· The purchase of material for this project is exempt from state sales tax. A Missouri Tax exempt certificate will be issues after contract award.

· The contractor is expected to maintain insurance coverage during the contract period as directed in the specifications.

· Progress payments will be made no later than 14 days after receipt of an acceptable invoice.

· Please pay particular attention to specification section 01 33 23 covering the submittal process. You will not be able to install materials without a submittal approval from the Government. Submittal Exchange has been paid for by the VA so this is not a cost to the contractor to provide.

· There are 3 important submittals that we need at contract start-up. They are: Memo providing contact information for Superintendent and Project Manager to include emergency contact information, providing Schedule of Values, and a Construction Schedule.

· The project involves work that is to be performed in strict accordance with the specifications, drawings that were posted to fbo.gov.

e. Questions

· If questions technical in nature surface, they need to be put in writing and submitted for answering at the end of this site visit or by e-mailing to Jeanette Mathena at: jeanette.mathena@va.gov. Do not contact the A&E or the COR and ask questions.

· Please make sure that all questions submitted by your company contain contact information: company name, phone number where you can be reached in case a clarification is necessary and your email address. All questions are to be submitted to Jeanette Mathena no later than end of the day on June 4, 2018 at 4:00 PM CDT. If you have more than one question, please number them 1, 2, 3 on the card or in your e-mail. Otherwise, you may not get an answer to each of them. An amendment will be issued to provide the answers to questions as well as to provide a copy of the attendance roster. If any questions warrant an amendment, one will be issued and posted to the fbo.gov. All interested contractors are responsible to check the fbo.gov website prior to submitting a proposal to ensure that they have seen and acknowledged all amendments.

f. Disclaimer Statements:

· Remarks and explanations at the conference shall not qualify the terms of the solicitation

g. Technical: I will now turn this conference over to the COR Ryan Brown and the A&E to discuss any project requirements they wish to point out before starting on the walk -thru. Please ensure that you stay together as a group so that information can be presented only once. Again, thank you for your attendance and your interest in this procurement. After the walk thru we will come back to this room to address any further questions and allow you to network with each other. Please do not leave the group before returning here.

ANSWERS TO QUESTIONS RECEIVED

1. What is the construction schedule?

The overall contract schedule will be 180 days. This includes all contract close out activities. The actual construction schedule where the room will be out of service will be no more than 10 weeks. This includes the de-installation of the old equipment all modifications and re-installation of the new equipment. This includes the equipment provided by the Government as well as the equipment provided by the Contractor.

1. What equipment is provided by the contractor?

Drawings AS 600 through AS 609 identify what equipment is provided by who. In general the Philips equipment, including the Maquet table, is provided by the government. The Stryker utility booms and lights are to be provided by the contractor. The Philips drawings indicate specific requirements that are to be provided by “Customer/Contractor,” these requirements are the responsibility of the contractor to provide and coordinate.

1. Who will provide the scrubs or bunny suits and do they have to be new after each use?

The contractor will be responsible for providing and ensuring all personnel are wearing proper OR coverings while entering and leaving the jobsite. The coverings must be donned prior to entering the outer core and removed once in the construction site vestibule. They cannot be worn while completing work in the construction site. The coverings shall include, full coveralls zipped up to cover all clothing, shoe covers, hair net, and facial hair cover. The shoe covers shall be replaced on each trip. The other items can be worn as long as they are not contaminated but cannot be used more than one day.

1. Will work be allowed during the day?

Only work that does not generate noise will be allowed during normal hours. There are active OR rooms on both sides of the construction site that will remain active throughout the construction. No activities that disrupt the active ORs will be permitted. The vast majority of the work on this project will have to take place during other than normal hours.

1. Will there be access to the room prior to the start of construction activities?

Once the project is awarded the VA will arrange for times after normal business hours for the contractor to access above ceiling to make final above ceiling measurements in order to coordinate all trades. The existing lights are removable for use as access panels. The contractor will be responsible for providing electricians to temporarily remove the lights to provide access. All activities must be completed after hours and the room must be fully restored prior to start of OR activities.

1. What are the lead shielding requirements?

The existing room is fully lead shielded with 1.5mm lead from floor to 7’-6” AFF. Any work that impacts the continuous integrity of this shielding will have to be done so as to ensure after the work is complete the continuous integrity remains. The government will test for shielding leaks after the contractor is complete. Any leaks resulting from the contractor’s work will be the responsibility of the contractor to correct.

1. Who is responsible for third party steel inspection?

The contractor is required to provide a third party steel inspector. Steel should be inspected as required by structural drawings and specifications.

1. What should be done with the equipment that is removed from the existing OR room by the general contractor?

The VA has first right to the equipment. If the government determines we do not want the equipment it will be the responsibility of the contractor to properly remove and dispose of the equipment.

1. Who will provide the dumpster?

The VA will provide the dumpster located in the South East gravel parking lot. If the contractor wishes to locate a dumpster at another location for their convenience it will be the responsibility of the contractor to pay for the dumpster. If the contractor chooses to locate a dumpster at an alternative location that location must be coordinated with the COR. If during the time the dumpster is located at an alternative location it becomes necessary to move or remove a dumpster to facilitate medical center activities it will be the responsibility of the contractor to arrange for the relocation of the dumpster at no additional cost to the government.

1. What work will have to be done below the operating room and where is it located?

There is minimal work that will be required to pull cables to through the junction box located below the operating table. This work will be in the sterile processing department and will have to be done from a HEPA cart.

Question 11. If the contractor chooses to take the bidders option to create a jobsite entrance point along the North side of the building by temporarily removing windows what requirements must be met. What will the access be to the access point be from the exterior of the building.

If the contractor chooses to pursue this option they will need to provide a weather tight temporary entrance. They will need to construct an OSHA compliant platform to access the opening from both the interior and exterior of the building. The entrance will have to be securable. This will include preventing access onto the temporary platform or ladder that the contractor decides to construct. After the work is complete the structure and landscaping both will have to be restored to their original state. The contractor will be able to access the site from the gate located to the Northeast of the jobsite. The contractor will be given a card to un-lock the gate. They will be able to stage material and vehicles in the parking spots adjacent to the gate starting at 5:30pm each evening. These spaces must be vacated and available for normal use by 5:30 am each morning.

1. Will the manufacture of the equipment (Philips) be taking care of their own infection control measures?

No, it is the requirement of the General Construction contractor.

1. Please confirm the Submittal Exchange requirement is already paid for, or is its cost to be part of the contractors bid?

Submittal Exchange has been paid for by the Government already no cost will be paid for by the construction contractor.
